7. Cambios con respecto a las anteriores ediciones:
Los despachadores deben ser entrenados para interrogar a las
personas que llaman con estrictos protocolos para obtener
información. Esta información debería centrarse en el
reconocimiento de la falta de respuesta y la calidad de la
respiración.
En combinación con la falta de respuesta, o ausencia de
respiración cualquier anomalía de la respiración deberían
iniciar un protocolo para sospecha de paro cardíaco. La
importancia de la disnea como signo de paro cardíaco se
acentúa.
8. Todos los reanimadores, entrenados o no, deberían
proporcionar compresiones torácicas a las víctimas
de paro cardiaco. Un fuerte énfasis en las
compresiones torácicas de calidad sigue siendo
esencial. El objetivo debería ser presionar a una
profundidad de al menos 5 cm a una velocidad de al
menos 100 compresiones por minuto, permitir que el
pecho se expanda, y minimizar las interrupciones de
las compresiones torácicas. Personal entrenado debe
proporcionar ventilaciones con una índice
compresión-ventilación de 30:2.

muertes en PP, Cuando la puntuación Glasgow (GCS) es

búsqueda de una lesión cerebral debe ser prioritaria, siempre
que los signos vitales lo permitan, con el fin de realizar
descompresión urgente. La TC permite identificar otras
posibles lesiones craneales asociadas, monitorizar su

Abstract
AIM:
The aim of this study was to validate the classification of hypovolaemic shock given by the Advanced Trauma Life Support
(ATLS).
METHODS:
Patients derived from the TraumaRegister DGU(®) database between 2002 and 2010 were analyzed. First, patients were
allocated into the four classes of hypovolaemic shock by matching the combination of heart rate (HR), systolic blood
pressure (SBP) and Glasgow Coma Scale (GCS) according to ATLS. Second, patients were classified by only one parameter
(HR, SBP or GCS) according to the ATLS classification and the corresponding changes of the remaining two parameters
were assessed within these four groups. Analyses of demographic, injury and therapy characteristics were performed as
well.
RESULTS:
36,504 patients were identified for further analysis. Only 3411 patients (9.3%) could be adequately classified according to
ATLS, whereas 33,093 did not match the combination of all three criteria given by ATLS. When patients were grouped by
HR, there was only a slight reduction of SBP associated with tachycardia. The median GCS declined from 12 to 3. When
grouped by SBP, GCS dropped from 13 to 3 while there was no relevant tachycardia observed in any group. Patients with a
GCS=15 presented normotensive and with a HR of 88/min, whereas patients with a GCS<12 showed a slight reduced SBP of
117mmHg and HR was unaltered.
CONCLUSION:
This study indicates that the ATLS classification of
hypovolaemic shock does not seem to reflect clinical reality
accurately.

short-axis views, the subxiphoid view, and the apical 4-chamber view (Fig. 1). The par-
asternal views are taken with the probe positioned just left of the sternumat intercostal
space 3 or 4. The subxiphoid 4-chamber view is obtained with the probe aimed up
toward the left shoulder from a position just below the subxiphoid tip of the sternum
(Fig. 2). The apical 4-chamber view of the heart is best evaluated by turning the patient
into a left lateral decubitus position and placing the probe just below the nipple line at
the point of maximal impulse of the heart. It is important for the EP to know all 4 views
