96. Vacunas: RV 144 El régimen tipo inducción-refuerzo evaluado consistió en seis inyecciones de dos candidatas a vacuna a lo largo de un período de seis meses. Se administraron cuatro inyecciones de la primera vacuna (conocida como ALVAC HIV [vCP1521]), que utilizó como vector un virus aviar modificado, que no puede provocar enfermedad en humanos, para transportar fragmentos no infecciosos del VIH al organismo. La vacuna de refuerzo fue AIDSVAX B/E , que consistió en una versión modificada de una de las proteínas de superficie del VIH (gp120).

Siguiendo al paciente de “Berlin”: genoterapia En un pequeño ensayo abierto, usando la tecnología de la nucleasa del dedo de Zinc (fractura en el ADN de doble cadena del gen del CCR5 de sus propios CD4 ) un total de 6 personas con VIH en tratamiento y con carga viral indetectable, y niveles de CD4 entre 200 y 500 células/mm3, repartieron en dos cohortes para recibir o bien 10.000 millones o 20.000 millones de células. Un tercer grupo, actualmente en estudio, recibe 30.000 millones de células. En el laboratorio, se expandieron células T autólogas con alteración del CCR5 con una tasa media de modificación de este correceptor del 25%. Las células resultantes se conocen con el nombre de SB-728-T. Las infusiones fueron bien toleradas y solo se dieron efectos secundarios leves, como un síndrome febril durante la infusión. Los participantes fueron seguidos de forma semanal durante un mes y mensualmente durante los 11 meses siguientes a la infusión. Retornando a los artículos en Science , Montagnier 1 hace una cronología sintética de lo que sucedió en la Unidad de Oncología Viral del Instituto Pasteur de Paris que dirigía desde 1972. En enero de 1983, de una biopsia ganglionar de un joven homosexual con adenopatías (BRU) se obtuvieron linfocitos que se cultivaron con IL-2 y suero humano anti-interferón _el cual aumentaba considerablemente el título de retrovirus en ratones. A los 15 días, se detectó transcriptasa reversa en el sobrenadante del cultivo indicando la presencia de un retrovirus. Este primer resultado se publicó en Science en mayo de 1983, y es en rigor el primer trabajo sobre el virus causante del sida 13 _ según también lo reconoce Gallo 2 . Los únicos retrovirus humanos conocidos hasta entonces eran HTLV-I y II, que habían sido recientemente descubiertos por Gallo et al 5,12 , pero los anticuerpos correspondientes no se encontraron en el cultivo BRU. Una segunda biopsia de otro paciente homosexual (MOI) sí resultó positiva tanto para el nuevo virus como para HTLV-I; a ese respecto, Montagnier señala que si MOI hubiera sido el caso inicial se hubiera creado mucha confusión. Luego se cultivaron sangre y tejido ganglionar de un paciente homosexual con sarcoma de Kaposi (LAI). El virus LAI pudo ser recuperado de la sangre y replicó rápidamente en cultivos de linfocitos T, destruyendo tanto los linfocitos T propios como los de dadores de sangre. Se pudo aislar un virus similar del suero de una mujer del Zaire que murió de sida una semana después. Estos virus presentes en sangre fueron designados IDAV ( immune deficiency-associated viruses ) mientras que los tipo BRU provenientes de ganglios y menos agresivos, fueron designados LAV ( lymphopathy-associated viruses ). Montagnier _contrariamente a Gallo_ estaba convencido que se trataba de un virus diferente de HTLV. Se puso el énfasis en reproducir el virus BRU en cultivos de linfocitos T, aunque sin éxito; si se hubiera usado LAI hubiera sido mucho más exitoso. Finalmente se consiguió cultivar BRU en linfocitos B transformados por EBV, pero luego se comprobó que el cultivo había sido contaminado con LAI, a su vez asociado con micoplasma, lo que aparentemente había facilitado la replicación viral. Abstract We conducted a double-blind, placebo-controlled trial of the efficacy of oral azidothymidine (AZT) in 282 patients with the acquired immunodeficiency syndrome (AIDS) manifested by Pneumocystis carinii pneumonia alone, or with advanced AIDS-related complex. The subjects were stratified according to numbers of T cells with CD4 surface markers and were randomly assigned to receive either 250 mg of AZT or placebo by mouth every four hours for a total of 24 weeks. One hundred forty-five subjects received AZT, and 137 received placebo. When the study was terminated, 27 subjects had completed 24 weeks of the study, 152 had completed 16 weeks, and the remainder had completed at least 8 weeks. Nineteen placebo recipients and 1 AZT recipient died during the study (P<0.001). Opportunistic infections developed in 45 subjects receiving placebo, as compared with 24 receiving AZT. The base-line Karnofsky performance score and weight increased significantly among AZT recipients (P<0.001).

We assessed the changes in markers concentrations between randomisation and week 12 between the CT and STI arms. At week 12, patients in the STI arm were off treatment. We found a significant difference between markers in CT versus STI arm with regards to MCP1, adiponectin and MIP1-alpha.

In the overall population, rilpivirine was found to be noninferior to efavirenz with 84.3% and 82.3% of patients, respectively, reaching HIV-1 RNA < 50 copies/mL. This resulted was mirrored in the responses in the individual trials. However, in the HIV-1 RNA stratification analyses, there were some differences. Patients in the higher HIV-1 RNA stratum ( > 100,000 copies/mL) appeared to have better results with efavirenz than with rilpivirine. This difference was most noted in the ECHO study.
EFV, efavirenz; TLOVR, time to loss of virologic response. This slide illustrates the 24-week results. At initial comparison, it would appear that S/GSK1349 572 is clearly superior to efavirenz, but we must remember that INSTIs suppress HIV-1 RNA more rapidly than efavirenz but not necessarily more completely when results are compared at 48 weeks. Therefore, 24 weeks is too early to be able to assess real differences between these 2 regimens. These are very preliminary data, but they suggest this drug is effective at all of the doses tested. Development of this drug is moving forward with phase III studies using the 50-mg dose. S/GSK1349 572 was very well tolerated in this trial; there were no serious adverse events and a better CD4+ cell count response when compared with efavirenz.

Coformulated cobicistat-boosted elvitegravir plus tenofovir/emtricitabine (“quad” regimen) has been compared in a phase II trial with tenofovir/emtricitabine/efavirenz in 71 treatment-naive patients. Virologic suppression was comparable between the 2 arms with a numerical difference favoring the quad regimen; a greater CD4+ cell count increase was also seen in the quad arm vs efavirenz. A phase III study of this comparison is ongoing.

in an open-label, randomized trial conducted in 642 sputum acid-fast bacilli smear-positive patients with HIV infection and CD4+ cell counts < 500 cells/mm3.[33] The median CD4+ cell count at baseline was approximately 150 cells/mm3. Overall, similar rates of AIDS-defining illness and death were observed during follow-up across the 2 arms, with no significant difference in the incidence rate ratio (IRR) when comparing earlier to later HAART. However, in persons with baseline CD4+ cell counts < 50 cells/mm3, there was a clear trend toward a reduction in the incidence of AIDS or death among patients starting HAART earlier rather than later (IRR: 0.32; 95% CI: 0.1-1.1; P = .06). This benefit was set against an almost 5-fold increase in risk of immune reconstitution inflammatory syndrome (IRIS) within the early HAART group at this CD4+ cell count stratum (IRR: 4.7; 95% CI: 1.5-19.6; P = .01).
The ACTG 5221 STRIDE trial was a randomized, open-label, phase IV study in which 806 HIV-infected persons with confirmed or suspected TB and CD4+ cell counts < 250 cells/mm3began immediate HAART, begun within 2 weeks of TB therapy initiation, or early HAART, begun 8-12 weeks after TB therapy initiation (Capsule Summary).[34] Median CD4+ cell count was 70-82 cells/mm3. After 48 weeks, there was no significant difference in the incidence of death or AIDS events with immediate vs early HAART in the overall population. In common with the observations from SAPIT, however, there was a significant reduction in the rate of AIDS or death with immediate HAART in the subgroup of patients with baseline CD4+ cell count < 50 cells/mm3 (15.5% vs 26.6%, respectively; 95% CI for the difference between the 2 arms: 1.5-20.5; P = .02). Incidence of IRIS was increased with immediate vs early HAART in the study group overall. In the STRIDE study, there were more deaths attributed to TB in the immediate vs early HAART arm (14 vs 7). Of course, this was an open-label study, but it may be that some of those excess deaths were due to IRIS. That does not take away from the overall message of the study, because there was a survival endpoint, but it does suggest that patients who start HAART at low CD4+ counts may need careful management for IRIS and perhaps more aggressive use of corticosteroids or antiinflammatory medications.

RESULTADOS: La edad promedio de las embarazadas fue de 27 años (16-40); el nivel de CD4+ 489 cel (DS 247); 63% de las que se contaban la CVP en el tercer trimestre del embarazo fueron indetectables. El 100% utilizo algún tipo de esquema de tratamiento; el 75% (52) 2 INTR + 1 IP, (33 lopinavir/r, 9 nelfinavir, 9 saquinavir/r, 1 atazanavir/r), el 22% (16) estaba con 2INTR + 1 INNTR (nevirapina), el 3% (2) biterapia con AZT/3TC. Nacieron vivos 68 neonatos, todos con PCR para VIH negativas. El peso promedio fue 2770g y 13 (16%) requirieron internación neonatal . Dieciséis neonatos (23%) fueron RNPT de los cuales (50%) nacieron antes de la semana 34. Once embarazadas (68%) recibieron IPP (RR: 2,2 IC95% 0,99-4,89), el 31% (5) nevirapina + 2INTR. El 43% (7) de los RNPT estuvo internado en neonatología. La posibilidad de RNPT en embarazadas tratadas con IPP fue mayor al conjunto de los otros tratamientos: 43 (11) VS 27 (5) RR: 1,38 IC95% (0,54-3,54); (p:0.03). CONCLUSIONES: la prevalencia de RNPT en el grupo de embarazadas HIV + bajo TAR estudiadas fue mayor que en la población general (8%). Se encontró además una asociación estadísticamente significativa entre RNPT y tratamiento con IPP.
La otra investigación -esta vez un estudio retrospectivo-, presentada en la misma rueda de prensa por Moupali Das-Douglas, del Departamento de Salud Pública de San Francisco, ha analizado la relación entre la disminución de la carga viral en la población con VIH -parámetro definido como carga viral comunitaria (CVC)- de San Francisco (EE UU) y el número de nuevas infecciones. En el análisis se incluyeron datos del sistema de seguimiento del VIH/sida de la ciudad, lo que permitió obtener la CVC media y total y examinar la tendencia de ésta a escala global y en poblaciones determinadas. Estos datos se compararon con el número de nuevos diagnósticos de la infección (entre 2004 y 2008) y los cambios en la incidencia calculada de la infección por VIH entre 2006 y 2007. Tanto la CVC media como la total se redujeron entre 2002 y 2008, y fueron acompañadas por descensos en la tasa de nuevas infecciones por VIH y en la incidencia de la infección. La disminución de la CVC media se asoció significativamente con la reducción en el número de nuevos diagnósticos de la infección entre 2004 y 2008 (p= 0,003) , además de con la incidencia decreciente de la infección por VIH entre 2006 y 2007 (p <0,0005). La CVC total, por su parte, también se relacionó de forma significativa con el número de nuevos diagnósticos (p= 0,002) y con la incidencia de la infección (p <0,0005). Los autores del estudio concluyeron que el incremento de las opciones terapéuticas, la cobertura sanitaria de la población con VIH y el acceso a pruebas diagnósticas conllevaron una disminución de la CVC en San Francisco, que se asoció a reducciones sustanciales en el número de nuevos diagnósticos y la incidencia de la infección.

Ello propició que su sistema inmunitario se reconstruyera con células carentes de CCR5. Aplicar esta estrategia -trasplante de médula ósea- conlleva un alto riesgo para la vida del paciente, por lo que no procede hacerla extensiva a todas las personas con VIH. Por este motivo, estos grupos de científicos están investigando la modificación de células T del propio paciente (autólogas), en el laboratorio, para más tarde volver a introducirlas en su organismo.
