1. FRECUENCIA
• 7 episodios en 1 año, o 5 al año por 2 consecutivos, o 3 al año por 3 años consecutivos.
2.HALLAZGOS CLINICOS
• Fiebre >38°
• Adenopatia cervical sensible >2cm
• Exudado amigdalino
• Cultivo positivo para SBHGA
3. TRATAMIENTO
• Episodios tratados con antibiótico adecuado
4.DOCUMENTACION
• Episodios evaluados por un médico, quedando registrados en el expediente clínico o al
menos 2 observados por ORL.

La melatonina es una hormona que se encuentra influida por las variaciones medioambientales luz-oscuridad y que actúa como moduladora del sistema inmunitario. Se han encontrado concentraciones más elevadas de melatonina amigdalar en la hipertrofia amigdalar, y disminuidas en la amigdalitis recurrente. Se especula que los valores elevados de melatonina amigdalar favorecerían la hipertrofia amigdalar, y los disminuidos la amigdalitis recurrente11 .
El tratamiento antibiótico de la FAA estreptocócica está justificado e indicado, al haberse demostrado con su empleo una resolución más rápida de los síntomas, reducción del tiempo de contagio y transmisión del EbhGA en la familia y la escuela, y prevención de las complicaciones supurativas locales y no supurativas, aunque no existe evidencia definitiva en la protección frente a la GMNPE y el síndrome PANDAS1,2
Aunque la penicilina es considerada desde hace anos ˜ como el antibiótico de elección, varios estudios muestran su infrautilización. La amoxicilina fue el fármaco seleccionado en primer lugar (54,6%), seguido de amoxicilina-clavulánico (42.7%), macrólidos (12.6%) y cefalosporinas de segunda generación (5.6%)

Serum Antistreptolysin O (ASO) titre is raised when there is infection of any organ of the body, by Group A beta haemolytic streptococci (GABHS), Group C or Group G streptococci [
he main outcome measure is to find out whether only the raised serum ASO titre is to be considered to perform tonsillectomy or not. Fifty consecutive patients (both children and adults) clinically diagnosed to have chronic tonsillitis were included in this prospective cohort study. Throat swab culture, tonsilar core tissue culture and Serum ASO titre tests were performed in all the patients. The results showed that out of the 50 patients 45 had raised ASO titre and 5 patients had normal ASO titre. GABHS was found in 5 cases (10 %) in throat swab culture and 8 cases (16 %) in FNA of tonsil core culture. The sensitivity of throat swab culture as compared to FNA Tonsil core culture was 62.5 % and positive predictive value was 100 %. The sensitivity of ASO titre as compared to core culture was 100 % and positive predictive value was 17.8 %. Specificity was only 12 %. From the results of this study, it is evident that FNA culture of the tonsil core is a valid and a reliable test for the diagnosis of bacterial micro flora in recurrent tonsillitis. One should perform throat swab culture and FNA culture from tonsil core along with ASO titre before doing tonsillectomy in absence of any other indications.

During the investigation, fiberoptic endoscopy images of the choanal openings are divided into four segments from the upper choanal border to the nasal floor. GRADE I is scored when fiberoptic endoscopy imaging revealed adenoid tissue occupying only the upper segment of the nasopharyngeal cavity (≤ 25%) with almost completely free choanal openings. GRADE II is scored if the adenoid tissue is confined to the upper half (≤ 50%) of the nasopharyngeal cavity with sufficiently pervious choana and perfect visualisation of the tube ostium. GRADE III obstruction is given if adenoid tissue occupies about 75% of the nasopharynx with partial involvement of the tube ostium and considerable obstruction of choanal openings. In GRADE IV cases, the adenoid tissue reaches the lower choanal border without allowing visualisation of the tube ostium (> 75%).
n 881 patients (54.7%) belonging to the Grade III group, medical treatment was effective in controlling nasal obstruction and related pathologies and complications.
