4. PATIENTS
• Between November 2009 and March 2010,
they prospectively studied 27 consecutive
adult patients with intracranial gliomas who
presented to the UCLA Department of
Neurosurgery for surgical resection of the
brain tumor
• All patients had measurable disease on
magnetic resonance imaging (MRI) for which
surgical resection was warranted

newly diagnosed or recurrent gliomas
underwent pre-operative magnetic resonance
imaging (MRI) and magnetic resonance
spetroscopy (MRS).

• Nine of the patients had IDH1 mutant tumors
and 15 were wild type for IDH1.
• Of the IDH1 mutant cases (n=9) there were
four females and five males, and the average
age was 43 years (range 22-67 years old).
• Of the patients with IDH1 wild type tumors
(n=15) eight were females and seven were
males, with an average age of 59 years (range
40-87 years old)

• 2-HG levels from MRS data were quantified using
LC-Model software, based upon a simulated
spectrum obtained from a GAMMA library added
to the existing prior knowledge database.
• The resected tumors were then analyzed for IDH1
mutational status by genomic DNA sequencing,
Ki-67 proliferation index immunohistochemistry,
and concentrations of 2-HG and other
metabolites by liquid chromatography–mass
spectrometry (LC–MS).

21. ANALISIS DE RESULTADOS
• MRS detected elevated 2-HG levels in gliomas with
IDH1mutations compared to those with wild-type
IDH1(P = 0.003).
• The 2-HG levels measured in vivo with MRS were
significantly correlated with those measured ex vivo
from the corresponding tumor samples using LC–MS(r2
= 0.56; P = 0.0001). Compared with wild-type tumors,
those with IDH1 mutations had elevated choline (P =
0.01) and decreased glutathione (P = 0.03) on MRS.
• Among the IDH1 mutated gliomas, quantitative 2-HG
values were correlated with the Ki-67 proliferation
index ofthe tumors (r2 = 0.59; P = 0.026)

24. UTILIDAD CLINICA DE RESULTADOS
• Determinan un nuevo biomarcador (2HG)en
pacientes con glioma asociado a mutación de
IDH1
• Determinan la utilidad de la espectroscopia
por resonancia magnetica como método no
invasivo para diagnostico de pacientes con
mutación de IDH1
25. • Identificaron que las mutaciones en IDH1
muestra alteraciones metabolicas adicionales,
por ejemplo elevación de niveles de colina en
comparación con gliomas sin mutación.
• Relacionaron que la mutación de IDH1 se
presenta en estadíos tempranos, lo cual es útil
para determinar la progresión del tumor.
• Abrieron la posibilidad de diseñar blancos
para inhibución farmacológica y diseño de
métodos no invasivos de tratamiento.
26. RESULTADOS AUSENTES
• Determinar los niveles de mutación en IDH1
en pacientes en estado de remisión.
• Identificar el grado de mutación de IDH1
según los estadíos de la enfermedad.
• Determinar la progresión de la enfermedad In
Vivo y alteraciones metabolicas globales.
27. CONCLUSIONES
• El tipo de muestreo no permite hacer
inferencias sobre la población ni probar
hipotesis.
• No se utilizó el intervalo de confianza porque
al ser la muestra tan pequeña, los intervalos
son muy amplios y no hay confiabilidad
• El 59% de los pacientes con proliferacion del
tumor presentan mutacion en IDH1
28. CONCLUSIONES
• El estudio aunque no alcanza a llegar a un r2
de 1 (r2=0.59) explica la habilidad de la
espectroscopia por resonancia magnetica para
establecer diagnostico.
• Este resultados podria relacionarse con el
tamaño de la muestra
