1. TRATAMIENTO DE LA PATOLOGÍA
PERITROCANTEREA DE CADERA

2. SÍNDROME DOLOROSO DEL TROCANTER
MAYOR
Dolor en región lateral del muslo que puede
irradiar hasta rodilla.
-Patología lumbar.
-Patología pélvica.
-Patología de cadera.
-Tendinopatía glúteos medio y menor.
-Bursitis.
-Síndromes de compresión nerviosa (Piramidal,
Obturador interno).
-Resorte externo.
3. EPIDEMIOLOGÍA
Sexo: Mujeres (23.5% frente al 8.5% de los
hombres).
Edad: 40-60 años.
CLÍNICA
-Dolor que interfiere con el sueño y actividades
diarias.
- Dolor en nalga y espacio peritrocantereo con
irradiación similar a ciática.
- Cojera.
- Unilateral.
4. DIAGNÓSTICO
-Palpación dolorosa.
-Trendelemburg.
-Prueba de resistencia a la abducción.
-Rotación externa en posición sentada.
-Dolor y debilidad en la rotación interna pasiva con
cadera extendida.
-Hipotrofia muscular.
-Dolor y entumecimiento de la pierna.
-Infiltración con A. local descarta patología lumbar.
-RMN.
6. DIAGNÓSTICO
Este estudio concluye que los pacientes con
alteración de la marcha y abducción contra resistencia
escasa, tienen más probabilidad de requerir cirugía.
8. TRATAMIENTO QUIRÚRGICO.
Abierto vs artroscópico.
Arthroscopy 2015.
Revisión sistemática de reparaciones del glúteo medio.
7 estudios (3 c. abierta 127 caderas y 4 artroscopia 40
caderas).
No diferencias significativas HHS, VAS, fuerza
abductora.
Complicaciones: abierta 10/127
artroscopia 0/40
Conclusión: se recomienda cirugía artroscópica.
17. POSTOPERATORIO.
Carga parcial con muletas 6 semanas.
Movilización pasiva para evitar adherencias.
Evitar abducción y rotación externa.
6-8 semanas carga total progresiva con muletas.
Marcha normal y sin dolor a los 3 meses.
18. CONCLUSIONES.
1.- Patología frecuente.
2.- Tendinopatía glútea primera causa.
3.- Tratamiento no quirúrgico en la mayoría de casos.
4.- Casos recalcitrantes tratamiento quirúrgico: ARTROSCOPIA.
