Cada 20 mmHG de aumento de la PAS duplica el riesgo de

Solo 1/3 de los hipertensos está bien controlado.

11. BASES FISIOPATOLOGICAS PARA LA D.R.
(Experiencia en animales)
• En animales nefrectomizados a quienes se les reimplantaron sus
riñones (por tanto denervados) se pudo comprobar un significativo
aumento de la natriuresis1.
• En ratas con Hipertensión Arterial esencial la presión se volvió más
controlable luego de la denervación2.
• La estimulación crónica de los nervios simpáticos renales de perros
normotensos logró causar hipertensión, sin demasiado cambio a
nivel del flujo renal, sugiriendo un efecto mediado por el aumento en
la actividad simpática3.

(Observaciones en humanos)
• Los enfermos renales crónicos tienen un aumento de la actividad simpática
pero esta hiperactividad mejora luego de la nefrectomía1.
• En los enfermos trasplantados a los que no se les extirpa sus propios
riñones, esta hiperactividad simpática continúa2.
• Los trasplantados que conservan sus riñones, los beta-bloqueantes son
muy efectivos para el manejo de la Hipertensión Arterial, mientras que en los
nefrectomizados carecen de utilidad3.
• Los ARA II e IECA tienen una modesta utilidad para el control de la presión
arterial en las etapas avanzadas de enfermedad renal, mientras que los
agentes simpatico-líticos centrales como la clonidina, suelen producir
marcados descensos 4.

• Se obtenían muy buenos resultados en cuanto a mejoría de los niveles de
presión arterial1,2.
• También se reportó disminución de la tasa de eventos cardíacos y cerebro-
vasculares3
• Disminución de la masa ventricular izquierda4
• Mejoría en la función renal5
• Sin embargo se dejó de utilizar por su elevada morbi-mortalidad quirúrgica
(10%) y por la elevada incidencia de efectos adversos (hipotensión
ortortática, impotencia, incontinencia)6

68. CONCLUSIONES
La D.R, es un nuevo método, basado en una vieja
intervención, para el tratamiento de la H.A.R.
Este procedimiento endovascular, de 40 min. de
duración y muy bajo riesgo, se asocia a significativas
reducciones de la T.A.
La evidencia actual ha demostrado seguridad y
eficacia a más de dos años.
Se requiere más información para evaluar efectos a
largo plazo y selección de pacientes ideales.
Se aguardan los resultados de Symplicity HTN3 y
otros trials para evaluar beneficios en ICC, DBT, apnea
del sueño, IRC…
