La paciente de 68 años fue ingresada para una cistolitoplaxia debido a litiasis vesical. Presenta antecedentes de cuadriplejia desde 1999 por lesión medular. 6. IPAYS
• Refiere resequedad en los ojos en control por oftalmo
• Niega asma o tos frecuente, refiere dificultad para chiflar o cantar
• Refiere estreñimiento crónico con necesidad de desimpactarla diariamente y gastritis

8. EXPLORACIÓN FÍSICA
• TA 150/90 FC 72 Sat 93%
• Peso 60kg
• Alerta, Glasgow 15

• Cuello con collarín rígido por 15 años, flácido, sin movilidad permitida por su medico tratante
• Tórax simétrico con muy pobre ventilación no estertores ni sibilancias
• Abdomen blando con cistoclisis
• Genital diferido
• Extremidades totalmente atróficas y flácidas excepto brazo izq.
• Refiere perdida de la sensibilidad en hemicuerpo derecho
• Y perdida de sensación térmica únicamente en el hemicuerpo izq.
13. INTUBACION
• Se realiza laringoscopia con hoja MAC 4, encontrando un cormac 4,
• Después de dos intentos fallidos, utilizando celic y guía, no es posible hiperexterder el
cuello por su patologia de base, se decide colocar mascarilla laringea no. 3.5 sin
complicaciones

14. PROCEDIMIENTO QUIRURGICO
• Se realiza cistoscopia vía cistostomia no encontrando litos en vejiga por lo que el
procedimiento no dura mas de 10 minutos
• DX POSTQUIRURGICO: Coprolitos
15. EXTUBACION
• Se decide esperar a la eliminación pasiva de los fármacos
• Una hora y media después se decide revertir con
• Nalmefeno
• Neostigmina 1.5 mg previa atropina
• Obteniendo ventilación autónoma por parte de la paciente
• Por lo que se decide retirar la mascarilla laríngea, descomprimir estomago con sonda
orogástrica y dar apoyo ventilatorio con mascarilla
• La paciente por si sola mantiene saturaciones aunque continua dormida
• Se decide su ingreso a sala de recuperación con monitor y mascarilla venturi.

• SE REALIZA INTUBACION CON FIBROSCOPIO Y REPONER BICARBNATO 4
FRASCOS DE 20ML AL 9.5%
• SE TOMAN GASOMETRIAS POSTERIORES Y
• SE ENTREGA EN TERAPIA INTERMEDIA INTUBADA
21. • Se da seguimiento a la paciente quien por la noche consiente intubada comunicándose
por un alfabeto
• Aproximadamente 36hrs después se extuba sin complicaciones por parte de servicio
tratante
• 48 horas después Alerta platicando con Sat 85% al aire ambiente y FC 87 alta a su casa
al día siguiente
22. CONSIDERACIONES
• ANESTESIA REGIONAL
• Se recomienda tomando en cuenta
• Dificultad para conocer el nivel del bloqueo
• Necesidad de ventilar al paciente
• ANESTESIA GENERAL
• Respuesta a los RMND
• Aumento del K con los RMD
23. • NO SE CONTRAINIDA LA INTUBACION
• La utilización de curares expone a un mayor riesgo de movilización del raquis cervical por
supresión de la contracción tónica de los músculos del cuello
• SE CONTRAINDICA LA SUCCINILCOLINA SOBRE TODO EL 1ER AÑO
• Es recomendable medir la presión inspiratoria máxima antes y después de la instalación
del paciente para verificar la ausencia de compresión torácica.
