La Infertilidad es un problema de salud importante, observándose un aumento
progresivo de su prevalencia, por diversos factores entre los que destaca la
postergación del primer embarazo.
A nivel internacional, se estima que su prevalencia alcanza a un 10-15 % de las
parejas: 40 a 45% de los casos aproximadamente afecta a mujeres
35% a 40% a hombres
20 a 30%casos, a ambos miembros de la pareja.
En Chile afecta al 10,4% de las mujeres en edad fértil al año de matrimonio; y que a

Objective
To compare TSH and prolactin levels in women with UI and women with a normal fertility evaluation except for an
azoospermic or severely oligospermic male partner.
Design, Setting, and Participants
Cross-sectional study including women evaluated at a large academic health system between 1 January 2000 and 31
December 2012 with normal TSH (levels within the normal range of the assay and ≤5 mIU/L) and normal prolactin
levels (≤20 ng/mL) and either UI (n = 187) or no other cause of infertility other than an azoospermic or severely
oligospermic partner (n = 52).
Results
Women with UI had significantly higher TSH levels than controls [UI: TSH 1.95 mIU/L, interquartile range: (1.54,
2.61); severe male factor: TSH 1.66 mIU/L, interquartile range: (1.25, 2.17); P = 0.003]. This finding remained
significant after we controlled for age, body mass index, and smoking status. Nearly twice as many women with UI
(26.9%) had a TSH ≥2.5 mIU/L compared with controls (13.5%; P < 0.05). Prolactin levels did not differ between the
groups.

BACKGROUND:
Abnormalities of spermatogenesis are associated with numerous diseases and aging. The objective of this study
was to investigate the impact of hypothyroidism on human spermatogenesis and different sperm function tests.
METHODS:
Twenty-five hypothyroid men and 15 normal individuals were investigated. Semen analysis, fructose and acid
phosphatase measurements, teratozoospermia index (TZI), and acridine orange test were determined before and
6-9 months after the initiation of treatment with levothyroxine.
RESULTS:
Morphology is the only sperm parameter that differs significantly between hypothyroid patients and controls (p <
0.0001). After treatment, morphology improved significantly (p < 0.001). Motility was also decreased before
treatment in comparison with controls, and improved after treatment. However, the difference was not significant.
TZI correlated with free thyroxine.
CONCLUSIONS:
Hypothyroidism has an adverse effect on human spermatogenesis. Morphology is the only sperm parameter that is
significantly affected. Motility may also be affected, but further studies regarding this are needed. Screening for
thyroid dysfunction in males who present with a defect in spermatogenesis is strongly recommended, and if
hypothyroidism is noted, the response to thyroid hormone should be evaluated before initiating other treatments.
El hipotiroidismo tiene un efecto adverso sobre la espermatogénesis humana. La
morfología es el único parámetro que se afecta perceptiblemente. La motilidad también
puede verse afectada, pero se necesitan estudios adicionales con respecto a esto.
Se recomienda encarecidamente la detección de disfunción tiroidea en varones que
presenten un defecto en espermatogénesis, y si se observa hipotiroidismo, se debe
evaluar la respuesta a la hormona tiroidea antes de iniciar otros tratamientos

Abstract
In males, congenital adrenal hyperplasia due to 21 hydroxylase deficiency is associated to normal fertility or
infertility caused by a hypogonadotrophic hypogonadism (HH) or gonadal damage caused by intratesticular adrenal
remnants. We report a 29-year-old male with azoospermia, without any important personal or family background.
Physical examination was normal, his height was 150 cm and his testicular volume was 10 ml (normal 15 to 25 ml).
Laboratory showed a normal testosterone and FSH and LH in the low normal limit. These results discarded a HH,
whose diagnostic requirements are a low testosterone and inadequately normal or low gonadotrophins. A testicular
biopsy was informed as compatible with HH. A 21 hydroxylase deficiency was suspected and confirmed with
extremely high levels of 17 hydroxyprogesterone at baseline and after stimulation with fast acting ACTH. Clomiphene
citrate did not increase testosterone or gonatrophin levels. Testicular ultrasound discarded the presence of adrenal
nodules. Betametasone therapy resulted in a normal testicular development, normalization of sperm count,
reduction of 17 hydroxyprogesterone and testosterone levels with an ulterior rise of the latter. Spontaneous
paternity was achieved twice. It must be remembered that in cases of azoospermia due to congenital adrenal
hyperplasia, testosterone produced by adrenal glands hinders the laboratory diagnosis of HH.
