ESTAMOS HABLANDO DE PREVENCIÓN SECUNDARIA. EN estas circunstancias,está demostrada la utilidad de aspirina enla prevención de eventos y disminución de morbi-mortalidad CV.
Pero surgen muchs preguntas
There are multiple ways by which platelets stick together and contribute to blood clot formation. Whereas aspirin blocks an enzyme that leads to sticky platelets, there are also receptors on the platelet surface that, when acti- vated, lead to platelet adherence and clot formation (Figure 3). One class of medications that block platelets does so by inhibiting a certain receptor (known as the P2y12 receptor) on the surface of platelets. This class includes medi- cations such as clopidogrel (Plavix), prasugrel (Effient), and ticagrelor (Brilinta). When DAPT is prescribed, it usually consists of aspirin plus 1 of these 3 drugs. DAPT leads to greater platelet inhibition than that achieved by aspirin alone. By decreasing the likelihood of blood clot formation inside coronary arteries, at the site of ruptured plaques, or on stents, DAPT decreases the likelihood of future heart attacks. Therefore, DAPT is an essential component of the medical regimen of certain patients with heart disease, specifically those with recent heart attacks or stents. LAS PLAQUETAS CONTRIBUYEN A LA FORMACIÓN DE COÁGULO POR MÚLTIPLES VÍAS. ASPIRINA Y LOS INHIBIDORES DEL RECEPTOR P2Y12 UTILIZANA VIÁS DISTINTAS. PERO CUANTO TIEMPO HAY QUE MANTENERLOS?
Compared with a standard 12 month duration, short term DAPT (<12 months) after drug eluting stent implementation yields reduced bleeding with no apparent increase in ischaemic complications, and could be considered for most patients. In selected patients with low bleeding risk and very high ischaemic risk, extended DAPT (>12 months) could be considered. The increase in all cause but not cardiovascular death with extended DAPT requires further investigation.

Hasta entonces estábamos hablando de el uso de estatinas en prevención primaria, pero aquí se nos coló la aspirina como tratamiento fundamental en los diabéticos

Aspirin has been shown to be effective in reducing cardiovascular morbidity and mortality in high-risk patients with pre- vious MI or stroke (secondary prevention). Its net benefit in primary prevention among patients with no previous cardio- vascular events is more controversial, both for patients with and without a history of diabetes (311). este estudio que se diseñó para valorar la mortalidad cardiovascular, se presentó como el 1º estudio que logró resultados positivos cuando en realidad se finalizó sin demostrar su objetivo principal.
A partir de aquí se establece un precedente: ASPIRINA EN PREVENCIÓN PRIMAIRIA PUEDE disminuir el nº de eventos CV
Historia de la antiagregación en prevención primaria En los años 70 comenzaron los estudios observaciones ( poner algunos estudios),….
Comenzaron los ensayos clínicos y la proliferación de guias clínicas que extrapolan el riesgo cardiovascular de IRC, diabetes…. y en las guias clínicas ase inicia la recomendación de uso en prevención primaria (ADA 2008, HTA 2005,….) posteriormente estudios en los que se investiga el uso de guias clínicas. Se comprueba que no se sigue y se sugieren actuaciones para mejorar su seguimiento.

LA CUESTIÓN MÁS IMPORTANTE ES SI LOS DIABETICOS SIN INFARTO PREVIO TIENEN UN RIESGO DE INFARTO SIMILAR A LOS NO DIABÉTICOS CON INFARTO.
SI ES CIERTO, ESTO SUGIERE UN RIESGO CV BASAL SIMILAR Y QUIZÁS ESTÉ INDICADO EL MISMO TRATAMIENTO DE LOS FACTORES DE RIESGO CV.
NUESTROS DATOS INDICAN UNA SIMILAR INCIDENCIA DE ENFERMEDAD CARDIOVASCULAR EN ESOS GRUPOS DESPUÉS DE AJUSTAR POR EDAD, SEXO Y OTROS FACTORES DE RIESGO.
COMO ES UN ESTUDIO DE BASE POBLACIONAL, ESTOS RESULTADOS PUEDEN SER GENERALIZABLES.
EN CONCLUSIÓN, HEMOS MOSTRADO QUE LOS PACIENTES CON DIABETES TIPO 2 QUE NO HAN TENIDO UN INFARTO TIENEN SIMILAR RIESGO QUE LOS NO DIABÉTICOS CON INFARTO PREVIO………. SUGIERE QUE TODAS LAS PERSONAS CON DIABETES DEBERÍAN SER TRATADOS COMO SI TUVIERAN ENF. CORONARIA ESTABLECIDA.
Esta conclusión se basó en una relación de riesgos no significativa entre diabéticos y no diabéticos. No encontro distinción de riesgos durante 7 años de seguimiento. Este estudio tuvo gran impacto en la literatura científica y fue citado >3500 veces hasta 2010 y justificó el uso de medidas de prevención cardiovascular con la misma intensidad que en prevención secundaria.
EL METAANALISIS DE LANCET 2009 SE HA CITADO HASTA AHORA EN 1146 OCASIONES (7 junio 2015)
No se ha podido reproducir estos resultados. Aunque se publicó en 1998, se comenzó antes del UKPDS y la media de glucemia era 210, todos estaban tratados con fármacos (indicando un grado mayor de severidad en la diabetes. Cómo se midió la enf. coronaria: también. (admitidos con dolor precordial) se excluyeron las muertes súbitas. recientes estudios encuentran que el riesgo cv de diabéticos es . 0,53-0,6 respecto de no diabéticos con IAM.

EN BASE fundamentalmente al estudio anterior, en 2001 EL 3º informe del NCEP sobre tratamiento de la hipercolesterolemia iguala el riesgo cv d diabetes al de infarto previo. (prevención secundaria).
De aquí a introducirlo en otras guías es sólo cuestión de tiempo.
ADA en 2003 ya lo incluye y comienza su recomendación de tto antiagregante.
Pero no será hasta 2007 cuando ADA yAHA publican unas recomendaciones conjuntas indicando la antiagregación con aspirina a dosis bajas en todos los diabéticos mayores de 40 años o con uno o más factores de riesgo cv.

LOS META ANALISIS DE PREVENCIÓN PRIMARIA
A number of reasons seem to justify this change in recommendations. There have been major advances in treatment approaches to type 2 diabetes, replica- tions of the finding by Haffner et al. are inconsistent, and recent data suggest that the utility of aspirin for CHD prevention may be of a lesser magnitude than pre- viously believed. First, it should be pointed out that even though the article by Haffner et al. was published in 1998, the data used for this analysis were generated in Finland between 1982 and 1990 (1). This was the pre-Diabetes Con- trol and Complications Trial (DCCT) and pre-UK Prospective Diabetes Study (UKPDS) era, and the mean fasting glu- cose value of 210 mg/dL in individuals with diabetes and no CHD history in the Finnish cohort reflects this fact, and this value would be inconsistent with the level of glucose control currently recom- mended (4–6). Also, all diabetic subjects were medication treated, thereby rep- resenting a group with greater disease bur- den than would be seen in all individuals with diabetes, including those maintained on lifestyle therapy. Third, presence of prior CHD was sought only in individuals who reported a previous hospital admis- sion for evaluation of chest pain. Given that diabetes is known to be associated with a higher frequency of silent myo- cardial infarction, this strategy may have resulted in misclassification of past myocardial infarction.
Perhaps not surprisingly, attempts to replicate the finding of Haffner et al. have for the most part been unsuccessful.
One could argue as to why aspirin use was recommended at all for individuals with diabetes given the lack of clearly demonstrated benefit. We think that the counterargument is that definitive re- search to rule out a benefit has not been performed, and that currently available data suggest a small but as yet unproven effect that can be balanced against poten- tial harms for individual patients.

Comparando con estudios similares en otros países de media y alta tasa de mortalidad cardiovascular, sugiere que existen variaciones geográaficas. En Europa existe un gradiente norte -sur en las tasas de incidencia y mortalidad por IAM.

The absolute risk of vascular complications is the major determinant of the absolute benefit of antiplatelet prophylaxis. CUANDO HABLAMOS DE PREVENCIÓN PRIMARIA
En los siguientes años 70 y 80, se acumula evidencia de su eficacia en la disminución de morbimortalidad en prevención secundaria.
En 1989 aparece el primer ensayo cl´nico en prevención primaria.
Tras el informe final del JNC de 2001, en 2003 la ADA comienza a recomendar la aspirina en prevención primaria. En 2007 sale las recomendaciones conjuntas con la AHA

COCHRANE: our systematic review (that finds a 13% lowering of myocardial infarction, a one third increase in bleeding, and no benefit on mortality outcomes) differs from these two previous studies. We suggest that the benefits of antiplatelet agents on cardiovascular events may be smaller in people with CKD compared with other populations at risk of cardiovascular events, for whom reductions in cardiovascular mortality, myocardial infarction, and stroke are one sixth, one third, and one quarter respectively.
efficacy for antiplatelet agents on total mortality in CKD is potentially explained by the competing mechanisms for cardio- vascular death in this population. Progressive kidney dysfunction is characterised by vascular stiffening and calcification, cardiomy- opathy, hyperkalaemia, and sudden cardiac death, in addition to occlusive vascular disease. About half of cardiovascular deaths in both dialysis and transplant patients are caused by cardiac arrest and heart failure ((ANZDATA 2009) for which the predominant pathogenetic mechanisms include hypertension, volume expan- sion, vascular calcification, and arrhythmia, rather than platelet aggregation and thrombosis. Therefore, while we find that an- tiplatelet agents prevent occlusive vascular events (myocardial infarction) in CKD as expected, they have a lower overall effect on nonthrombotic causes of death (both vascular and nonvascular). The results of the present review are consistent with the effects of antiplatelet agents in primary prevention of cardiovascular events, which reduce nonfatal myocardial infarction by 20% but do not prevent stroke or vascular mortality with similar effects in men and women (ATT 2009). Notably, in that review, the authors concluded that aspirin may be of uncertain net value, because reducing occlusive events may not be outweighed by risks of major bleeding.This systematic review has shown that antiplatelet treatment in people with CKD for approximately 12 months reduces the risk of myocardial infarction, but not stroke, or all-cause or cardiovascular mortality. Antiplatelet agents given at the time of access surgery reduce thrombosis or failure of vascular access, but effects on suitability for dialysis and access maturation remain unclear. Major bleeding is increased by one third across all antiplatelet drugs and stages of CKD. The relative benefits of treatment in kidney transplant recipients and with primary prevention strategies in CKD are in- sufficient to inform practice.
This table shows that while antiplatelet treatment in people a thigh risk of myocardial infarction (dialysis patients) is reasonable (preventing 32 myocardial in- farctions in 1000 patients treated for one year versus incurring seven major bleeding events), the harms of antiplatelet treatment probably exceed the benefits for people with lower annual risks of vascular events (transplant recipients or stage 1 to 2 CKD; two myocardial infarctions prevented per 1000 patients treated for one year versus seven major bleeding events), assuming that myocardial infarction and major bleeding are equivalent outcomes. Insufficient information about the nature of major bleeding events incurred (particularly intracranial haemorrhage) in the available studies will make a decision about risk and benefit for intermediate risk people incompletely informed. Overall, antiplatelet agents given for four to six months after vascular access surgery will substantially reduce the risk of access fail- ure and thrombosis (preventing 20 patients per 100 at risk experiencing access thrombosis or patency failure over six months of treatment at the risk of three to four experiencing a major bleed), although the effects of antiplatelet therapy of the important out- come of suitability for dialysis remain uncertain.

siguen recomendando en pacientes de riesgo alto el tto con aspirina. pero los ensayos de prevención primaria se han realizado en pacientes con múltiples FRCV. A pesar de ello, no encuentran diferencias. y entre las conclusiones está el bajo nº de eventos en el grupo control. Es decir que personas a priori con RCV elevado, en los ensayos clínicos tienen un riesgo mucho menor.
ESTÁ LA INERCIA TERAPEUTICA: CÒMO HAN AFECTADO LAS GUÍAS CAMBIANTES A LOS MÉDICOS?
La decisión de los pacientes en la práctica clínica se ve influenciada por la manera en que se presenta la información EFECTO ENCUADRE. No es lo mismo decir que un procedimiento quirúrgico tiene un 90% de éxitos que enfatizar el 10% de muertes.
El efecto encuadre se produce a 2 niveles:
1.- Los médicos se ven influenciados por la manera en que se presentan los datos en los ensayos clínicos y revistas.
2.- Los pacientes están influidos por los médicos.
Un paciente o un medico puede rechazar una intervención que reduce la morbilidad CV en un 25% si se presenta como que mejorará la probabilidad de estar sano del 1% desde el 96 al 97%.
Independiente- de cómo se presentan los datos, hay que tratar a muchos pacientes para beneficiarse unos pocos. Cuando la intervención está libre de riesgos, este aspecto es menos importante

Aspirin is not licensed for the primary prevention of vascular events. If aspirin is used in primary prevention, the balance of benefits and risks should be considered for each individual, particularly the presence of risk factors for vascular disease (including conditions such as diabetes) and the risk of gastrointestinal bleeding.
