Este documento describe el glaucoma neovascular, una forma grave de glaucoma secundario. Se define su etiología, fisiopatología, curso clínico, diagnóstico y tratamiento. La fisiopatología involucra la angiogénesis retinal debido a la isquemia crónica, lo que conduce a la formación de vasos frágiles y aumento de la presión intraocular. El tratamiento depende del estadio y apunta a controlar los neovasos, la presión ocular y el dolor ocular.

Neovascularization of the angle (NVA). Open-angle glaucoma
can develop at this stage before synechial formation occurs
21. Peripheral anterior synechiae (PAS)
Neovascularization
(C) Chamber angle vessels (arrow) cross the scleral
spur and arborize in the trabecular meshwork, with
later contraction of the fibrovascular membrane to
produce peripheral synechiae (arrowhead).

OBJETIVO
Tratamiento causa base: prevenir GNV, Eliminar la
estimulación neovascular, Regresión vasos
Disminuir PIO

33. TRATAMIENTO
SEGÚN ESTADIO:
Profiláctico:
Control patología base
Estadios tempranos:
-Control neovasos: PRP (sin OM),
Crioterapia (con OM) + antiVEGF.
-Control PIO.
Estadios tardíos: Igual anterior
-Control PIO: Medico-Quirúrgico:
Valvula>Trabe+MMC.
Terminal:
1. Esteroide, Ciclopléjicos.
2. Procedimientos ciclodestructivos.
3. Inyecciones alcohol?
4. Evisceración-enucleación.

34. • Estadio temprano (Rubeosis Iridis)
Meta: control de neovasos.
TRATAMIENTO
35. Fotocoagulación Panretiniana
Util: Neovasos disminuye angiogénicos. Permanente.
+en RD, OVCR, -SIO, OR.
Éxito: 68% regresión vasos, 42% PIO
Número disparos: 1200-1600 en 3 sesiones----70.4%vs

36. Crioterapia panretinal
Util: Neovasos en opacidad de
medio o CI anterior.
Efectos: similares: ↓ neovasos, ↓

Meta: control de PIO
TRATAMIENTO
43. Éxito en control del PIO95%<20mmhg en 1 sem, a los 6m de sgto.
Desenlace 2dario: ↓ hifema y antiglaucomatosos 3 veces menos POP.
