Este documento resume una investigación sobre la transfusión clínica en España. Presenta datos sobre la alta prevalencia de la transfusión de sangre en el contexto quirúrgico en España, con más del 50% de las transfusiones realizadas en este contexto. También destaca los problemas asociados a la transfusión de sangre alogénica como los altos costes, la escasez de la sangre como recurso, y los riesgos para la salud como errores de identificación o infecciones. Finalmente, enfatiza la importancia de informar a los pacientes

67. Patients and methods: We assessed the requirements for ABT in 156 consecutive patients
undergoing surgery for primary TKR, who received iron ferrous sulphate (256 mg/day; 80
mg of Fe2+
), vitamin C (1000 mg/day) and folic acid (5 mg/day) during the 30-45 days
preceding surgery, and who were transfused if Hb <80 g/L and/or clinical signs/symptoms
of acute anaemia or hypoxemia (Group 2). A previous series of 156 TKR patients serves as a
control group (Group 1).

“For patients undergoing orthopaedic surgery expected to develop
severe postoperative anaemia we currently suggest IV iron
administration during the perioperative period”.
For all other surgeries no evidence-based recommendation can be
made. We strongly recommend that large prospective randomised
controlled trials are undertaken in patients undergoing surgery expected
to develop severe post operative anaemia.

84. Artículo 40. Comités de transfusión.
Las comunidades autónomas, en su ámbito de competencias,
establecerán que en cada centro con práctica transfusional se
constituya un comité de transfusión, para la correcta
utilización de la sangre y sus componentes, cuyas funciones,
como mínimo, serán las siguientes:
85. a) Determinar los estándares y procedimientos de la práctica transfusional
adaptados a las actividades clínicas que se realicen en cada centro
hospitalario.
b) El análisis y la evaluación periódica de la práctica transfusional.
c) El análisis y la evaluación de las reacciones adversas asociadas a la
transfusión.
d) El desarrollo de programas educacionales que fomenten el uso
óptimo de los componentes sanguíneos.
e) El desarrollo e implantación de medidas preventivas y/o correctoras
dirigidas a la seguridad.
f) Cuantas otras funciones considere que puedan ayudar a fomentar
un uso óptimo de la sangre, componentes y derivados, y al ahorro de
su consumo.
