Objetivo de la doble terapia antiagregante:
Inmediato: estabilización de la placa y prevención
de la trombosis del stent.
A largo plazo: prevención de nuevos eventos.

La mayoría de p con SCASEST van a ICP entre 24-48 h tras
el diagnóstico . ¿Debemos dejarlos ese tiempo sin DAP,
máxime sabiendo que tenemos un fármaco, ticagrelor, que
si podemos usar?

de la PCI en SCASEST para prevenir mionecrosis periprocedimiento
In the ticagrelor group, patients received a 180 mg LD as soon as possible after the diagnosis of NSTE-ACS followed
by 90 mg twice daily as maintenance dose. All patients received their LD at least 4 hours before PCI (13.4 +/- 8.3
hours). In the prasugrel group, patients who underwent PCI received a 60 mg LD as soon as the coronary anatomy
was known and the decision to proceed to PCI taken. They received prasugrel 10 mg daily as maintenance dose.”

• Hypothesis: considering the short median time between the 2 LD (31 minutes) and the observed
(NS) improvement in post-PCI ST-segment resolution (≥70%) it was hypothesized that the effect
of earlier, pre-hospital ticagrelor may not have manifested until after PCI.
• Aim: exploratory analysis, focusing, among the 1.629 patients who actually underwent PCI, on the
data available during the first 24 hours following the procedure, including platelet function
analysis, coronary reperfusion, clinical efficacy and safety outcomes.

Del mundo real tenemos pocos datos:
1. TRASLATE-ACS prasugrel vs clopidogrel en AMI y PCI.
2. US Prometheus prasugrel vs clopidogrel en SCA y stent.
3. Registro SWEDWHEART. Uso ticagrelor en SCA.

Los pacientes tratados con prasugrel no más del 20% o ticagrelor hasta en
un 44 % eran sin embargo, los de menor riesgo.
El 60-80% sigue recibiendo clopidogrel.

Clonclusiones
1. Disponemos de 2 antiagregantes de tercera generación que
debemos usar preferenetemente.
Prasugrel: solo en SCA con ICP. Más restriciones de uso.
Ticagrelor: todo tipo SCA. Disminuye la mortalidad CV y
total.
2. El inicio de la DAP debe ser cuanto antes en la mayoría de los
casos. Prasugrel no en preICP.
3. El Cangrelor tiene establecida una indicación IIb-A en las nuevas
guías. Su uso pre-hospital y frente a antiagregantes potentes no
ha sido validado.
4. La duración de la DA debe ser de 12 m post SCA. Pauta larga:
Ticagrelor probablemente hasta 3 años postIM en pacientes
elegidos. Las guías establecen una pauta corta de solo 3-6 m en
pacientes con alto riesgo de sangrado.

Clonclusiones
5. Tenemos pocos datos y contradictorios sobre DAP con inhibidores
del P2Y12 potentes en vida real. Están infrautilizados y de forma
muy variable entre regiones. Lo peor, se utilizan en pacientes de
bajo riesgo.
6. Los profesionales debemos optimizar la DAP en SCA:
Usar los antiagregantes de tercera generación
preferentemente.
Considerar carácterísticas del paciente y fármaco y el balance
riesgo isquémico/hemorrágico. A largo plazo el isquémico es
más determinante del mal pronóstico.
Manejar protocolos comunes.
