GO MOMS is a team of Ob/Gyns from the San Francisco Bay Area who use simulation
techniques to train health care providers in Central America. This guide is comprised of
resources obtained through various organizations including ACOG, WHO, FIGO, and
AAGL, and includes instructions for making simulation models. These tools can be used
to practice and to train others on new procedures.

Hemorragia Postparto/Postpartum Hemorrhage
Aproximadamente el 30% (más del 50% en algunos

Approximately 30% (and more than 50% in some
countries) of maternal deaths worldwide result
from hemorrhage; in the majority of these cases
hemorrhage occurs during the postpartum period.
Definition of Postpartum Hemorrhage:
Blood loss greater than 500 mL after a vaginal
delivery or exceeding 1 L after a Cesarean
section
Signs and symptoms of hypovolemia:
Tachycardia is an early indicator, followed by
a decline in blood pressure, pallor, sweating,
and loss of warmth in the extremities.
Symptoms can include fainting/dizziness,
nausea, and thirst.
Causes of PPH:
Uterine atony – the cause of approximately
70% of PPH cases
Retained placenta
Placenta accreta
o Increasing in incidence due to rising
rates of C-section
o Number of prior uterine incisions
increases the risk of placenta accreta
Lacerations – vaginal or cervical, uterine
rupture
Uterine inversion
Coagulation disorders
o Acquired – Severe preeclampsia/HELLP
syndrome, placental abruption,
disseminated intravascular coagulation
(DIC)
o Congenital – Von Willebrand disease,
inherited thrombocytopenia
Uterine blood flow: 10% of cardiac output

Es difícil predecir quien presentara HPP basándose
en factores de riesgo porque dos tercios de las
mujeres que cursan con HPP no tienen factores de
riesgo.
Prevención de la hemorragia postparto:
Manejo activo de la tercera etapa del parto
(MATEP) -- La incidencia de HPP con MATEP es
5.9%, en vez de la incidencia de hemorragia
postparto con manejo expectante que es 17.9%
Los componentes habituales del MATEP incluyen:
Administración de oxitocina dentro de 1
minuto luego del nacimiento del bebe
Tracción controlada del cordón
Masaje uterino después de la expulsión de
la placenta
Cuando la tercera etapa del trabajo de parto dura
más de 30 minutos se presenta HPP seis veces más
a menudo de lo que ocurre entre las mujeres cuya
tercera etapa duro menos de 30 minutos.
Los siguientes medicamentos se recomiendan para

Risk Factors for PPH:
Prolonged third stage of labor
Multiple pregnancies
Fetal macrosomia
History of postpartum hemorrhage
Pre-eclampsia
Episiotomy
Obesity
Chorioamnionitis
Coagulation disorders
It is difficult to predict if a woman will
hemorrhage based on risk factors because 2/3 of
women with PPH do not have any risk factors.
Preventing Postpartum Hemorrhage:
Active management of the third stage of labor
(AMTSL) – the incidence of PPH during active
management of the third stage of labor is 5.9%,
compared to the incidence of PPH during
expectant management which is 17.9%
Components of active management include:
Administration of oxytocin within one minute
of birth
Controlled cord traction
Uterine massage after delivery of the
placenta
When the third stage of labor lasts more than 30
minutes, PPH is six times more likely to occur than
among women whose third stage lasts less than
30 minutes.
The following pharmaceuticals are recommended
for the prevention and treatment of PPH:
Oxytocin is the drug of choice
o 10 IU IM or 5 IU IV slowly or 20-40 IU/L
IV infusion
Ergot alkaloids

o 0.2 mg IM, que puede repetirse

Otras opciones
o Carboprost -- 0.25 mg IM cada 15
minutos (2 mg como máximo)
o Sintometrina – 1 ampolla por vía IM
o Carbetocin
o Ácido tranexámico—1 gram IV
(Nueva recomendación)
Manejo de la hemorragia postparto:
Administre el uterotonico cuanto antes
posible
Vacié la vejiga. Es posible que la mujer
pueda hacerlo por sí sola, o que necesite
colocar un catéter
Aplique oxígeno, si hay disponible
Obtenga ayuda y realice una infusión IV con
una cánula de calibre grueso, y analice la
posibilidad de iniciar una segunda infusión

o 0.2 mg IM, which can be taken every 2
to 4 hours for a maximum of 5 doses (1
mg) in 24 hours
o Ergot alkaloids are contraindicated in
women with hypertension, heart
disease, or preeclampsia as they cause
hypertension
Misoprostol
o Studies have shown that a single dose
of 800 μg (4 tablets of 200 μg)
misoprostal administered sublingually is
a safe and effective treatment for PPH
caused by uterine atony in women who
have received oxytocin as well as those
who have not received oxytocin during
the third stage of labor
Other options:
o Carboprost -- 0.25 mg IM every 15
minutes (maximum dose of 2 mg)
o Syntometrine – 1 ampule IM
o Carbetocin
o Transexamic Acid – 1 gram IV (New
recommendation)
Postpartum hemorrhage management:
Administer uterotonic agents as early as
possible
Empty the bladder. It is possible the patient
can void on her own, or she may need to be
catheterized
Apply oxygen, if available
Get help and place a large bore IV, and
consider the possibility of starting a second IV
If the bleeding persists after administering
uterotonics, consider the possibility of performing
the following procedures:

Si persiste el sangrado luego de la administración

Bimanual uterine compression (external or
internal)
Intrauterine balloon tamponade
o A balloon tamponade can stop bleeding
in 77.5% to 88.8% of cases without
needing surgical treatment. Other

Anti-shock garment for the treatment of
shock, pending transfer to a higher level of
care hospital or while waiting for a surgical
intervention
Aortic Compression:
Aortic compression is a simple procedure that
can be used while waiting for treatment or
for transfer to another hospital
Aortic compression can help limit the amount
of blood loss by decreasing blood flow to the
uterine artery
Aortic compression is accomplished by
applying pressure with the flat surface of the
knuckles above and slightly to the left of the
collapsed uterus
The absence of a femoral pulse confirms a
proper pressure placement and complete
occlusion of the aorta
It is crucial to release and reapply the
pressure every 30 minutes to allow blood to
flow to the lower limbs

Intrauterine Balloon Tamponade:
Condom Balloon Model and Simulation
Materials:
Balloon Tamponade:
Condom
Foley catheter, any size
Piece of spare suture or string
Sterile water or saline
60cc syringe or similar
Hemostat or small clamp
Uterus:
Neoprene uterus from B-Lynch
simulation if available
Preparations:
1. Fill the Foley catheter tip balloon bulb with 5-10cc of water.
2. To secure the condom over the end of the Foley, open the condom and fit the condom
over the end of the Foley.
3. Secure the condom in place by firmly tying with a piece of suture or string. The
string must be tied tight enough to stop water from leaking out of the condom, but not
so tight that water cannot be pushed up through the Foley.
4. Through the urine drainage port, fill the condom with an initial 60cc of water to test for
leaking.
Instructions for using the condom tamponade (Refer to the videos on the website):
1. Introduce the condom balloon catheter into the uterus
2. Inflate the condom through the urine drainage port using water or saline
a. 250-500cc water as needed to fill uterine cavity and bleeding stops
b. Assess with bimanual exam
c. Use the hemostat to clamp off the catheter between fillings
3. Document the amount of fluid used in order to later confirm complete
drainage before removing.
4. Keep hand in vagina to make sure the balloon tamponade stays inside the uterus
5. Then pack the vagina with moistened vaginal packing or gauze to keep the balloon
tamponade in place. Be sure to count the number of gauze to assure complete removal
6. Mark the top of the fundus and check frequently to assure the mark is not rising
as indication that blood is collecting in the uterus
7. Remove the balloon tamponade in maximum 24 hours (can remove before 24 hours if
the patient is stable)

Balloon tamponade is primarily used for bleeding post-vaginal delivery, but it also can be used
for post-operative delivery. Place the balloon into the uterine cavity prior to closing the uterine
incision and guide the other end of the catheter through the cervical opening into the
vagina and out. Close the uterine incision in the usual fashion but be extremely careful not
to incorporate the condom as it is non-absorbable. Inflate the balloon and check for vaginal
bleeding.

Hemostat or small clamp
Absorbable suture only, size 1-0 or 0, with large
needle (ex. CT or CTX)
Preparation:
1. Turn the wash mitt inside out with the opening at
the bottom. Secure the bottom of the mitt to the
table using the clamp.
2. Use a marker to denote where the hysterotomy
incision would be located for reference while
stitching.
Simulation:
Refer to the B-Lynch diagram and the GO MOMS video on the website.

needle. We suggest CTX. It is important that only
absorbable suture is used for this procedure, so
Vicryl and Chromic are great options.
Place a Kelly at the end of the suture so that the
surgeon does not pull the suture all the way

Then go to the back side of the uterus. The
second stitch will be horizontal and mirror where
the hysterotomy is; sewing can be done either in
one or two stitches.
Now having a partner becomes important
because the surgeon should not pull with the
suture – it will tear through a postpartum uterus.
Instead, the partner should push and compress
the uterus down.
The third stitch, again, is placed 3 cm above the
hysterotomy and comes out 3 cm below. The
partner helps the surgeon by pushing down the
uterus as far as possible. Some people roll it and
others push so that the surgeon can make a nice,
tight closure. Then tie the suture.
There is the option to close the hysterotomy
either before or after the placement of the B-
Lynch. We prefer to close the incision before
placement of the B-Lynch, but if the surgeon
choses to close the incision after placing a B-Lynch
suture, be sure to close the hysterotomy
completely.
B-Lynch suture can also be placed for PPH on an
intact uterus after a vaginal delivery. At time of
laparotomy, the surgeon can envision where the
hysterotomy line would be to guide suture
placement.

Vaginal Pessary:

El dedo examinador debe pasar fácilmente
entre la vagina y la circunferencia del pesario
Pedir a la paciente que realice Valsalva (Esta
bien ver el borde del pesario en el introito
durante la fuerza máxima)

There are many types of pessaries available; the
most common are the incontinence ring (used
for support) and the donut (used to fill space).
Indications for Pessary:
Prolapse
Urinary incontinence
Urinary retention: relieve obstruction and
improve renal function
Before surgery: allow inflammation and
ulceration in procidentia to heal
preoperatively
Failed surgical correction
Pessary is most beneficial for patients who prefer
nonsurgical management and for patients who
have significant risk of surgical morbidity.
Contraindications for Pessary:
Severe erosions
Active vaginitis
Pelvic inflammatory disease
Non-compliant patient (unable to return for
exams)
Fitting a Pessary:
First the patient should empty her bladder
Use the largest pessary that can be
comfortably placed
Examiner’s finger should easily pass
between vagina and circumference of
pessary
Have patient perform Valsalva (with
maximum force, fine to see pessary edge at
introitus)
Patient should be able to sit, stand, urinate,
and defecate with pessary in place (not
necessary to do all of these at initial visit)

Seguimiento:

Cultivos vaginales no se recomiendan

estrogenizar, insertar nuevo pesario cuando la

Atrofia Vaginal:

Follow Up Visits:
After initial fitting: Return in 1-2 weeks
(specifically ask questions about comfort,
voiding, defecation, discharge, and ease of
care)
For the 1st year: return every 3 months
After the 1st year: return every 6 months (if
the patient is removing the pessary
regularly—at least once a week—can return
every 12 months)
Pessary Management:
Remove pessary and clean with soap and
water
Speculum exam: evaluate for abrasions,
erosions, vaginal discharge, atrophy.
Irritation is common, full thickness ulcer is a
problem
Common Problems:
Pessary expulsion
New onset urinary incontinence (20%)
Rectal pain
Difficulty with defecation
Vaginal discharge
Vaginal atrophy
Ulceration with bleeding
Impaction
Vaginal Discharge:
Some vaginal discharge is expected
Increased risk for bacterial vaginosis (4X)
Vaginal cultures are not recommended
If discharge is a problem, remove,
estrogenize, insert new pessary when
irritation is gone
Vaginal Atrophy:
Local estrogen therapy is recommended in
pessary users to prevent ulcers, abrasion,
incarcerations

Mejor predictor de aceptabilidad: edad.
Mujeres mayor que 65 años cumplen con uso
de pesario prolongado
Si la paciente prefiere cirugía desde la visita
inicial o si ella tiene prolapso de pared
posterior etapa 3 y 4, ella va a discontinuar el
uso
If significant atrophy is present, use vaginal
estrogen for 6 weeks prior to starting
pessary use
Ulceration:
Remove pessary
Intravaginal estrogen (0.5-1.0 gm/day) for
2-3 weeks
Replace pessary if healed
Continue local estrogen 2-3 times per week
Recurrent/persistent ulcers biopsy
Impaction:
More common with space filling pessaries
Use a clamp or tenaculum
Apply local estrogen 2-3x per week and
then attempt removal
May require removal in the OR (rare)
Severe Complications:
Incarceration of pessary
Vesicovaginal fistula
Rectovaginal fistula
Neglected pessary + noncompliant patient =
potential for severe complications
Patient Satisfaction:
After 2 months, 94% are satisfied
After 1 year, 73% are satisfied
After 2 years, 64% are satisfied
Best predictor of acceptability: age. Women
over 65 are more likely to comply with
pessary use for longer time
If patient desires surgery from the
beginning or has stage 3 or 4 posterior wall
prolapse, more likely to discontinue use of
pessary

Expand the hose clamp to fit around the
foam can holder
Tighten the clamp so that it is slightly
snug
The clamp should be kept at the
12 o’clock position and is landmark for
the pubic arch or symphysis pubis
Instructions (Refer to the video on the website):
1. Apply lubricant to the pessary and
distribute it evenly over the pessary
before placement
2. Gently squeeze the pessary so that it can
be inserted into the vaginal canal
3. Push so that the pessary is at the end of
the canal; once the pessary is in place it
should be oriented vertically toward the
clamp or arch
4. To remove the pessary, put one finger
inside the vaginal canal and hook a
finger into the center of the pessary;
with gentle traction it can be pulled out

Preparaciones:

Póngala un poco ajustada
La orientación para esto será que la
abrazadera en si este a las 12 en punto,
simulando el arco púbico
Instrucciones:
1. Ponga un poco de gel en el pesario,
aplicándolo a todo el rededor.
2. Coloque el pesario dentro del canal
vaginal, simplemente empujándolo
suavemente hacia adentro; se debe
deslizar con el gel.
3. Cuando este correctamente montado, su
orientación será completamente vertical.
4. Para sacar el pesario, coloque un dedo
dentro del canal vaginal, enganche el dedo
en el centro del pesario tipo dona, y con
una suave tracción podrá sacarlo.

Vaginal Hysterectomy:
Simulation Model of Complete Uterus with Ligaments,
Blood Vessels, Tubes/Ovaries and Vagina
Materials:
Uterus and cervix:
1 pool noodle 2.5” diameter
Ligaments and blood vessels:
8 long, full length balloons for 4 pairs
Ovary/utero-ovarian ligament and fallopian tube:
2 cotton balls
2 long balloons, cut in half for 2 pairs
Vagina/vaginal canal:
8 x 12” piece of 4 way stretchable fabric (used for swim suits)
Bladder:
2 x 2” piece of cotton cloth
Tools:
Scissors/knife
Sewing machine
Spray glue
Press and Seal wrap
Needle and thread or any available suture with needle
Instructions:
1. Cut the pool noodle into 6 inch pieces, 1 piece = 1 Uterus/cervix
2. Use scissors or knife to taper the last 1-2” of each piece
3. Use sewing machine to sew the 8 x 12” stretchable cloth lengthwise to make a tube
approx. 2.5-3” diameter
4. Turn the tube inside out and place the pool noodle inside with the tapered end about 1”
from the end
5. Fold the fabric back over, spray with glue and fold back
6. Pull fabric down to expose pool noodle
7. Using needle with thread or any available suture, attach the full-length long balloons in
pairs (4 x 2 = 8) to the pool noodle at the correct anatomical position. The balloons
represent:
a. uterosacral ligaments
b. uterine arteries
c. cardinal ligaments
d. round ligaments
8. Cut 1 full-length long balloon in half to make utero-ovarian ligaments

9. Attach one to each cotton ball ovary, then attach to the pool noodle in the correct
anatomical position
10. Cut 1 full-length long balloon in half to make fallopian tubes and attach them to the pool
noodle in the correct anatomical position
11. Glue the 2 x 2” piece of cotton cloth to the lower anterior uterus
12. Cover the entire model with Press and Seal
