• 665 pacientes 1992 a 2001, randomizados a 6

mg/mt2 o no, posterior a 45 Gy a 1,8 Gy de
RT concurrente 5FU-LU semanas 1-5.
• Fallo no hay diferencias en SG y SLE.

quimioterapia 5 FU/L –Capecitabina vs
observación posterior a RMR, previa a RT(QT).
• 470 pacientes, seguimiento 5 años,

[hazard ratio (HR) 0.93, 95% intervalo de
confianza (CI) 0.62-1.39; P = 0.73]. Sobrevida
libre de enfermedad 0.80 (95% CI 0.60-1.07; P
= 0.13). Recurrencia a distancia (P = 0.39)
CONCLUSION: The PROCTOR-SCRIPT trial could not demonstrate a significant benefit
of adjuvant chemotherapy with fluoropyrimidine monotherapy after preoperative
(chemo)radiotherapy and TME on overall survival, disease-free survival, and
recurrence rate. However, this trial did not complete planned accrual.
27. Chronicle: results of a randomised phase III trial in locally
advanced rectal cancer after neoadjuvant chemoradiation
randomising postoperative adjuvant capecitabine plus oxaliplatin
(XELOX) versus control

QT en base a 5FU.
• Resultados a 3 años SLE XELOX vs observación
[hazard ratio (HR) for DFS = 0.80; 95%
confidence interval (CI) 0.38-1.69; P = 0.56].
• a 3 años SG XELOX vs observación (HR for OS =
1.18; 95% CI 0.43-3.26; P = 0.75).
• CONCLUSIONES: no hay diferencias.

• INTERPRETATION: Overall, adjuvant fluorouracil-
based chemotherapy did not improve overall
survival, disease-free survival, or distant
recurrences. However, adjuvant chemotherapy
might benefit patients with a tumour 10-15 cm
from the anal verge in terms of disease-free
survival and distant recurrence. Further studies of
preoperative and postoperative treatment for
this subgroup of patients are warranted.
Adjuvant chemotherapy after preoperative (chemo)radiotherapy and surgery for

in subgroup analyses, patients with a tumour 10-15 cm from the anal verge had
improved disease-free survival (0·59, 0·40-0·85; p=0·005, pinteraction=0·107) and
fewer distant recurrences (0·61, 0·40-0·94; p=0·025, pinteraction=0·126) when
treated with adjuvant chemotherapy compared with patients undergoing
observation.
30. • Guidelines from the NCCN recommend that all such
patients receive chemotherapy even if they have a
pathologic complete response (pCR) to neoadjuvant
therapy.
• European Society of Medical Oncology (ESMO)
recommendations state that, similar to the situation in
colon cancer (stage III and "high-risk" stage II), adjuvant
chemotherapy can be provided even if the scientific
support for sufficient effect is less
• In contrast, experts convened by a European Rectal Cancer
Conference concluded there is insufficient evidence on the
benefit of adjuvant chemotherapy after preoperative
chemoradiotherapy to recommend its use .
33. Conclusiones
1- RT QT neoadyuvante es un estándar en pacientes con
neoplasia de recto.
2.-Se puede omitir la cirugía en paciente seleccionados
con un estricto seguimiento.
3.-El fraccionamiento de 5X5 es utilizado por algunas
instituciones.
4.-Lo ideal es realizar la cirugía a las 7 semanas post
neoadyuvancia
5.- No hay evidencia contundente para realizar
quimioterapia neoadyuvante, peso los consenso
sugieren realizar 4 a 6 ciclos en base a fluropimidinas.
