• Diseñada para estandarizar el informe mamográfico, reducir
confusión en la interpretación y facilitar la monitorización de
resultados.
• Se 7y para el manejo de hallazgos radiològicos no palpables.
• El sistema BI-RADS es la base de la semiología mamaria y comprende
tres aspectos:

• COMO NORMA GENERAL SE
EMPLEAN DOS PROYECCIONES
UNA CRANEOCAUDAL Y OTRA
MEDIO LATERAL OBLICUA .
• La proyección cráneo caudal
debe incluir:
• Banda grasa retromamaria y algo
de mùsculo
5. PROYECCIONES
• Proyecciòn mediolateral oblicua
• Abarca practicamente todo el
tejido mamario desde la axila
hasta la pared abdominal .
• Debe mostrar la grasa retro
glandular y gran parte del
m.pectoral(debe observarse 1cm
por debajo de la linea posterior
del pezòn).
22. Macrocalcificaciones (palomita de maíz)
• Mayores de 3mm de diámetro
• Secundarias a un fibroadenoma en
evolución.
• Generalmente situadas en la
periferia de un nòdulo
23. Calcificaciones lineales gruesas
• Calcificaciones benignas asociadas a
ectasia ductal.
• >60 años
• Forma de huso liso con diámetro de
0,5 a màs.
• Generalmente bilaterales, se irradian
hacia el pezón.
24. Calcificaciones redondeadas
• Se consideran benignas cuando son difusas y
pequeñas <1mm
• Son frecuentes a nivel de los acinos lobulillares.
• Las menores de 0,5mm corresponde emplear
“puntiformes”.
• Existe un grupo de calcificaciones puntiformes
considerado “probablemente benigno”->vigilancia
mamográfica.
• Recomienda biopsia
• De reciente aparición
• Forma lineal
• Segmentaria
26. Calcificaciones anulares (càscara de huevo)
• Finas y benignas.
• Grosor inferior a 1mm.
• Ovaladas o redondedas
• Superficie lisa radiotransparente
• Asociada a la necrosis grasa y
paredes de los quistes
28. Calcificaciones-Leche càlcica
• Representa sedimiento de
calcificaciones en macro o
microquistes.
• Mejor evaluadas en proyección OML en
morfología semilunar, curvilínea o
lineales.
• La característica màs importante es el
cambio de forma en las diferentes
proyecciones.
36. CALCIFICACIONES AMORFAS (INDEFINIDAS)
• Pequeñas y borrosas que no es posible
definir la forma de las partículas
calcificadas.
• Su distribución lineal, segmentaria o
agrupada son sospechosas.
• Si son difusas y bilaterales es apropiado
considerarlas benignas
38. Calcificaciones sospechosas - groseras
heterogèneas
• Irregulares, suelen coalescer (màs
pequeñas que las distròficas)
• Miden entre 0,5 y 1mm .
• Si se observan varios grupos bilaterales se
las puede considerar benignas.
40. Calcificaciones sospechosas-finas pleomorfas
• Suelen ser màs evidentes que las
c.amorfas con formas màs
reconocibles.
• Pueden ser de distinto tamaño y
forma. No tienen partículas
lineales finas.
• Generalmente menores a
0,5mm
42. Calcificaciones sospechosas-lineales finas o
lineales finas ramificadas
• Son delgadas, lineales e
irregulares.

• Son las que tienen mayor VPP :

• Hallazgos que representan

• Aparecen solo en una
proyección.
• Suelen ser de bordes

mama contralateral.
• Se considera cuando ocupa al

• Tejido fibroglandular denso y
asimétrico ocupa una porción
limitada de la mama.
• Se observa en diferentes
proyecciones
• Carece de borde convexo
• Intercalado con grasa
54. Asimetrìas en desarrollo
• Son asimetrías focales nuevas, mas grandes o màs evidentes que las
focales de imágenes anteriores.
• 15% representan lesiones malignas.
• Se recomeinda ampliar con otros estudios de imágenes y biopsia a
menos que se observa alguna lesión benigna.

Obscured or partially obscured, when the margin is hidden by superimposed fibroglandular tissue. Ultrasound can be helpful to define the margin better.
Microlobulated. This implies a suspicious finding.
Indistinct (historically ill-defined).
This is also a suspicious finding.
Spiculated with radiating lines from the mass is a very suspicious finding.
LIPOFILING:These were the result of lipofilling, which is transplantation of body fat to the breast.
B. This was reported as BI-RADS 5 and proved to be an invasive ductal carcinoma.
