Este documento trata sobre la constipación funcional en pediatría. Resume que la constipación funcional es un problema común en la infancia con una prevalencia estimada del 3% y se caracteriza por infrecuencia en la defecación, defecación dolorosa e incontinencia fecal. Explica que el diagnóstico se basa en la historia clínica y examen físico sin necesidad de pruebas adicionales y que el tratamiento incluye desimpactación, lubricantes, osmóticos y formadores de bolo.

4. Irritantes o estimulantes: efecto terapéutico se logra entre

Constipación: Constipare = Constreñir.
Estreñimiento: Retención anormal de materia fecal con una frecuencia menor a tres deposiciones por semana, con dificultad para evacuar.
Encopresis: Paso involuntario de heces formadas, semiformadas o líquidas que manchan su ropa interior.
Incontinencia fecal: Es el paso involuntario de las heces asociado a la incapacidad de retener el contenido intestinal en el recto.
Impactación fecal: Resultado de la acumulación de materia fecal en el recto que se expresa como una masa abdominal dura e inamovible en hemiabdomen inferior.

Entre más tiempo dure la materia fecal en contacto con el colonocito, se reabsorberá más agua y el bolo fecal se tornará más duro hasta impactarse, produciendo dolor abdominal y dificultad para defecar.
The age of the
child when symptoms began is one of the easiest and most important
pieces of information to obtain in the evaluation of the problem.
Onset of symptoms in infants <1 month old raises the suspicion of
the presence of an organic condition such as Hirschsprung disease
(HD) (21). The timing of passage of the first meconium is especially
relevant to the risk of having HD; delayed passage of meconium by
48 hours in a term neonate suggests the need for definitive testing to
rule out the diagnosis. Although 99% of healthy term neonates pass
their first meconium before 48 hours of life (22), 50% of children
with HD also pass meconium within 48 hours of birth (23). Thus,
the failure of passage of meconium within the first 48 hours of life,
although suggestive of HD, does not establish the diagnosis.
it may reveal a fecal impaction that requires a cleanout approach or the presence of occult blood that requires further diagnostic testing.
Hemoccult testing of the stool should be performed in most cases, particularly in infants with constipation, in whom subclinical milk protein intolerance (or other food protein intolerance) may present as constipation (see 'Cow's milk intolerance' above). In infants and children with marked abdominal distension or who are ill-appearing, hemoccult positive or bloody stools may indicate enterocolitis, and the patient requires prompt further evaluation. (See 'Hirschsprung disease' above.)
A plain abdominal radiograph is not indicated for the routine evaluation of functional constipation [17]. However, it can be helpful to document retained stool when there is inadequate historical information to determine if the patient has constipation or if the physical examination is limited by patient cooperation, obesity, or is deferred for psychological considerations. It should be recognized that abdominal radiographs are inconsistently interpreted by different observers, are not particularly specific for constipation, and rarely add to the diagnosis if the rectal examination reveals a large amount of retained stool [17,53,54]. Thus, they are not an essential part of the evaluation of constipation and should not be used as a substitute for a thorough history and physical examination.
Barium enema – A barium enema provides supportive evidence for Hirschsprung disease in children with features suggestive of this disorder, such as early onset constipation from the neonatal period, especially with delayed passage of meconium, or suggestive findings on anorectal examination. The study should be performed "unprepped", ie, without measures to remove stool from the rectum (image 1A-B). Some providers use anorectal manometry as the initial investigation or proceed directly to rectal biopsy. In very young infants, the barium enema may be normal and the diagnosis must be established by rectal biopsy. (See 'Hirschsprung disease' above and "Congenital aganglionic megacolon (Hirschsprung disease)".)
●Spine radiographs – Plain films of the lumbosacral spine should be performed for children with evidence of spinal dysraphism or neurological impairment of the perianal area or lower extremities. If there is a high suspicion of neurologic dysfunction, magnetic resonance imaging (MRI) should be considered to investigate the possibility of tethered cord and spinal cord tumors [17,55,56]. Mayores de dos años: 5 g de fibra/día 6 meses y los 2 años de vida 0,5 g de fibra/kg/día hasta los 10 años de edad 34. Tener horarios fijos para acudir al baño, sentarse en el inodoro 3-4 veces al día,durante 5 a 10 minutos, contar con un sanitario cómodo, que pueda apoyar los pies mientras está sentado en él y tenga un ambiente libre de temores. Promover la actividad física y la práctica de deportes. Esto mejora el ritmo evacuatorio y sirve como parte integral del tratamiento.
