External fixation of mandible fractures is a technique in which segments are manipulated inplace by pins and then fixated with some type of connectors
Excluyendo las fracturas de huesos propios nasales, las fracturas de la mandibula corresponden al 60% de la totalidad de fracturas que ocurren en la cara
Variationsof this principle were employed for a long time, beingintroduced into Germany by Rutenick in 1799, who appliedfurther stabilisation by a head harness attached to a helmetby ribbons.9está documentado el manejo de una fractura a nivelde la sínfisis mandibular en 1822, causada por armade fuego, al general Tomás Cipriano de Mosquera,quien fuera Presidente de la República, fijación quefue realizada con una placa de oro, la cual terminó enpseudoartrosis5.
En el museo de Medicina de Paris existe el modeloreprestantivo de un defecto mandibular por resección de ameloblastoma manejado con placa de reconstrucción en hierro.
La mandibula deriva del primer arco faringeo, a partir de la 4ta semana celulas de la cresta neural migran ventralmente conformando un centro primario central denominado cartilago de merckel que da la estructura básica inicial. Lateralmente el día 48 en la bifurcación del nervio alveolar inferior se forma el centro de osificación mandíbular que permite el desarrollo superior e inferior mandibular. Progresivamente y a partir de centros superiores entre la semana 7 y 10 se da la formación de la artículacióntemporomandibular que con el inicio de los movimientos mandibulares obtiene su conformación articular típica con la fosa glenoidea. Entre la semana 10 y 14 los centros mentonianos, coronoideo y condilares completan la estructura básica de la mandíbula. Hacia la semana 24 el cartílago de Meckel se calcifica y de ahí en adelante el crecimiento de la mandibula es lineal con respecto a la edad gestacional.

The medial pterygoid muscle originates in the pterygoidfossa, inserts on the medial surface of the ramus and angle of the mandible. function of the medial pterygoid muscle is upward, medial, and forward traction on the mandible.The lateral pterygoidmuscle,The innermost or upper portion pulls the mandible upward, medially, and forward; the external portion pulls the condyle downward, medially, and forward. Simultaneous contraction of the muscle segments on one side pulls the mandible to the opposite side. Simultaneous contraction of both lateral pterygoid muscles protrudes the mandible.

REQUIERE FORMACIÓN DE UN COÁGULO LOCAL, REABSORCIÓN DE LOS BORDES ÓSEOS POR DISRUPCIÓN HAVERSIANA, FORMACIÓN DE MATRIZ CARTILAGINOSA INICIAL (CALLO BLANDO) QUE POSTERIORMENTE SE CALCIFICA DANDO UN CALLO DURO.

Mandibular fractures that are directed downward and forward are classified as horizontally favorable because the posterior group of muscles and the anterior group of muscles pull in antagonistic directions, favoring stability at the site of the fracture. Fractures running from above, downward, and posteriorly are classified as horizontally unfavorable. The bevel of the fracture may also influence a displacement medially. If a fracture runs from posteriorly forward and medially, displacement would take place in a medial direction because of the medial pull of the elevator muscles of mastication (vertically unfavorable). The fracture that passes from the lateral surface of the mandible posteriorly and medially is a favorable fracture because the muscle pull tends to prevent displacement. It is called a vertically favorable fracture

LA fijación con arcos de Erich implica la medición inicial del arco, la colocación de asas de alambre circundentales primero a los premolares y posteriormente a los demás dientes, prefiriendo excluir aquellos de una sola raíz (anteriores) por la posibilidad de lesionarlos, distraerlos de forma ortodóntica entre otras. Una vez fijos los arcos en maxilar y mandibula el px es llevado a oclusión con elástico o alambre manteniendo una dieta líquida estricta. Se le explican recomendaciones puntuales de higiene oral.
