2. CASO CLÍNICO:
Mujer de 75 años, hipertensa, consultó por dolor abdominal de
meses de evolución y pérdida de peso.
Hepatograma con patrón colestásico
Ecografía con imagen nodular en lóbulo derecho del hígado
Serologías para hepatitis B y C y HIV negativas.
Marcadores tumorales: antígeno carcinoembrionario (CEA) y α-
fetoproteína normales, LDH alta
Resto del laboratorio normal.
Tomografía: lesión sólida en lóbulo derecho hepático sin realce
con contraste
3. •Se realizó biopsia hepática cuyo diagnóstico fue
linfoma no Hodgkin (LNH) Difuso a Células B Grandes.
• Se completó estadificación que confirmó compromiso
exclusivo de hígado (médula ósea, VCC, TC t, a y p)
•Recibió protocolo de quimioterapia con esquema R-
CHOP.
4. DISCUSIÓN:
El LNH primario de hígado representa menos del 1% de todos los
LNH.
Ocurre principalmente entre la quinta y la sexta década de la vida,
con predominio 2-3 veces en el sexo masculino.
Asociado a cirrosis biliar primaria, a inmunosupresión y a infección
crónica por el virus de la hepatitis C (20-60%); HIV y Epstein-Barr.
Se han reportado en la literatura menos de 100 pacientes con esta
patología sin antecedentes de hepatopatía.
El diagnóstico del LHP es difícil desde el punto de vista clínico,
radiológico e histológico.
En el laboratorio los marcadores tumorales normales (100%).
5. El diagnóstico definitivo puede ser alcanzado sólo por el
examen histológico del material obtenido por biopsia.
La mayoría de las lesiones reportadas son Linfomas B de
células grandes
Es necesario el análisis inmunohistoquímico y de citometría
de flujo
Los factores de mal pronóstico son la edad avanzada, la
hepatopatía previa, los niveles elevados de β2 microglobulina
y LDH y los síntomas B.
El tratamiento consiste en la radioterapia, la cirugía y/o la
quimioterapia. Esta última ha demostrado ser la mejor opción
por la buena supervivencia y menor invasividad.
