Se ha estudiado a 286 pacientes, de los que el 26,1% desarrolló IN, siendo la
infección urinaria la más frecuente. Los valores descendidos de Hb o ferritina al
ingresar, así como los valores elevados de ASA, se asociaron a un mayor riesgo de
desarrollo de IN. El número de concentrados de hematíes transfundidos también
se asoció a un aumento del riesgo de infección. No se encontró relación entre la
administración de hierro intravenoso y la IN.
CONCLUSIONES: La hemoglobinemia, la ferropenia y la TSA son factores
asociados al riesgo de IN en fracturas de cadera.

It is necessary to reduce the unnecessary transfusions. This can be achieved
through the appropriate clinical use of blood, avoiding the needs for transfusion
and use of alternatives to transfusion.
The commitment of the health authorities, health care providers and clinicians
are important in prevention, early diagnosis and treatment of
diseases/conditions that could lead to the need for blood transfusion.

Blood transfusion is an essential part of modern health care. Used correctly, it
can save life and improve health. However, as with any therapeutic intervention,
it may result in acute or delayed complications and carries the risk of

"Whenever clinically feasible, patients undergoing elective
surgery with a high risk of severe postoperative anaemia
should have their haemoglobin level and iron status tested,
preferably at least 28 days before the surgical procedure. For
patients >60 yr old, vitamin B12 and folic acid should also be
measured".

Intravenous iron: either 2-5 days preoperatively and/or 2-3
days postoperatively, with a maximum 600 mg.
rHuEPO: single preoperative dose (40,000 IU, sc) was
administered at the orthopedic ward to some patients

Respect to control, very short-term perioperative IV iron
administration, with or without rHuEPO, significantly reduced
(*p<0.01):
No clinically relevant AEs were observed.
The scheduled IV iron dose (200-600 mg) may not cover total iron
loss, especially in patients with preoperative iron deficiency.
Preoperative rHuEPO was only administered in 351 out of 1059
patients presenting with Hb level <13 g/dL and no contraindication.

In conclusion, the use of FCM, compared with oral iron, increases
EPO response, with increased discharge haemoglobin levels, and
prevents the depletion of iron stores induced by EPO.
This could be another benefit of i.v. iron, as depleted iron stores may impair the
correction of postoperative anaemia. In addition, there is accumulating evidence
that iron deficiency per se, independently of anaemia, is associated with fatigue
and muscle weakness. Thus, the correction of iron deficiency, with or without
anaemia, could be effective in improving fatigue and physical performance.

COMENTARIOS FINALES
• Entre un 30% y un 50% de los pacientes quirúrgicos puede presentar una
anemia preoperatoria, causada o no por la patología motivo de la cirugía.
• Hasta un 90% de los pacientes quirúrgicos pueden presentar anemia
postoperatoria debido al sangrado y/o la inhibición de la eritropoyesis.
• En el paciente quirúrgico, la anemia preoperatrioa se asocia con un mayor
riesgo de transfusión sanguínea (TSA). La Hb es el principal factor predictor de
la necesidad de TSA.
• En los pacientes quirúrgicos, la presencia de anemia se correlaciona con un
aumento de la morbi-mortalidad postoperatoria y un descenso de la calidad
de vida.
• Los pacientes quirúrgicos transfundidos presentan una mayor incidencia de
complicaciones (infecciones, trombosis, sobrecarga volumen, reintervención,
reingreso, estancias más largas, recidivas tumorales, etc) y mortalidad.

- Administración pre (peri) operatoria:
Pacientes de cirugía ortopédica programada con anemia
(Hb entre 10 y 13 g/dL) y sangrado moderado-alto
(Grade 1A)
• 600 UI/Kg por semana durante tres semanas antes
(días -21, -14 y -7) y el día de la cirugía.
• 300 UI/Kg diariamente durante 10 días consecutivos antes
de la cirugía, el día de la misma y 4 días posteriores.
• Asegurarnos de que los pacientes no presentan deficiencias
de hierro al comienzo del tratamiento (FeIV) y que reciben
tromboprofilaxis.
¡Se desconoce cual es la dosis mínima eficaz!
