MINDACT is a large, multi-center, prospective, randomized, phase III clinical trial that screened >11,000 patients and enrolled 6693 patients from 2007 to 2011 in Europe. The study compared the ability of a genomic classifier (MammaPrint®) with that of clinical factors (determined by modified Adjuvant! Online) as a risk stratifier
“C” group risk defined by clinical factors with Adjuvant! Online (AOL)
“G” group risk defined by genomic profile with the MammaPrint 70-gene assay
Clinical low/Genomic low (MammaPrint low) risk patients (C-low/G-low; n=2745) received endocrine therapy alone while Clinical high/Genomic high (MammaPrint high) risk patients (C-high/G-high; n=1806) received chemotherapy followed by endocrine therapy. Discordant groups (in which AOL clinical risk and genomic risk estimates differ) were randomized to chemotherapy or no chemotherapy

Main Points:
This data set displays 5-year disease-free survival in the clinical high-risk/genomic low-risk (MammaPrint® low-risk) group according to the randomized treatment (chemotherapy versus no chemotherapy). This analysis was performed in the intent-to-treat population (ITT), including patients with discordant risk at the time of enrollment and who were analyzed according to treatment assignment
Among patients in the ITT at clinical high-risk and genomic low-risk groups at enrollment, those who underwent randomization on the basis of clinical risk (and therefore received chemotherapy or CT) had a 5-year rate of disease-free survival of 92.9% (95% CI, 90.5-974.7), whereas those who underwent randomization on the basis of genomic risk (and therefore received no CT) had a rate of 90.1% (95% CI, 87.5-92.1), ~3 percentage points lower than the rate among those who received CT. Hazard ratio for disease-free survival with CT versus no CT was 0.71 (0.50-1.01), indicating a 29% relative risk reduction that was very close to being statistically significant (P = 0.055)

The hazard ratio was 1.08, and 95% confidence intervals were 0.94 and 1.24, not crossing the prespecified non-inferiority margin of 1.322, with a P value of 0.26, indicating that the primary trial endpoint of non-inferiority was met
