Las células placentarias trofoblásticas poseen la habilidad de proliferar, invadir tejido
huésped, evadir la respuesta inmune e incluso metastatizar.
La enfermedad trofoblástica gestacional es definida como un grupo heterogéneo de
lesiones interrelacionadas las cuales surgen a partir del epitelio trofoblástico de la
placenta luego de una fertilización anormal.
2. Objetivo
Los/Lxs Estudiantxs, al final de esta clase podrán caracterizar la enfermedad
trofoblástica como alteración de la placentación, lo que genera un riesgo
materno y perinatal.
En condiciones normales, el tejido trofoblástico sano invade agresivamente el endometrio
y desarrolla una rica vasculatura uterina, lo que genera una íntima conexión entre el feto y
la madre, conocida como la placenta.
Sin embargo, en la enfermedad trofoblástica gestacional los
mecanismos de regulación fallan, lo que resulta en tumores altamente
invasivos, metastásicos y muy vascularizados

6. Conceptos Generales
El término Enfermedad Trofoblástica Gestacional (ETG) corresponde a un conjunto de lesiones
proliferativas originadas en la placenta, frecuentemente secundarias a un evento fecundante anormal
o aberrante.
Cada una de estas lesiones difiere en el grado, tipo y patrón de proliferación trofoblástica presente, así
como también en su comportamiento biológico, el cual puede ser desde un tipo benigno hasta uno
francamente maligno (con alto potencial invasor local y a distancia).
Debe tenerse en cuenta dentro del diagnóstico diferencial de la metrorragia del primer trimestre de
embarazo.
Desde un punto de vista anatomopatológico, las lesiones agrupadas bajo la denominación
Enfermedad Trofoblástica Gestacional (ETG) pueden clasificarse en tres grupos que se correlacionan
con su comportamiento biológico (OMS, modificada):

La Lesión Trofoblástica Maligna,
Neoplásica, corresponde a una lesión
francamente neoplásica o neoplasia
trofoblástica gestacional "genuina"; la
ETG maligna comúnmente se
diagnostica posterior a un embarazo
molar, pero puede ocurrir luego de
cualquier evento gestacional,
incluyendo embarazo de término,
embarazo ectópico y aborto. La Mola
Hidatidiforme Invasora (*), dado su
potencial invasor local y su capacidad
eventual de diseminación a distancia, es
ocasionalmente considerada por
algunos autores dentro de este último
grupo.

11. Enfermedad trofoblástica gestacional (ETG), su forma más representativa es la mola hidatidiforme.
Habitualmente descubierta a raíz de la evaluación ecográfica de una metrorragia de la primera mitad del
embarazo (habitualmente 1° trimestre). Su manejo incluye el vaciamiento uterino y el seguimiento
posterior con gonadotropina coriónica humana (β-HCG) hasta evidenciar la remisión mantenida de dicho
marcador.
Neoplasia trofoblástica gestacional (NTG) o
enfermedad trofoblástica gestacional persistente; sus
tipos más frecuentes son:
Neoplasia trofoblástica gestacional no
metastásica: Se puede generar a partir de
cualquier evento gestacional, molar o no molar
(embarazos de término, ectópicos o abortos). La
enfermedad está localizada en el útero.
Neoplasia trofoblástica gestacional metastásica:
Con fines terapéuticos y pronósticos, es de mayor
relevancia establecer la forma de presentación que
la histología definitiva (mola invasora o
coriocarcinoma gestacional, más frecuentemente).
Su pilar de manejo es la quimioterapia, ya sea con
monodroga o múltiples drogas según los factores
de riesgo, teniendo tasas de cura cercanas al 100%.

13. Diagnostico
La presentación clínica ha cambiado con la introducción del
ultrasonido y de la β-hCG, permitiendo un diagnóstico precoz e
impidiendo la aparición de síntomas floridos característicos de un
diagnóstico tardío.
Antiguamente el diagnóstico se basaba en la clínica (metrorragia del
primer trimestre, hiperémesis, toxemia, hipertiroidismo); y se
sospechaba el diagnóstico por la intensidad del sangrado, o la
expulsión de tejido con aspecto de vesículas.

Hoy día tiene mucha importancia para
definir el diagnóstico, el pronóstico,
respuesta a tratamiento y seguimiento.
En esta enfermedad, los valores
plasmáticos de β-hCG son mayores que
en un embarazo normal, a menudo
excediendo las 100.000 mUI/ml.
Frente al diagnóstico diferencial (con
síntomas de aborto o un embarazo
ectópico), el seguimiento de β-hCG
confirma la enfermedad trofoblástica
gestacional.

A las 12 semanas es el Pick máximo de la HCG en un
embarazo normal llegando a los variando entre los

22. Ultrasonido
Es un método confiable y sensible para el
diagnóstico.
En la mola completa se observa un patrón
ecográfico descrito como “panal de abeja”, en
“tormenta de nieve” o “queso suizo”,
(caracterizado por áreas sólidas e
hiperecogénicas con formas diversas
intercaladas con áreas líquidas de tamaño
variable) ocupando por completo la cavidad
uterina asociado a ausencia de embrión o feto.
En la mola parcial es posible ver un embrión o
feto y áreas vesiculares focales en el
trofoblasto. Además, en los anexos pueden
verse quistes teca-luteínicos como
consecuencia de los altos niveles de B-hCG.
23. Tratamiento
Si bien algunas formas de ETG pueden llegar a ser muy agresivas, esta enfermedad
ofrece la oportunidad única dentro de los cánceres ginecológicos y de los cánceres
en general de lograr altísimas tasas de curación y de conservación del potencial
reproductivo.
Esto debido principalmente (salvo raras excepciones), a su gran sensibilidad a la
quimioterapia.
24. Expresiones Clínicas de la TG
MOLA HIDATIDIFORME
Esta es la forma más frecuente de presentación de la ETG, representando
aproximadamente el 80% de estas.
Consiste en un embarazo generalmente intrauterino (excepcionalmente puede ser
extrauterino), en el cual las vellosidades coriales normales han sido reemplazadas por
múltiples estructuras quísticas que representan una dilatación vellositaria de grado
variable, asociada a áreas de proliferación trofoblástica (tanto cito como
sincitiotrofoblasto), también de magnitud variable.
El embrión y los anexos o membranas ovulares pueden estar presentes (mola parcial)
o ausentes (mola completa). Esta última presentación, Mola Completa con ausencia
de tejido embrionario, es el más sintomático, asociado a niveles más altos de B-hCG,
mayor riesgo de persistencia y por esto es el tipo de mola que se diagnostica con
