1. RECONSTRUCCIÓN DEL LIGAMENTO
CRUZADO ANTERIOR TRIPLE-BANDA.

2. INTRODUCCIÓN
• El ligamento Anterolateral se ha identificado en
estudios cadavéricos en 97%.
• Claramente distinguible de la cápsula articular,
evidenciándose su estructura ligamentaria
histológicamente.
• Su origen es la prominencia del epicóndilo lateral y
se inserta entre el tubérculo de Gerdy y el borde
superior de la cabeza de la fíbula.
• Hipotéticamente posee una función en la rotación
interna, afectando el fenómeno de Pivot Shift.
3. OBJETIVO PRINCIPAL
• Describir una técnica quirúrgica para la
reconstrucción del ligamento Cruzado Anterior
Doble Banda Transtibial + Ligamento
Anterolateral anatómico con injerto Autólogo
de Semitendinoso-Gracilis.
4. Anatomía
CORTE HISTOLÓGIICO DEL LAL, EL CUAL SE EVIDENCIA TEJIDO
CONECTIVO ORDENADO PARECIDO AL ENCONTRADO EN
ELEMENTOS LIGAMENTARIOS.

EL ORIGEN ANATÓMICO DEL HAZ PL SE UTILIZA COMO
REFERENCIA, Y MEDIANTE UNA GUIA DE 90 SE PERFORA
DESDE EL ORIGEN DEL LAL EN EL EPICONDILO LATERAL.
9. SE DESLIZA DE FORMA PERCUTÁNEA HASTA UN PUNTO
INTERMEDIO ENTRE LA TUBEROSIDAD DE GERDY Y EL
TOPE DE LA CABEZA DE LA FÍBULA EL LAL.
10. FIJACIÓN DEL HAZ LAL
FIJACIÓN CON TORNILLOS DE BIOTENODESIS DE 5,5mm.
SE FIJA EL HAZ PM-LAL
POSTERIORMENTE SE PROCEDE A COLOCAR EL HAZ AM
DE MANERA STANDARD, CON UN MEDIO DE FIJACIÓM
CORTICAL
11. CONCLUSION
• Existe gran debate sobre la utilidad de añadir la
reconstrucción del ligamento Anterolateral a la
del Ligamento Cruzado Anterior.
• Es necesario un mayor número de estudios
biomecánicos que justifiquen su reconstrucción.
• Pudieran ser indicaciones pacientes con
hiperelasticidad, alto grado de pivot shift,
fracturas de Segond y en cirugías de revisión.
