IMPACTO DE PATOLOGÍA TIROIDEA DURANTE EMBARAZO
1. Hipotiroidismo (0,8-2,5%): riesgo ↑ x 2-3 de complicaciones obstétricas: preeclampsia,
HTA, desprendimiento de placenta, parto pretérmino, bajo peso al nacer, cesáreas,
morbimortalidad neonatal, hemorragia postparto, aborto, infertilidad. Alterac cognitivas
2. Anticuerpos antitiroideos positivos (5-15%, prevalencia aumenta con la edad):
• Bloqueantes (ATPO, ATG, 8-14%, pasan placenta)*: abortos, parto pretérmino (tratam.
revierte riesgo), DM gestacional, peor neurodesarrollo, hipoacusia, TDAH, fertilidad
• Estimulantes (TSI, pasan placenta 2ª mitad gestación)*: hipertiroidismo fetal (bocio,
CIR, hidrops, craneosinostosis), Graves neonatal transitorio 2-20% (mortalidad 20%)
3. Hipertiroidismo (0,1-2,7%): preeclampsia, HTA, parto pretérmino, fallo cardíaco
materno, desprendimiento placentario, tormenta tiroidea, bajo peso neonatal
4. Tratamiento médico para hipertiroidismo:
• Metimazol: aplasia cutis, embriopatía con facies dismófica, atresia de coanas y
esofágica, atelia, defectos de pared abdominal y ventriculares septales, umbilicocele,
alteraciones del tracto urinario (exposición en primer trimestre)
• PTU: fallo hepático fulminante materno (muerte), malformaciones congénitas menores
(quistes en cara y cuello y anomalías del tracto urinario)
5. Hashimoto / enfermedad de Graves: niños con aumento del riesgo de padecer
Hashimoto y trastornos psiquiátricos /enf de Graves y DM-1 (cohorte danesa, Sept 2018)

RESULTADOS ADVERSOS SOBRE EL FETO
Conclusions: Undiagnosed hypothyroidism in pregnant women may adversely affect their
fetuses; therefore, screening for thyroid deficiency during pregnancy may be warranted.

6. • CONCLUSIONS: Low maternal FT4 was consistently associated with a lower IQ across
the cohorts. Further studies are needed to replicate the findings of autistic traits and
investigate the potential modifying role of maternal iodine status. FT4 seems a reliable
marker of fetal thyroid state in early pregnancy, regardless of the type of immunoassay.
HIPOTIROXINEMIA MATERNA (T4L BAJA)
RESULTADOS ADVERSOS SOBRE EL FETO
• OBJECTIVE: To assess the association of maternal thyroid function in
early pregnancy with child neurodevelopment in countries with a different iodine status.
• DESIGN: Meta-analysis of individual participant data from 9036 mother-child pairs from
3 prospective population-based birth cohorts: Spain, Netherlands, United Kingdom.
• MAIN OUTCOMES: Child nonverbal IQ at 5 to 8 years of age, verbal IQ at 1.5 to 8 years
of age, and autistic traits within the clinical range at 5 to 8 years of age.
• RESULTS: FT4 <2.5th percentile was associated with a 3.9-point (95% CI, -5.7 to -2.2)
lower nonverbal IQ and a 2.1-point (95% CI, -4.0 to -0.1) lower verbal IQ. A suggestive
association of hypothyroxinemia with a greater risk of autistic traits was observed.
FT4 >97.5th percentile was associated with a 1.9-fold (95% CI, 1.0 to 3.4) greater risk of
autistic traits. No independent associations were found with TSH.

for hypothyroidism did NOT result in improved cognitive function
in children at 3 years of age.
Universal screening compared with case finding did NOT result in a decrease in adverse
outcomes. Treatment of hypo or hyperthyroidism identified by screening a low-
risk group was associated with a lower rate of adverse outcomes.

13. CONCLUSION: Thyroid hormone treatment was associated with
decreased risk of pregnancy loss among women with subclinical
hypothyroidism, especially those with pre-treatment TSH concentrations of
4.1-10 mIU/L. However, the increased risk of other pregnancy related adverse
outcomes calls for additional studies evaluating the safety of thyroid hormone
treatment in this patient population.

Management of Thyroid Disease during Pregnancy and the Postpartum
Recommendation 97. All patients seeking pregnancy, or newly pregnant, should undergo
clinical evaluation. If any of the following risk factors are identified, testing for serum TSH
is recommended.
1. A history of hypo/hyperthyroidism or current symptoms/signs of thyroid dysfunction
2. Known thyroid antibody positivity or presence of a goiter
3. History of head or neck radiation or prior thyroid surgery
4. Age >30 years
5. Type 1 diabetes or other autoimmune disorders
6. History of pregnancy loss, preterm delivery, or infertility
7. Multiple prior pregnancies (2)
8. Family history of autoimmune thyroid disease or thyroid dysfunction
9. Morbid obesity (BMI 40 kg/m2)
10. Use of amiodarone, lithium, or recent administration of iodinated radiologic contrast
