18. Valor diagnóstico de la TVS vs SHG
RESULTADOS:
TVS : especificidad (95.7%) en detección de pólipos intrauterinos, buena
sensibilidad (85,7%) y especificidad (89.2%) en investigación de hiperplasia
endometrial y excelente VPN (92.2%) in el diagnóstico de mioma
submucosos. Exactitud diagnóstica para sinequias fue muy baja.
SHG: tuvo alta especificidad (92.8%) en la detección de pólipos intrauterinos
y alta sensibilidad (92.9%) y especificidad (96.8%)en el diagnóstica de
hiperplasia endometrial. En adición mostro alta sensibilidad (90%),
especificidad (99%), PPV (92.2%) y NPV (99%) para la detección de mioma
submucoso. Finalmente SHG mostró alto PPV (100%) y NPV (100%)
para valoración de sinequias.
CONCLUSION:
TVS debería ser usado como el primer paso para la investigación de patología
intrauterina. TVS podría reducir el número de histeroscopías diagnósticas
normales. Por otro lado SHG debería ser utilizado para el diagnóstico de
patologías y para decidir entre histeroscopía de oficina o con resectoscopio.

In postmenopausal breast cancer patients treated with tamoxifen, TVS alone is not sufficient for the detection of endometrial pathologies. Additional use of diagnostic hysteroscopy considerably improves the detection of polyps, thus significantly reducing the rate of false-negative findings of endometrial pathologies.
SHG has greater sensitivity, positive predictive value, and
accuracy than HSG has for detecting intrauterine lesions.
Although HSG will continue to be an important screening
tool in infertility for its proven ability to evaluate the
architecture and patency of the fallopian tubes, SHG is a
more reliable diagnostic tool for the evaluation of intrauterine
defects.
Our results have substantiated that sonohysterography is a better tool than transvaginal sonography for the
assessment of endometrial intra-cavity lesions. By providing accurate differentiation between focal and diffuse endometrial
lesions, it can help in decision making regarding selection of cases for hysteroscopy and directed biopsy. We recommend that
saline contrast sonohysterography should be used as an initial investigation in cases of abnormal uterine bleeding.

Diagnostic accuracy of sonohysterography and transvaginal sonography as compared with hysteroscopy and endometrial biopsy.
116 pacientes con infertilidad y/o HUA y/o sospecha de cavidad uterina patológicca.. Women consecutively underwent during the same day, to TVS, SHG and Hys plus EB by three different operators.
RESULTS:
TVS shows excellent specificity (95.7%) in uterine polyps detection, good sensitivity (85,7%) and specificity (89.2%) in investigating endometrial hyperplasia, and excellent NPV (92.2%) in the diagnosis of submucous myomas. Diagnostic accuracy of TVS for synechiae is not evaluable. SHG demonstrates high specificity (92.8%) in the detection of uterine polyps, and high sensitivity (92.9%) and specificity (96.8%) in the diagnosis of endometrial hyperplasia. In addition it shows high sensitivity (90%), specificity (99%), PPV (92.2%), and NPV (99%) for detection of submucous myomas. Finally, SHG shows high PPV (100%) and NPV (100%) for synechiae assessment.
CONCLUSION:
TVS could be used as first step investigation to exclude uterine pathologies. TVS could reduce the number of diagnostic Hys normally performed in women with normal uterine cavity. Furthermore SHG should be useful to diagnose the pathologies and to decide between operative Hys in-office or resectoscopic treatment.

En un estuidio que involucró más de 5000 pacientes; se evidencio diferencia significativa en los hallazgos evidenciados entre la ecografía en pacientes asitomaticas con endometrio menor de 4 mm y la histeroscopìa.
CONCLUSION: AUN EN ASINTOMATICAS CON ECOGRAFIA TV NORMAL, LA HISTEROSCOPIA DE RUTINA EVIDENCIA PATOLOGIA INCLUSO CANCERIGENA.

The aim of this study was to determine if hysteroscopic examination of uterine cavity and consequent treatment of intrauterine lesions prior to IVF could improve the pregnancy rate in women under 38. METHODS: This study included 480 patients under 38, who had undergone IVF or IVF\ICSI--embryo transfer cycles, in which one or more good quality embryos were transferred. By transvaginal sonography performed within the past 2 months, the uterus was found normal in all the patients enrolled in our IVF unit. The patients were divided into three groups: group A--with no hysteroscopic evaluation and no pathology, group B --with hysteroscopy but no pathology, and group C--with abnormal hysteroscopy finding and corresponding treatment. RESULTS: The obtained results revaled no difference in the mean age, duration of infertility, number of mature oocytes in either group (p > 0.05). Clinical pregnancy rates in the groups A, B and C were 36.9%, 58.75% and 32.7%, respectively, and delivery rates were 27.5%, 48.7% and 25.7%, respectively. There was a statistically significant difference among the groups concerning pregnancy and delivery rates. CONCLUSION: Considering the results of this study we could conclude that hysteroscopy, as a routine examination, should be performed before the first IVF-ET cycle in all patients thereby reducing the failures and then the costs of IVF-ET.

BACKGROUND: Many couples fail to achieve pregnancy instead repeated FIV-TE-ICSI cycles. Good quality embryos for successful pregnancy rates should not be count apart over endometrial receptivity. OBJECTIVE: To assess the impact of office hysteroscopy in a group of patients with two or more previous failed cycles of FIV-TE-ICSI in an assisted reproduction center. MATERIAL AND METHOD: Retrospective case control study in patients with problems to conceive in two or more previous failed FIV-TE-ICSI cycles instead good quality embryos and an apparently normal uterine cavity. They were divided in two groups: A with no hysteroscopy perfomed, and 8 with hysteroscopy. Second group was divided in 81 (without pathologic findings) and 82 (with intrauterine abnormalities). Demographic variables were analized, hysteroscopic findings, good quality embryos transferred, biochemical and clinical pregnancies and perinatal results in all groups. RESULTS: No significant differences were found in demographic variables analyzed among patient with or without office hysteroscopy. Intrauterine abnormalities were found in 16 (64%) patients of group 82. No significant statistical differences were found respect biochemical, clinical pregnancies or live birth new born. CONCLUSIONS: Instead previous diagnosis of an apparently normal uterine cavity, pathologic abnormalities were found in a significant number of patients. We observed an improvement in pregnancy rates in patients in with HC was realized previous to FIV-TE-ICSI, particularly on those were endometrial pathology was found and corrected, even though the study sample did not allowed to reach results with statistical difference.
