86. Results. – Among AR reported, 25% were serious, including 368 deaths.
The 284 transfusion-transmitted infections included 187 bacterial infections, 84 viral and 13 parasitic or fungal; nine
deaths resulted.
AR related to the respiratory system Transfusion-Associated Circulatory Overload (TACO),
Transfusion-Related Acute Lung Injury (TRALI) and Transfusion-Associated Dyspnoea (TAD)
accounted for 8.3% of all AR, 20.1% of serious, and 52.2% of deaths.
Of 1634 rare AR, 1565 were hypotensive, 38 PTP, 17 GvHD, 9 hyperkalemia and 5 hypercalcemia. Half were serious and
16 fatalities were recorded (13 hypotensive, 2 GvHD, one PTP). Among 14 countries that reported any hypotensive AR,
incidences diverged widely.

97. Mortality and relapse associated with perioperative transfusion due to cancer surgery were evaluated
among participants (n = 123 studies) total of 184,190 subjects (range 35–29,899).
Overall, RBC transfusions were associated with an increased risk of death [HR = 1.50
(95% CI 1.42–1.57), p < 0.01] and relapse [HR = 1.36 (95% CI 1.26–1.46), p < 0.01].
The survival was reduced even in cancer at early stages [HR = 1.45 (1.36–1.55), p < 0.01].
In cancer patients undergoing surgery, red blood cell transfusions reduced the survival
and increased the risk of relapse.

Es necesario reducir las transfusiones innecesarias. Esto puede lograrse mediante el uso
clínico adecuado de la sangre, evitando la necesidad de transfusión, y con el uso de
alternativas a la transfusión.
El compromiso de las autoridades de salud, los proveedores de atención médica y los
médicos es importante en la prevención, el diagnóstico temprano y el tratamiento de
enfermedades/condiciones que podrían llevar a la necesidad de una transfusión de sangre.
La transfusión de sangre es una parte esencial de la atención médica moderna. Utilizado correctamente,
puede salvar vidas y mejorar la salud. Sin embargo, como toda intervención terapéutica, puede dar lugar a
complicaciones agudas o tardías y conlleva el riesgo de transmisión de agentes infecciosos.

Global Nutrition Targets for 2025 (1) and of the Sustainable

Statement 1 People with iron-deficiency anaemia who are having surgery are offered iron
supplementation before and after surgery.
Statement 2 Adults who are having surgery and expected to have moderate blood loss
are offered tranexamic acid.
Statement 3 People are clinically reassessed and have their haemoglobin levels checked
after each unit of red blood cells they receive, unless they are bleeding or are on a chronic
transfusion programme.
Statement 4 People who may need or who have had a transfusion are given verbal and
written information about blood transfusion.

184. El GERM o Grupo Español Rehabilitacion Multimodal se fundó en 2008 con el objetivo de implantar
protocolos Fast Track o de rehabilitación precoz en España y valorar todos los aspectos derivados de su
implantación, incluyendo su grado de cumplimiento y de beneficio para el paciente.
La rehabilitación precoz o multimodal busca la disminución del dolor del paciente así
como la minimización de su estancia hospitalaria optimizando una serie de parámetros
no solo propios de la cirugía sino también peri- y post-operatorios

201. 54,922 consecutive surgical admissions, 1,997 received an RBC transfusion, with 1,125 considered inappropriate (56%).
The adjusted cost of each inappropriate RBC transfusions was estimated in USD 9,779 (95% CI, 9,358 – 10,199; p < 0.001) and
totaled USD 11,001,410 in our series. Inappropriately transfused patients stayed 1.6 times (95% CI, 1.5–1.6; p < 0.001) longer
in hospital (10.6 days vs. 6.7 days) than non-transfused patients and a mean 2.35 RBCunits per patient were administered.
Conclusion: Inappropriate RBC transfusions in elective surgical patients seem to be common and may represent a
significant economic burden. In our experience, inappropriate transfusions significantly increased hospital costs by
an average of USD 9,779 compared to non-transfused patients. Through specific PBM policy, hospitals may
improve cost-effectiveness of their elective surgical activity by lowering inappropriate transfusions.

211. Studies have shown that the vast majority of transfusions (as many as 94%) in surgical patients can be
attributed to low preoperative hemoglobin levels, excessive surgical blood loss, and/or inappropriate transfusion
practices, in consideration of which patient blood management relies on 3 pillars: (1) optimizing hematopoiesis,
(2) minimizing bleeding and blood loss, and (3) harnessing and optimizing physiological tolerance of anemia.

213. PBM es en este siglo el nuevo paradigma de los antiguos programas de ahorro de sangre. El PBM surge para aglutinar todas
estas estrategias encaminadas, desde un punto de vista multidisciplinar, a la optimización de los resultados médicos y
quirúrgicos de los pacientes en la gestión clínica de su sangre, constituyéndose en la actualidad como el modelo integral
recomendado.
El objetivo no es no transfundir, sino optimizar de forma individualizada las condiciones de nuestros pacientes,
médicos y quirúrgicos, para mejorar sus resultados clínicos, a través de optimizar y conservar su masa
eritrocitaria.

224. This policy brief aims to:
- CREATE AWARENESS about the enormous, but greatly under-appreciated global disease burden of iron
deficiency, anaemia, blood loss and bleeding disorders;
- CREATE A SENSE OF URGENCY for health care entities to implement PBM, a systematic,
multidisciplinary, multiprofessional concept to routinely minimize these risk factors, and, in
so doing, significantly and cost-effectively improve health and clinical outcomes for
hundreds of millions of medical and surgical patients, pregnant women, neonates,
children, adolescents, elderly people, and the population as a whole;
- ANNOUNCE the upcoming World Health Organization (WHO) initiative to develop PBM Implementation
Guidelines that will serve as a framework for health care leaders of all Member States;
- ALERT HEALTH MINISTRIES, SOCIAL SECURITY SERVICES, HEALTH DEPARTMENTS AND POLICY-MAKERS about
this global initiative and call on them to prepare for and foster the rapid dissemination and implementation of
PBM in their jurisdiction;
- COORDINATE THESE EFFORTS with existing initiatives pertaining to improved patient-centred care, patient
safety and quality of care, including maternal, prenatal and child care, and nutritional supplementation
programmes;
- act as an ACCELERANT FOR CHANGE by educating the readers about what PBM is and is not, why PBM
implementation is critical, and calling attention to the barriers to implementation.
