27. The results of the PROWESS SHOCK trial (1,696
patients) were released in late 2011, showing no benefit of
rhAPC in patients with septic shock (mortality 26.4 % for
188 rhAPC, 24.2 % placebo) with a relative risk of 1.09
and a P value of 0.31 [231]. The drug was withdrawn from
the market and is no longer available, negating any need
for an SSC recommendation regarding its use.

Con la actualización de la revisión no se encontraron
pruebas que sugieran que la PCA pueda utilizarse para el

The evidence supporting the clinical and economic attractiveness of
drotrecogin alfa is not conclusive and further research appears to
be indicated.
Cost-effectiveness of drotrecogin alfa is uncertain (< 59%
probability that incremental cost-effectiveness ratio (ICER) life year
gained (LYG) ≤ $50,000/LYG) when applied to all patients with
severe sepsis.
Simulation results indicate that where the NHS is
willing to pay £20,000 per QALY, drotrecogin al fa
(activated) is a cost-effective use of resources in
98.7% of cases.

29. Videolaryngoscopes offer an improved view compared with
conventional direct laryngoscopy and are now the first choice
or default device for some anaesthetists.
The Difficult Airway Society's 2015 guidelines recommend that
videolaryngoscopes should be immediately available at all
times and that all anaesthetists should be trained and skilled in
their use.
Authors’ conclusions
Videolaryngoscopes may reduce the number of failed
intubations, particularly among patients presenting with a
difficult airway. They improve the glottic view and may
reduce laryngeal/airway trauma. Currently, no evidence
indicates that use of a VLS reduces the number of
intubation attempts or the incidence of hypoxia or
respiratory complications, and no evidence indicates that
use of a VLS affects time required for intubation.
