MOTIVOS PARA REDUCIR LA TRANSFUSIÓN
30. TRANSFUSIÓN DEL PACIENTE ONCOLÓGICO
The findings of the 1998 meta-analysis were confirmed, with small variations in some
estimates. Identification of 237 references. Thirty-six studies on 12,127 patients were
included: 23 showed a detrimental effect of PBT; 22 used also multivariable analyses,
and 14 found PBT to be an independent prognostic factor.
Pooled estimates of PBT effect on colorectal cancer recurrence yielded overall
OR of 1.42 (95% CI, 1.20 to 1.67) against transfused patients in randomized
controlled studies.
Stratified meta-analyses confirmed these findings, also when stratifying patients by site
and stage of disease. The PBT effect was observed regardless of timing, type, and in a
dose-related fashion, although heterogeneity was detected. Data on surgical techniques
was not available for further analysis.

transfusionales graves e infecciones transmitidas.
El uso innecesario reduce gravemente la disponibilidad de
productos sanguíneos para los pacientes que los necesitan.

It is necessary to reduce the unnecessary transfusions. This can be
achieved through the appropriate clinical use of blood, avoiding
the needs for transfusion and use of alternatives to transfusion.
The commitment of the health authorities, health care providers
and clinicians are important in prevention, early diagnosis and
treatment of diseases/conditions that could lead to the need for
blood transfusion.
http://www.who.int/bloodsafety/clinical_use/en/
Blood transfusion is an essential part of modern health care. Used correctly, it
can save life and improve health. However, as with any therapeutic intervention,
it may result in acute or delayed complications and carries the risk of
transmission of infectious agents.

71. First Recommendation
The AABB recommends a restrictive RBC transfusion threshold
in which the transfusion is not indicated until the Hb level is
7g/dL for hospitalized adult patients who are hemodynamically
stable, including critically ill patients, rather than a liberal
threshold when the Hb level is 10g/dL (strong recommendation,
moderate quality evidence).
For patients undergoing orthopedic surgery or cardiac surgery
and those with preexisting cardiovascular disease, the AABB
recommends a restrictive RBC transfusion threshold (Hb level
of 8g/dL) (strong recommendation, moderate quality evidence).
The restrictive Hb transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, but
RCT evidence is not available for all patient categories.

"Whenever clinically feasible, patients undergoing elective surgery
with a high risk of severe postoperative anaemia should have their
haemoglobin level and iron status tested, preferably at least 28
days before the surgical procedure. For patients >60 yr old,

129. The goal of minimizing transfusions to limit patient exposure to
adverse reactions is reasonable and practical.
To accomplish this, PATIENT BLOOD MANAGEMENT strategies
are being widely implemented and have been successful in
reducing blood transfusions in tertiary care centers.
The principles of PATIENT BLOOD MANAGEMENT are based on
the three pillars concept: optimizing preoperative
erythropoiesis, reducing operative blood loss, and harnessing
the patient’s physiological tolerance of anemia.
Anesthesiologists should recognize this changing landscape as
we are often on the front lines of blood transfusion.
Anesthesiology 2015; 122: 1-2
