• Ubicación: Húmero proximal
• Edad: Adolescencia – adultos
jóvenes (tumor deja de
crecer)
• Asintomático
• Dx: Rx simple
• Excrecencias óseas pedunculadas
continuas con el hueso cortical y
medular.
• En casos de exostosis hereditaria
multiple (malignidad hasta de 2.7%)
• TTO: Resección Qx en
lesiones sintomáticas
• Retrasarse hasta la
adolescencia debido al
riesgo de recurrencia.

• Ubicación: Metáfisis de
húmero proximal.
• Asintomático
• Dx: Rx simple (Incidental), hasta en el 2.1% RM
• Metafisaria , el borde bien marginado , calcificaciones
“en palomitas de maíz”.
• Dx diferencial: Condrosarcoma de bajo grado, infarto
óseo.
• Transformación maligna es baja para los encondromas
solitarios, riesgo es del 25% para la enfermedad de
Ollier ( encondromatosis múltiple)
• TTO: observación, salvo sospecha
de malignidad (Resección Qx)

• Ubicación: Metáfisis, epífisis, hueso subcondral.
• Edad: 20-40 años.
• Síntomas: Dolor + tumefacción.
• Dx: Rx simple, RM(compromiso de partes blandas).
TAC (diseminación)
• Metafisaria radiolúcida agresiva, destrucción cortical y
extensión en los tejidos blandos.
• Dx diferencial: Condroblastoma , osteosarcoma y
tumor marrón de hiperparatiroidismo
• Localmente agresiva, metástasis al pulmón en hasta el
7%
• TTO: El legrado intralesional + adyuvantes +
injerto o cementación.
• Recurrencia local del 9%,

• 4 veces menos frecuente que el TCG.
• Ubicación: epífisis (de donde se origina)
• Edad: 20-40 años.
• Síntomas: Dolor + tumefacción, limitación funcional (articular)
• Dx: Rx simple,
• Lesión excéntrica pequeña, bien definida.
• Dx diferencial: tumor de células gigantes, osteomielitis
y condrosarcoma
• Rara posibilidad de metástasis pulmonar
• TTO: El legrado intralesional + injerto vs
ablación con radiofrecuencia en lesiones
muy pequeñas

• Frecuencia no especifica (<10 % de su frecuencia habitual en
otros huesos)
• Edad: 5-25 años.
• Síntomas: dolor nocturno aliviado con aspirina
• Dx: Rx simple, TAC cuando en muy equeno
• Pequeño nido (osteoide) radiolúcido (1,5 cm) con un
borde de hueso reactivo
• Dx diferencialabsceso de Brodie y la fractura por estrés
• Rara posibilidad de metástasis pulmonar
• TTO: AINES para el control de los síntomas
o la ablación por radiofrecuencia para
destruir el nido.
OSTEOMA OSTEOIDE
(Deriva de osteoblastos)
SI LA LESIÓN NIDUS < 0,2CM Y NO SEDE CON AINES EL DX PROBABLE ES

• Lesión excéntrica dentro de la corteza metafisaria con
un borde esclerótico y un abultamiento del contorno
cortical.
• TTO: Observación (osifican en la edad
adulta).

• SINTOMATOLOGÍA CLÍNICA ES IMPORTANTE, ASOCIADO A LA EDAD E
IMAGENES
• EL HÚMERO PROXIMAL, EL RADIO DISTAL Y LA MANO SON LOS LUGARES
MÁS COMUNES PARA PATOLOGÍA BENIGNA
• LAS ZONAS DIAFISIARIAS DE LA EXTREMIDAD SUPERIOR, SE RELACIONAN
MÁS CON PATOLOGIA TUMORAL MALIGNA
• EL TGC, TIENE UN ALTO POTENCIAL METASTASICO PULMONAR, ANTE LO
CUAL EL TTO DEBE SER AGRESIVO.
• LA PATOLOGÍA TUMORAL BENIGNA DE LA MANO ES VARIABLE, LA
SINTOMATOLOGÍA SOBRE TODO DOLOR O LIMITACIÓN FUNCIONAL DEBEN
ESTABLECER EL TTO MÁS ADECUADO.
