A 52-year-old man with type 2 diabetes of 8 years was uncontrolled on insulin therapy and gaining weight. He was obese, had hypertension and dyslipidemia. Dapagliflozin was added to his insulin regimen while reducing his insulin dose by 25%. This led to reductions in his HbA1c, weight, blood pressure, and lipid levels over 6 months of follow up while preventing further increases to his insulin needs. Dapagliflozin provided glycemic control and weight loss without increasing hypoglycemia risk for this patient with multiple comorbidities.
The 30+ year DCCT-EDIC trial provided important insights into the long-term effects of intensive glycemic control in type 1 diabetes patients. The DCCT found that intensive control significantly reduced the risk of developing diabetic complications compared to conventional treatment. The EDIC observational follow-up study found these benefits were durable, and intensive control continued to have beneficial effects on complications even after glycemic control was relaxed. Together, DCCT-EDIC established intensive glycemic control as the standard of care for type 1 diabetes and demonstrated its benefits in both preventing and delaying complications over the long term.

CAUSES :

PHARMACOLOGICAL TREATMENT :
Insulin:
People with type 1 diabetes must take insulin every day. You usually take the insulin through an injection.
Metformin :
Metformin is a type of oral diabetes medication. For many years, it was only used in people with type 2 diabetes. However, some people with type 1 diabetes can develop insulin resistance. That means the insulin they get from injections doesn’t work as well as it should.
Metformin helps lower sugar in the blood by reducing sugar production in the liver. Your doctor may advise you to take Metformin in addition to insulin.

intensificación de su control metabólico.
• Intervenciones:
- Optimización del tratamiento médico.
