Este documento resume las actualizaciones de las guías CHEST 2016 sobre el manejo de la enfermedad tromboembólica venosa. Algunas de las recomendaciones principales son: anticoagular de 3 a 6 meses para TVP proximal o EP, dependiendo de factores de riesgo; los ACOD son el tratamiento de elección a largo plazo excepto en síndrome antifosfolípido; y considerar aspirina u otros anticoagulantes orales de dosis baja para la fase extendida en pacientes sin cáncer.

Se denomina ictus agudo a un déficit neurológico, focal o global, de al menos 24h de duración y comienzo súbito, secundario a un compromiso brusco del flujo sanguíneo cerebral. Puede ser de causa isquémica (85% de los casos) o hemorrágica (15%).
El ictus isquémico agudo es una causa importante de morbimortalidad en los países desarrollados. Representa actualmente el 10% de la mortalidad global en España, constituyendo la segunda causa de muerte en los países occidentales. Además, es una de las principales causas de discapacidad en los adultos.
Una vez establecida la sospecha diagnóstica, la historia clínica del ictus debería incidir en tres aspectos.

Empatía cognitiva: Es la capacidad de comprender el punto de vista o el estado mental de otra persona. Es decir, saber lo que otra persona está pensando o sintiendo.
Empatía emocional: Es la capacidad de compartir los sentimientos de otra persona. Esto significa que, cuando otra persona está triste, tú también sientes tristeza.
Empatía compasiva: Va más allá de simplemente comprender y compartir sentimientos; implica la voluntad de ayudar a la otra persona a lidiar con su situación.
La empatía es importante en las relaciones interpersonales, ya que facilita la comunicación efectiva, reduce los conflictos y fortalece los vínculos. También es fundamental en profesiones que requieren interacción constante con otras personas, como la atención médica, la educación y el trabajo social.
Para desarrollar la empatía, se pueden practicar varias técnicas, como la escucha activa, la observación de las señales no verbales, la reflexión sobre las propias emociones y la exposición a diversas perspectivas y experiencias.
La empatía es esencial en todas las relaciones interpersonales, ya que permite comprender y compartir los sentimientos de los demás. Es una habilidad emocional que nos ayuda a ponernos en el lugar de otra persona y experimentar sus emociones y puntos de vista. Existen diferentes tipos de empatía, como la cognitiva, que implica comprender el estado mental de otra persona, la emocional, que consiste en compartir sus sentimientos, y la compasiva, que va más allá al involucrar la voluntad de ayudar a la otra persona.
La empatía facilita la comunicación efectiva, reduce los conflictos y fortalece los lazos entre las personas. También es fundamental en profesiones que requieren contacto constante con otras personas, como la atención médica, la educación y el trabajo social.
Para desarrollar la empatía, es importante practicar diferentes técnicas como la escucha activa, la observación de las señales no verbales, la reflexión sobre las propias emociones y la exposición a diferentes perspectivas y experiencias.

Los ACOD se recomiendan como primera opción para la
anticoagulación. Los AVK son una alternativa a los ACOD.
44. CONCLUSIONES
Pacientes con TVP proximal o EP se recomienda anticoagular
mínimo 3 meses con ACOD de 1ª elección excepto en SAF
ETEV provocada se trata 3 meses, no provocada o recurrente
fase extendida revisando anualmente
Si se suspende ACO y tiene FR se recomienda aspirina
TVP distal solo se anticoagula si síntomas severos o factores de
riesgo progresión
Ante TVP proximal se recomienda aco vs trombolisis
45. CONCLUSIONES
No se recomienda añadir Filtro de VCI a la ACO
No se recomiendan las medias para prevenir SPO
EP subsegmentario solicitar eco doppler mmii y se anticoagula si alto
riesgo
En EP de bajo riesgo se podría tratar ambulatoriamente
EP con hipotensión o deterioro tras iniciar tratamiento se recomienda
trombolisis vía sistémica vs trombectomía

3ª CAUSA MUERTE después del INFARTO y el ICTUS
1 muerte cada 37 segundos en el mundo occidental.
45 años --> riesgo de 8,5% de desarrollar ETEV durante el resto de la vida
se provee que aumente al 10,9% si obeso y 17,1% si trombofilia como factor V Leyden.
EUROPA: 23 millones tienen riesgo de padecer ETEV.
SINTOMAS: edema unilateral + enrojecimiento + dolor
1/2-1/3 casos de ETEV no factor precipitante o predisponente ETEV no provocada.
Cancer activo (20% ETEV)
Inmovilización prolongada y la cirugía mayor recientes (15% ETEV)
Hay 3 fases:
Fase de inicio: (~ 5-21 días) La provisión inicial de anticoagulantes después del diagnóstico de TEV.
Fase de tratamiento: (3 meses) El período después del inicio que completa el tratamiento para el evento de TEV agudo.
Fase extendida: (3 meses - sin fecha de finalización planificada) El período de uso de anticoagulante al máximo o dosis reducida para el objetivo de la prevención secundaria
