3. GENERALIDADES
POR QUÉ ESTUDIAR ESTE
PROCEDIMIENTO:
ES EL GOLD STANDARD EN MANEJO DE LA HPB
INCREMENTA EN PREVALENCIA DE LUTS
MODERADOS A SEVEROS
MAS PACIENTES ACEPTAN LA CIRUGÍA
OTRAS PATOLOGÍAS NEOPLÁSICAS
COMPLECACIONES:
5. RESECCIÓN TRANSURETRAL
-POSICIÓN DE LITOTOMÍA
-30-90min DEPENDE DE
TAMAÑO Y EXPERIENCIA DEL
CIRUJANO
-EN OCASIONES AMBULATORIO
-IRRIGACIÓN CONTINUA POR
MÁS DE 24 HORAS
8. OTRAS
TÉCNICAS
SE HA DESCRITO EL USO DE ANESTESIA LOCAL:
-Próstatas menores de 40grs
-Otras técnicas de RTU
-Pacientes ambulatorios
Silla de montar vs Espinal vs Caudal
Hipotensión
10. CUANTIFICACIÓN
DELSANGRADO
FÓRMULAS MÁS USADAS EN GENERAL PERO
IMPRECISAS:
a) 15 ml de sangrado por cada gramo de tejido
prostático resecado.
b) b) 2.6 a 4.6 ml de sangrado, por cada minuto de
tiempo de resección.
11. SÍNDROME POST RTUP
ES DEFINIDO COMO EL DESCENSO DE LA CONCENTRACIÓN
PLASMÁTICA DE NA+ DEBAJO DE 125mmol/l junto con manifestaciones
cardiovasculares y neurológicas.
Pueden ocurrir con niveles superiores
0.5-15% de las RTU
Menor con nuevas tecnologías
14. FACTORESDE
RIESGO NULA INCIDENCIA CON BIPOLAR
AÚN NO DE USO MASIVO
NUEVO ESTÁNDAR DE ORO
AUMENTO DE OTRAS COMPLICACIONES
UROLÓGICAS
15. FR ESQUEMA DE IRRIGACIÓN A BAJA
PRESIÓN Y LA SITUACIÓN DEL PACIENTE
DURACIÓN DEL PROCEDIMIENTO (+90MIN)
MANIPULACIÓN DE SINUSOIDES PROSTÁTICOS
ALTA PRESIÓN DE IRRIGACIÓN
19. MANEJOAGUDO
