10. Cetoacidosis diabética/diabetic ketoacidosis
• NaCl bolo 1L, luego 500ml/hr
• Insulina cristalina bolo 0,1U/Kg luego infusión continua a
0,1U/K/hr
• Glucemia horaria
• Si glucemia no baja 10% en 1 hora, bolo insulina cristalina
0,14U/K
• Si baja <200mg/dl reducir infusión a 0,02-0,05U/K/h y
añadir D5%
• Mantener glucemias entre 150 y 200mg/dl
• Transición de insulina EV a SC debe hacerse 1-2h posterior
a inicio de SC.
• Protocolo insulina SC…¿?
• NaCl bolus 1L, then 500ml/hr
• Crystalline Insulin bolus 0,1U/Kg then infusion 0,1U/K/hr
• Blood glucose each hour
• If glycaemia no does not fall by 10% in 1 hour, bolus of
regular insulin 0,14U/K
• If falls<200mg/dl reduce infusion to 0,02-0,05U/K/h and
add D5%
• Blood glucose levels goals 150 y 200mg/dl
• Transition of EV to SC must be 1-2h posterior to SC first
dose.

18. Recomendaciones actuales/Actual recommendations
•El uso de esquema Basal-Bolo es superior al esquema deslizante.
•Basal-Bolus is superior to sliding scale
•El esquema deslizante tiene mayor mortalidad e hiperglucemias a compararlo con otros
esquemas
•Sliding scale have more mortality and hyperglycemias than other scales
•NPH y Glargina tienen misma efectividad en disminuir glucemia e igual seguridad.
•NPH and Glargine have same effect on lowering blood glucose and have same security
•Se recomienda inicio con esquema Basal-Bolo en pacientes diabéticos tipo 2
hospitalizados
•Recommends start with Basal-Bolus scale in Diabetic type 2 inpatients
•Si la glucemia se mantiene >180mg/dl, pasar a insulina en infusión.
•Is glycaemia persist >180mg/dl, start infusion protocol.
