Restructuración
• Restructuración de lo que antiguamente conocíamos como
espondiloartropatias seronegativas
• ¿Porque?
Se hizo necesario para un diagnóstico temprano de la
enfermedad, y evitar confusiones diagnosticas
• ¿Qué ventajas trajo?
Delimitó de una mejor manera el manejo terapéutico.
Rompió paradigmas clásicos de la enfermedad como por
ejemplo la predominancia del sexo masculino .
29. Caso clínico
• Varón de 43 años. No antecedentes
personales de interés. AHF: padre con
psoriasis.
• Inicia en 2009 clínica de artritis
manos, hombro derecho e
importante rigidez matutina. A la
exploración, artritis en muñecas.
Durante el curso de la enfermedad
presenta dactilitis en segundo dedo
de mano derecha.
• Además, lumbalgia inflamatoria de
más de 10 años de evolución
• TAC con sacroilitis.
• Talalgia, con entesitis del tendón de
Aquiles.
• Rx sacroilitis G2
• El paciente refiere síntomas
digestivos inespecíficos. Toma AINEs
con frecuencia.
• Laboratorios: FR negativo. HLA B 27
negativo. Persistencia de PCR elevada

39. Conclusión
• Los nuevos criterios de clasificación buscan dar un
diagnostico mas temprano, y el dominio de estos conceptos
a nivel de medicina general puede contribuir a un
diagnóstico mas temprano de la enfermedad
This is a cartoon summarizing the true story of a French soldier aged 19 in 1957 while participating within the French Army at the Algerian independence war.
In 1957, he suffered from a TRIADE (urethritis, arthritis, conjunctivitis) easily recognized as REACTIVE ARTHRITIS.
Thereafter, he suffered from different symtoms diagnosed as
“amicrobial uretritis”
“acute anterior uveitis without obvious aetiology”,
inflammatory back pain (not shown on the slide) which led, in 1970, to a diagnosis of ANKYLOSING SPONDYLITIS because sacroiliitis was observed at pelvic plain X-rays.
In 1974 (data not shown) he was one of the first B27 typed patients and was B27 positive.
Thereafter, he was diagnosed as PSORIATIC ARTHRITIS.
Finally, the main question is whether you consider this patient as suffering from 5 different disorders or a single one with different clinical presentations.
This latter hypothesis was recognized as the “concept of spondyloarthritis” by Wright and Moll in the 1960’.
This slide is very important. It has the objective to understand whether it is better to describe a patient by suffering from a specific disease (sub-group of SpA) (left part) or (and better) to describe a patient as suffering from SpA and thereafter describe the clinical manifestations.
We will see during the SPARKLE meeting that the “right part” description is much more relevant than the “left part” description.
