N: 295 pacientes y 6 hospitales fueron incluidos en el estudio.
Anemia presente al diagnóstico en el 38,6%, pero fue tratado sólo el 32,6%.

electiva no cardiaca: resultados del análisis de una cohorte multicéntrica de 2699
pacientes.

Cuatro centros. cirugía ortopédica (CO, n=1286), ginecológica (CG, n=207), urológica (CU,
n=381), de resección de cáncer de colon (RCC, n=678) y de resección de metástasis
hepáticas (RMH, n=147).
Un total de 947 (35%) pacientes presentaban anemia preoperatoria, aunque
hubo diferencias en función de la patología quirúrgica: CO, 26%; CG, 63%; CU, 9%;
RCC, 58%; RMH, 38%. De los pacientes con AP el 70% fueron mujeres (p<0.001).
Las alteraciones del status férrico observadas en el grupo con AP fueron: 43%
ferritina <30, 77% ferritina <100 ng/mL, 66% SAT <20 ng/mL, 37% PCR >5 mg/L.
Por el contrario, de los pacientes sin AP, solo el 35% fueron mujeres, aunque
presencia de valores anormales en los parámetros férricos fue también frecuente
en este grupo: 15% ferritina <15 ng/mL, 57% ferritina <100 ng/mL; 47% SAT
<20%, 26% PCR >5 mg/L.

The findings of the 1998 meta-analysis were confirmed, with small variations in some
estimates. Identification of 237 references. Thirty-six studies on 12,127 patients were
included: 23 showed a detrimental effect of PBT; 22 used also multivariable analyses,
and 14 found PBT to be an independent prognostic factor.
Pooled estimates of PBT effect on colorectal cancer recurrence yielded overall
OR of 1.42 (95% CI, 1.20 to 1.67) against transfused patients in randomized
controlled studies.
Stratified meta-analyses confirmed these findings, also when stratifying patients by site
and stage of disease. The PBT effect was observed regardless of timing, type, and in a
dose-related fashion, although heterogeneity was detected. Data on surgical techniques
was not available for further analysis.

46. Eighteen studies (9120 GC patients) were included, of which 36.3% received
transfusions. ABT was associated with increased all-cause mortality (OR, 2.17;
95% confidence interval [CI], 1.72-2.74; p < 0.001).
Dose response meta-analysis revealed that all-cause mortality was significantly lower in
patients transfused with 800 mL of blood than those transfused with >800 mL (OR, 0.58;
95% CI, 0.37e0.92; p ¼ 0.02; I2 ¼ 54%). ABT was also associated with increased cancer-
related mortality (OR, 2.57, p ¼ 0.011) and recurrence (OR, 1.52, p ¼ 0.017).
Conclusions: In GC patients undergoing curative surgeries, ABTs are associated
with a worse prognosis, including all-cause mortality, cancer-related mortality
and recurrence. PATIENT BLOOD MANAGEMENT should be investigated further
to minimize use of ABT.

blood transfusion.

92. PROTOCOLO OPTIMIZACION ANEMIA PERIOPERATORIA EN LA NEOPLASIA DE COLON
Consenso: Dra L. Mora (Coloproctología), Dra I. Roig (Hematología), Dra M Queralt Gorgas (Farmacia), Dra Ch. Laso
(Anestesiología)
1- Los pacientes con neoplasia de colon que presentan Hb < 13g/dl ó metabolismo de Fe alterado
en el momento del diagnóstico de la neoplasia son tratados con Fe IV (200mg/48h) en Hospital de Día de
Hematología durante las 4-6 semanas que suelen preceder a la cirugía.
2- Los pacientes que no pueden desplazarse al hospital para recibir tratamiento IV
se tratan con Fe OR (100 mg/d=Ferogradumet®, u 80mg/d=Tardyferon) hasta el día de la cirugía
5- El dia 30 de postoperatorio acuden a Consultas Externas de Cirugía con control analítico que contiene
Hb y metabolismo de Fe.
- Hb < 11g/dl, Fe EV 1g (Hospital de día Hematología)
- Hb 11-13 g/dl, seguir con Fe OR hasta conseguir Hb=13g/dl (control por médico de cabecera)
- Hb > 13g/dl, nada
3- El día de la cirugía, todos los pacientes (independientemente del valor de la Hb) reciben 200mg de Fe IV en
postoperatorio inmediato (URPA)
(dosis equivalentes a las pérdidas de Fe por la cirugía, aprox. 300-400cc de sangre)
4- El dia 1 de postoperatorio, en sala hospitalización, el cirujano solicita y valora control analítico con
Hb y metabolismo del Fe.

“For patients undergoing orthopaedic surgery expected to
develop severe postoperative anaemia we currently suggest IV
iron administration during the perioperative period”.
For all other surgeries no evidence-based recommendation can be made.
We strongly recommend that large prospective randomised controlled
trials are undertaken in patients undergoing surgery expected to develop
severe post operative anaemia.

Respect to control, very short-term perioperative IV iron
administration, with or without rHuEPO, significantly reduced
(*p<0.01):
No clinically relevant AEs were observed.
The scheduled IV iron dose (200-600 mg) may not cover total iron
loss, especially in patients with preoperative iron deficiency.
Preoperative rHuEPO was only administered in 351 out of 1059
patients presenting with Hb level <13 g/dL and no contraindication.

in the early postoperative period for reducing ABT
rate or hastening the recovery from anemia

• In 6 out of 7 RCTs of non iron deficiency patients who underwent
elective or non elective orthopedic surgery or cardiac surgery, oral
iron supplementation for 4-10 weeks did not improve Hb levels
with respect to placebo.
• Moreover, only 1 out of 7 RCTs patients recovered baseline Hb
levels after 8 weeks on oral iron.
• Up to 30% of patients experienced adverse side effect to oral iron
(mostly gastrointestinal). Up to 10% of patients discontinued
therapy due to side effects.

- 88% de los pacientes intervenidos de neo. colon presentaron
ANEMIA en el postop inmediato (45% MODERADA-SEVERA)
- No se administró Fe EV a un 68,6% de los pac que lo tenían prescrito
- El cumplimiento estricto del protocolo de ferroterapia fue del 10,2%
- La implementación de la Trayectoria Clínica ha supuesto una mejora
en el manejo de la anemia postoperatoria
- La prevalencia de la anemia a los 30 días de la cirugía es > del 50%
- Es necesario incrementar el uso del Fe en el postoperatorio de los
pacientes con cáncer de colon
