The main four aspects to be dealt with are: some introductory information on atherothrombotic disease and its complications; the effects, current indications and unmet needs in antiplatelet therapy; and finally, some important clinical experience with triflusal.
La aterotrombosis representa el 22.3% de todas las causas de mortalidad mundial, según datos de la Organización Mundial de la Salud en 2001.
La repercusión de las manifestaciones de la aterotrombosis como la Cardiopatía Isquémica y la Enfermedad Cerebrovascular en la mortalidad global es alarmante. En los datos de OMS se puede observar que la Cardiopatía Isquémica es causa de muerte de más de 7 millones de personas cada año y el Ictus de casi de 6 millones.
Estos estudios muestran que un paciente con enfermedad coronaria también pueden tener enfermedad cerebrovascular y/o EAP.
El papel de las plaquetas en la aterotrombosis es un factor muy importante a considerar en la prevención de la muerte cardiovascular, IAM e ictus. A partir de la ruptura de la placa o del daño vascular, las plaquetas sufren 3 procesos: adhesión, activación y agregación, y el trombo se forma. Por tanto, un fármaco que inhibiera estos procesos plaquetarios sería claramente beneficioso y efectivo en la prevención de los eventos aterotrombóticos.

Thrombosis is the ultimate cause of cardiovascular events. Cardiovascular disease is a model to which the three components of Virchow’s triad apply: blood stasis, damage of endothelium, and a prothrombotic status. There is an interplay of pathophysiologic mechanisms that lead to atherothrombotic manifestations such as MI, stroke, and vascular death. These are the following: generalized disorders, lifestyle, systemic conditions, local vascular factors, inflammation, and genetic predisposition. Main risk factors leading to atherosclerosis are: hipertension, diabetes, hipercholesterolemia, smoking and obesity.
En esta diapositiva se compara la reducción de riesgo en diferentes subgrupos de alto riesgo y en el total, que demostró un beneficio considerable en todos los grupos de pacientes con una media del 22%. El odds ratio estratificado de un evento en los grupos de tratamiento frente al de los grupos control se estableció para cada grupo de estudios (rombo blanco) con un intervalo de confianza del 99% (linea horizontal). El grupo de alto riesgo ‘Otras’ (reducción total 13% ±7) incluye hemodiálisis (reducción 41% ±16), diabetes mellitus (reducción 7% ±8), enfermedad carotidea (reducción 19% ±22).
En el estudio CAPRIE, a pesar de que clopidogrel mostró un 8,7% de superioridad en eficacia vs el AAS en todas los pacientes, es importante considerar los subgrupos de ictus e IAM en los que clopidogrel no alcanzó significación estadística. De este modo, puede decirse que en la variable de reducción de riesgo coronario y cerebral, clopidogrel no es diferente del AAS. Definir q` es el RR ’: La posibilidad de padecer una enf. En el estudio CAPRIE, clopidogrel no demostró ser más seguro que el AAS. Significancia Estadistica de P 0.05)
La eficacia del AAS en la aterotrombosis ha sido ya demostrada en varios ensayos clínicos, sin embargo se sabe también que el AAS incluso a dosis bajas demostró complicaciones hemorrágicas significativas. En esta tabla adaptada del estudio BRAVO, un porcentaje significativo de pacientes experimentaron sangrado de moderado a severo con el AAS.
Están apareciendo artículos recientes referentes a la resistencia del AAS y clopidogrel. En este abstract de Martha Kerr presentado en el ACC 2005 Annual Scientific Sessions, se valoraron diferentes parámetros para documentar casos de resistencia al AAS y clopidogrel. Los resultados provienen de un estudio realizado en Baylor College of Medicine in Houston, Texas. Se observó que en el análisis realizado en 80 pacientes, el 15% fueron resistentes al AAS, 24% fueron resistentes al clopidogrel, y el 7,5% fueron resistentes a ambos. El mecanismo de resistencia no está completamente definido, pero hay otros factores específicos clínicos, celulares y genéticos que influyen en el fracaso terapéutico. Los pacientes con hipersensibilidad al AAS son frecuentemente tratados con clopidogrel solo, sin soporte científico. Aproximadamente el 10% de la población presenta hipersensibilidad al AAS, que se manifiesta como exacerbación asmática, rinorrea, angioedema, urticaria y anafilaxis.
Podemos identificar las necesidades no cubiertas del tratamiento antiagregante: Problemas de hemorragias Resistencia a la medicación Hipersensibilidad/Alergia Un fármaco que cubra estas necesidades del actual tratamiento antiagregante, mejorará sin duda el cumplimiento del tratamiento y en último término reducirá el riesgo cardiovascular.

Triflusal exerts its pharmacolgic effects through the following mechanisms: Inhibition of TxA2 production by platelets by inhibition of platelet cyclooxygenase (like aspirin) Inhibition of platelet cAMP phosphodiesterase, the enzyme that degrades cAMP (like dipyridamole) Activation of nitric oxide (NO) synthesis by neutrophils It is also important to note that compared with aspirin, triflusal has little inhibitory effect on prostacyclin synthesis inhibition.
Triflusal is absorbed fast and undergoes plasma hydrolysis giving rise to HTB, its main metabolite. Maximum plasma concentration is reached within 1-2 hours for triflusal and 5 hours for HTB. Half-life of triflusal is 30 minutes, while for HTB it is 35 hours. Main route of elimination is through the kidney. Triflusal’s initial activity can be seen as early as 2 hours after intake.
Approved indications for triflusal are secondary prevention after an acute atherothrombotic event (usually 600 mg od or 300 mg bid), prevention of coronary vein grafts (900 mg/day in divided doses), and prevention of embolic events in atrial fibrillation in association with moderate oral anticoagulation (see after). The clinical trials supporting the abovementioned indications will be reviewed next.
NF- B is a nuclear factor which plays a regulating role in the expression of numerous genes and in the transcription of several cytokines and the consequent expression of COX2 and other proinflammatory mediators: adhesion proteins VCAM-1 and E-selectin and some cytokines like the IL-1 (interleukin 1). These proinflammatory mediators express themselves in the endotelium and contribute to vascular inflammatory and atherogenic pathogenic mechanisms. At neuronal level these mechanisms also take place and can have their relevance in the evolution of the neurodegenerative processes and its prevention by drugs like triflusal and HTB that act at this level. Triflusal and HTB inhibit these proinflammatory processes in both territories due to their capacity to block the activation of the transcription factor NF- B.
Some studies are in pilot or planning phases. Revisar que ha pasado con estos estudios?
Pilot, double blind, placebo-compared study to assess the effect of triflusal (900 mg/día) in patients with amnestic mild cognitive impairment. The average period of follow-up was of 13 months and the main outcome was the evaluation the cognitive state of the patients through the scale of neurological evaluation ADAS-cog. Secondary end-point was the rate of conversion to dementia.
In this figure a favourable tendency in the group of patients treated with triflusal is observed. The curves of evaluation of the cognitive state by ADAS-cog scale are separated throughout the treatment, although the difference after 18 months (maximum follow-up) does not reach significance level.
The survival curves of the secondary end-point (rate of conversion to dementia) show a significant reduction by triflusal versus placebo superior to 40% (p=0,024; HR = 2,102; IC 95% 1,102-4,008). These results are highly promising and will require a later confirmation in a pivotal study.
To conclude, four important points might be stressed.
Triflusal has demonstrated an antiplatelet effect clinically relevant, similar to aspirin but with three main differences: preservation of PGI 2 synthesis; phosphodiesterase inhibition that increases anti-aggregatory effects; and nitric oxide release by neutrophils.
Clinical efficacy of triflusal has been demonstrated in many atherothrombotic scenarios: coronary vein grafts, unstable angina, acute myocardial infarction, acute stroke. The results of clinical trials consistently demonstrate as good similar protective effects as aspirin, but with significantly less bleeding complications.
A new indication of triflusal is the combination with moderate oral anticoagulation in high risk patients wtih atrial fibrillation. The better protective effect against thromboembolic complications is achieved without increasing haemorrhagic risk.
Although not previously shown, triflusal has been studied in patients intolerant to aspirin, with no complications. The promising horizon of preventing cognitive impairment needs not to be stressed.
