15. Síntomas
• La insuficiencia adrenal usualmente inicia pre-
pubertad. Sin embargo, hay casos reportados de
inicio en la tercera década.
• El diagnóstico no se debe excluir en ausencia de
insuficiencia adrenal.
• Alacrima se considera un síntoma temprano,
usualmente aparece en la infancia.

18. Manifestaciones neurológicas
• Atrofia distal extremidades y debilidad muscular.
• Alteraciones pares craneanos bajos que resulta en
disartria y voz nasal.
• Hiperreflexia, ataxia, atrofia óptica, y rara vez,
discapacidad sensorial .
• Disfunción autonómica, que se manifiesta por
anormalidades pupilares, anisocoria, miosis e
hipotensión ortos tatica.
• Estudios de conducción nerviosa por lo general
revelan un neuropatía motora axonal.

19. Diagnóstico
• Sospecharlo en pacientes con insuficiencia
adrenal atípica.
• Seguimiento estricto
• Test genéticos para identificar la mutación.
• Tratamiento sintomático.
Triple A syndrome, or Allgrove syndrome, is an autosomal recessive genetic disease (OMIM ID: 231550). Patients typically suffer from chronic adrenal insufficiency due to resistance to ACTH (Addison's disease), achalasia of the cardia, and defective tear formation (alacrima); hence the name, which describes the Achalasia–Addisonianism– Alacrima (triple A) syndrome. In addition, many patients develop neurological symptoms and autonomic dysfunction in adulthood, while osteoporosis or dermatological manifestations and other symptoms may also be present. The association between these three main conditions was originally made by endocrinologist Dr. Jeremy Allgrove and colleagues who also noticed the inherited nature of the disease (Allgrove et al., 1978), which is therefore also referred to as Allgrove syndrome. The large number of affected systems, together with the complexity and variability of presentation, are factors that may contribute tomisdiagnosis of the disorder andmay hinder in-depth understanding of its pathogenesis.

Recent studies indicate that the mutant ALADIN results in a failure to import DNA repair proteins (aprataxin and DNA ligase I) and ferritin heavy chains in to the nucleus, and the resultant hypersensitivity to oxidative stress may trigger cell death in the tissues where ALADIN is widely expressed
