Este documento describe las ventajas potenciales del balón medicado como alternativa a los stents medicados en ciertas situaciones especiales. El balón medicado podría proporcionar una liberación más homogénea y rápida de fármacos sin dejar una malla metálica permanente. Estudios como PEPCAD han demostrado que el balón medicado es efectivo para el tratamiento de lesiones de alto riesgo de reestenosis como la reestenosis intrastent, vasos pequeños y bifurcaciones, con resultados angiográficos y clínicos sim

¿Todos los pacientes requieren stent?
“La tecnologia basada en la liberaciòn de droga local sin stent, en particular el balón medicado podría
llenar las espectativas de los stent medicados dramaticamente ,evitando los problemas producidos por
los mismos . Pudiera ser la alternativa en lesiones de alto riesgo de reestenosis como lo son la
reestenosis intrastent, vasos pequeños y lesiones en bifurcación.
• Transferencia mas homogenea, mas rápida y mayor dosis de la droga a la pared del vaso
• Sin dejar protesis metalica en la pared del vaso
• Ausencia de deposito de polimero eluidor (que induce inflamación)
• Elimina los estimulos protromboticos
• Reduciendo así la duración de la doble antiagregación plaquetaria
• La liberación local con Balón Medicado esta recomendado en situaciones donde el stent no es
necesario o mas bien indeseable.

3. ¿POR QUÉ EL BALÓN MEDICADO?
DEB: Drug Eluting Balloon
Idea:
• Liberación local de la droga con la
superficie de un balón al contrario de un
stent con fármaco
• La dosis del paclitaxel es 3 µg por mm2 de
superficie del balón
• No hay una liberación a largo tiempo - >
10 segundos de inflado es suficiente
• Resultado pre-clínicos y clínicos son
excelentes y confirman el concepto del
balón con fármaco (DEB)
4. Nuevo Abordaje: SeQuent® Please
• El Balon SeQuent Please® es un balón de ATC cubierto
con una matriz homogenea de paclitaxel y medio de
contraste (iopromide)
• El paclitaxel es el farmaco activo de la matriz,
• Ampliamente estudiado como antireestenotico en
intervencionismo.
• El Paclitaxel inhibe la division de la celulas musculares
lisas mediante la estabilización de los microtubulos
durante la mitosis.
