Artículo 4.
Objetivos.
El Banco de Sangre y Tejidos de Navarra tiene como objetivos generales:
a) Velar por el cumplimiento del Real Decreto 1088/2005,…
d) Conseguir la autosuficiencia de sangre y hemoderivados en Navarra.
f) Promover la disminución de la exposición a sangre homóloga y
hemoderivados, actualizando y divulgando recomendaciones de uso e
indicaciones y potenciando otras alternativas a la transfusión, incluyendo
todas las modalidades de autotransfusión.
j) Promover programas de formación del personal del Banco de Sangre y Tejidos
de Navarra, de carácter continuo, que incluirán programas de buenas prácticas.

Artículo 5.
Funciones.
1. Como Centro Comunitario de Transfusión desempeñará las funciones
siguientes:
a) Coordinar las actuaciones de los Centros y Servicios en la Red de transfusión
y de células y tejidos humanos de Navarra.
n) Fomentar programas de autotransfusión, ahorro de sangre y esquemas
mínimos de transfusión en todos los centros hospitalarios de la Comunidad.
p) Implantar programas de formación del personal vinculado a la hemoterapia

MOTIVOS PARA REDUCIR LA TRANSFUSIÓN

16. The findings of the 1998 meta-analysis were confirmed, with small variations in some
estimates. Identification of 237 references. Thirty-six studies on 12,127 patients were
included: 23 showed a detrimental effect of PBT; 22 used also multivariable analyses,
and 14 found PBT to be an independent prognostic factor.
Pooled estimates of PBT effect on colorectal cancer recurrence yielded overall
OR of 1.42 (95% CI, 1.20 to 1.67) against transfused patients in randomized
controlled studies.
Stratified meta-analyses confirmed these findings, also when stratifying patients by site
and stage of disease. The PBT effect was observed regardless of timing, type, and in a
dose-related fashion, although heterogeneity was detected. Data on surgical techniques
was not available for further analysis.

25. Criterios generales de indicación
Uso adecuado:
• Presencia de signos o síntomas de hipoxia
tisular
• Ausencia de tratamiento específico de la
anemia
• Refractariedad al tratamiento específico
• La situación clínica del paciente precisa una
reposición inmediata de la masa eritrocitaria

35. First Recommendation
The AABB recommends a restrictive RBC transfusion threshold
in which the transfusion is not indicated until the Hb level is
7g/dL for hospitalized adult patients who are hemodynamically
stable, including critically ill patients, rather than a liberal
threshold when the Hb level is 10g/dL (strong recommendation,
moderate quality evidence).
For patients undergoing orthopedic surgery or cardiac surgery
and those with preexisting cardiovascular disease, the AABB
recommends a restrictive RBC transfusion threshold (Hb level
of 8g/dL) (strong recommendation,moderate quality evidence).
The restrictive Hb transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, but
RCT evidence is not available for all patient categories.

36. Recommendation 1: a restrictive RBC transfusion threshold in which the transfusion is
not indicated until the hemoglobin level is 7 g/dL is recommended for hospitalized adult
patients who are hemodynamically stable, including critically ill patients, rather than
when the hemoglobin level is 10 g/dL (strong recommendation, moderate quality
evidence).
A restrictive RBC transfusion threshold of 8 g/dL is recommended for patients
undergoing orthopedic surgery, cardiac surgery, and those with preexisting
cardiovascular disease (strong recommendation, moderate quality evidence).
The restrictive transfusion threshold of 7 g/dL is likely comparable with 8 g/dL, but RCT
evidence is not available for all patient categories.
These recommendations do not apply to patients with acute coronary syndrome, severe
thrombocytopenia (patients treated for hematological or oncological reasons who are at
risk of bleeding), and chronic transfusion–dependent anemia (not recommended due to
insufficient evidence).
