11. Sobretransfusión en Cirugía Abdominal
Mayor
2,905 patients:
895 (31%) were transfused (51% pancreatic,
14% hepatobiliary, 21% colorectal, and 14% other)
512 (57%) were overtransfused :
33% transfused at too high trigger
59% received 2 packed red blood cell units
There was significant variation in
overtransfusion among surgeons (range 0%
to 80%, P = .003)
CONCLUSIONS: Excess use of blood
transfusion is common and was due to too
high a transfusion trigger, as well as too
many units transfused
Packed red blood cell transfusion after surgery: are we ‘‘overtranfusing’’ our patients?

26. The AABB recommends a RESTRICTIVE RBC TRANSFUSION THRESHOLD:
• Hb level 7g/dL for hospitalized adult patients who are hemodynamically stable,
including critically ill patients
(strong recommendation, moderate quality evidence).
• Hb level 8g/dL for patients undergoing orthopedis surgery, cardiac surgery and
those with preexisting cardiovascular disease
(strong recommendation, moderate quality evidence).
Conditions for which the evidence is insufficient : acute coronary syndrome, severe
thrombocytopenia, and chronic transfusion–dependent anemia.

comorbilidades
– Transfundir CH siempre de 1 en 1

RESULTS The risk ratio (RR) for the association between transfusion strategies and serious infection was 0.82
(95%CI,0.72-0.95) The number needed to treat (NNT) with restrictive strategies to prevent serious infection was 38
(95%CI, 24-122).
CONCLUSIONS AND RELEVANCE Among hospitalized patients, a restrictive RBC transfusion strategy was
associated with a reduced risk of health care–associated infection compared with a liberal transfusion strategy.

preoperatoria ?
44. • Anemia preoperatoria hasta 39% en cirugía no cardíaca. Hasta 57% en ca colorectal. Criterios WHO
• Pruebas diagnósticas de laboratorio: HEMOGRAMA, Ferritina sérica, Sat. Transferrina, Cr, PCR, B12, fólico, LDH.
