IgA nephropathy [20].
Astudy in Nigeria described renal disease
(proteinuria or abnormal serum creatinine) in
38% of 400 patients. Of the 10 patients
biopsied, the majority had collapsing focal
segmental glomerulosclerosis (FSGS) [21].
In Cote D’Ivoire a study compared HIV-
infected patients in Paris with patients in
Abidjan and the prevalence of albuminuria
was higher in the cohort from Abidjan (26%
versus 5%, respectively) [22]. A Tanzanian
study found albuminuria to be more frequent
in an HIV-infected cohort, when compared
with a non-HIV-infected cohort (28.4% vs.

En 1984 se registran los primero casos de HIVAN en Nueva York y Miami. Inicialmente llamada Nefropatía asociada a SIDA. Pero su nombre fue cambiado a HIVAN al documentarse caracteristicas clinicas e histopatologicas en pacientes asintomaticos.
1999 : 3era causa de ERET en pacientes entre 20 y 64 años de raza afroamericana.
- El lumenes de los capilares glomerulares se encuentran globalemente obliterados por colapso de la membrana basal glomerular, con hipertrofia
-Depósitos de IgM, C3, menos común C1 en segmentos colapsantes
Estas enfermedades se encuentran caracterizadas por la presencia de complejos inmunes conformados por anticuerpos unidos antigenos del VIH en los capilares tubulares y en el mesangio. Puede haber activacion del complemento resultado en un cuadro clinico similar a una nefritis lupica sin presentar manifestaciones extrarenales. En contraste con HIVAN, en este grupo de patologias se puede documentar la ploriferación mesangial.
Otras condiciones reportadas en las biopsias de pacientes con HIV son: PTT, Nefropatia membranosa, glomerulonefritis membranoploriferativa (Asociada a hepatitis B o C)
- Estudios en modelos animales sugieren que la expresión renal de genes del VIH juegan un papel primordial en la patogenesis del HIVAN.
Aunque las celulas del epitelio renal no expresan tipicamente receptores clasicos de VIH y coreceptores, la pregunta de cómo entra el VIH a las celulas epiteliales permanece sin resolver.
Se ha mostrado que la infección linfocitica ocurre de manera mas eficiente cuando el virus es liberado directamente de un linfocito infectado a un linfocito no infectado comparado con la infección del virus libre. El mismo grupo ha demostrado in vitro que la infección de celulas del epitelio tubular por virus libre es extremadamente baja.
En eso orden de ideas, la transferencia del RNA viral y subsecuente expresión de genes de VIH es marcadamente incrementada en las cululas tubilares expuestas a linfocitos infectados.
The recent discovery that polymorphisms in apolipoprotein L1 (APOL1), located on chromosome 22, explain most of the genetic susceptibility of blacks to several forms of non-diabetic kidney disease including HIVAN, was a critical advance in our under- standing of how viral and host factors interact to predispose patients to kidney disease.

Los agentes antiretrovirales tienen potencial efecto nefrotoxico y el medicamento sobre el cual mas atención se le ha puesto es el tenofovir es un inhibidor de la transcriptasa inversa.
La toxicidad por tenofovir abarca un amplio espectro de sintomas y cambios en la funcion renal, incluyendo disfuncion proximal tubular, lesion renal aguda, enfermedad renal cronica, diabetes insipida nefrogenica.
La toxicidad tubular es la mas frecuente entre todas estas complicaciones, resultando en una deplecion del ADN mitocondrial que es mediado por la acumulación de drogas dentro del tubulo proximal renal.

Sin embargo, PFR en orina es un marcador inespecifico para este tipo de lesion, por lo cual no se puede ser concuclyente con este resultados.
Los corticoides para el tratamiento de HIVAN no han sido asociado a incremento de las infecciones oportunistas, pero aumentan de 4 a 7 veces el riesgo de necrosis avascular.
