5. Introducción
1 estudio publicado (Ozbek et al) encontraron que
pacientes con absceso PA que recibieron CE mejoraba su
recuperación y mejoría de síntomas comparado con placebo.
A pesar de esta evidencia aun hacen falta estudios para
recomendar de manera rutinaria el uso de CE en absceso PA
Ensayo clínico controlado doble ciego, con placebo
6. Materiales y métodos
1 dosis IV de dexametasona (10mg) administrada después del drenaje de
un absceso periamigdalino resulta en disminución del dolor comparado
con placebo con el uso de una escala subjetiva del dolor.
Efecto de dexameasona DU en: apertura bucal, temperatura corporal,
tiempo sin dolor y tiempo para regresar a sus actividades normales
comparado con placebo.

Compromiso en vía aérea, espacio profundo, deshidratación severa.

20. Discusión
Although any age group can be affected, PTA is most common in
adolescents and young adults between the ages of 20 and 40 years.
During the past half century, with Streptococcus species being the most
common organisms cultured.
This methodologically rigorous trial evaluated the effectiveness of
dexamethasone compared to placebo
21. Discusión
A statistically significant difference in subjective pain scores was found at
the 24-hour post-treatment time point
Using a comprehensive search strategy, only one ther published study
was identified that assessed the efficacy of adjunctive steroids in PTA
treatment. Ozbeket al. compared intramuscular methylprednisolone
to placebo in a randomized prospective fashion.
The authors found a statistically significant difference in trismus
severity, dysphagia levels, body temperature, and hours hospitalized
favoring the use of steroids without noting any increase in frequency
of complications.
22. Discusión
No significant differences were noted in trismus severity or dysphagia
levels in this trial.
23. Limitaciones
Concluye prematuramente por la dificultad del seguimiento
Muestra pequeña grupos no equilibrados favorecen a grupo placebo
Sin seguimiento mas allá de 7 días efectos adversos pudieron ocultarse
24. Conclusión
En Absceso periamigdalino –> 1 dosis de dexametasona10mg IV (previo
AB IV y aspiración) reduce dolor subjetivo (VAS) comparado con solo
AB IV
Opción considerable en absceso periamigdalino con gran dolor
Sin embargo dado los beneficios a corto plazo el tratamiento debe
ponderarse pensando en posibles efectos adversos

. Patients verbally rated pain intensity as an integer from 0 to 10 (0 no pain, 10 worst possible pain)
Secondary outcomes included mouth opening (in centimeters),
body temperature (in degrees Celsius), and time to
painless PO intake (in hours). Mouth opening, as recorded as
interincisor distance, was measured with a standard 12-cm
ruler, with the smallest increments of measure being millimeters.
The incisal surfaces of the mesial upper and lower incisors
were used as reference measuring points in all patients.
Body temperature was measured with a standard oral thermometer
in units of degrees Celsius, with precision to the nearest
10th of a degree. Time to painless PO intake was measured
in hours and subjectively reported by patients in a take-home
diary.

at the 24-hour time point but not at the 48-hour

Despite its frequency, significant debate exists regarding
PTA management7; specifically, the method of abscess
drainage and the role of adjunctive corticosteroids. The
use of systemic corticosteroids as adjunctive therapy has
been reported as safe and effective, and has become
more commonplace in recent years
Post hoc power calculation confirmed that our sample of 41
patients was more than adequate to detect the observed
difference between treatment groups >80% of the time
