29. ABSTRACT
Objective
Splenectomy is the most effective treatment for idiopathic thrombocytopenic purpura
(ITP) patients who fail to respond to initial medical therapy. We analysed the positive
predictive value of response to intravenous immune globulin (IVIG) in ITP
splenectomized patients.
Subjects and Methods
A chart record analysis of 88 ITP patients treated with splenectomy between 1990 and
2004 in a third level hospital setting was done. We reviewed only ITP patients
previously treated with IVIG. The response to IVIG was evaluated at the third day
after infusion; and response to splenectomy at one, six and twelve months. Patients
were classified as responders (>50 x 109
platelets/L) and non-responders (<50 x 109
platelets/L).

32. Results
A total of 44 patients were included with a median age of
51.6 years (range 18-78) at the time of splenectomy. Ten out of
the 44 patients did not respond to IVIG. Five of the IVIG
responders failed to respond to splenectomy at one and 6
months, and three at 12 months. Sensitivity of IVIG response
as a predictor of the response to splenectomy was 80.5%,
87.8% and 88.6% at one, six and twelve months after
splenectomy, respectively. Specificity was 37.5%, 50% and
62.5% at one, six and twelve months after surgery.
33. Interpretation and conclusions
IVIG adminstration is an independent positive predictive
value of response at six and twelve months after
splenectomy. A good response to IVIG treatment is usually
associated with a positive response to splenectomy, but
even if therapy with IVIG has failed, splenectomy can be
effective.

Enfermedad autoinmune, caracterizada clínicamente por vesículoampollas
y erosiones localizadas en la piel y/o mucosas. Los paciente presentan
autoanticuerpos IgG circulantes en sangre periférica dirigidos contra
diferentes proteínas de los desmosomas, produciendo una rotura de las
uniones intercelulares entre los queratinocitos, y la aparición posterior de
ampollas.
