Se recomienda tromboprofilaxis rutinaria usando
Fondaparinux (Grado 1 A), HBPM a dosis de alto riesgo
(Grado 1B); ACO ajustados a un RIN 2,5 (2-3) (Grado 1B);
o HNF (Grado 1B).
En aquellos que la cirugía se retrase, se recomienda iniciar
desde el ingreso hasta la cirugía con HBPM ó HNF (Grado
1C).
14. Enfermedad Tromboembólica Venosa en
Cirugía Fractura Osteoporótica de Cadera
Estudio piloto de la eficacia y seguridad de los inhibidores
directo de la trombina en fractura de cadera
Desde el ingreso administración de HBPM ajustada a peso. Al alta,
administración en régimen ambulatorio de Pradaxa vía oral, ajustada con la
edad y función renal, hasta el mes de la cirugía.
Resto del protocolo ATB, analgesia y ahorro de sangre igual (Feev + EPO +
recuperadores.
Resultados: N: 23 evaluados.
Transfusión: 30% [x: 2,28 CH] (2 violaciones protocolo/2 preIQ)
Trombosis o eventos cardiovasculares: 0/23
Descenso significativo de la Bb directa, GOT, LDH y PCR.
Incremento significativo de fosfatasa alcalina, proteínas y
albúmina. No incremento significativo de: urea, creatinina, GPT, GGT,
bilirrubina total.

“Prolonged thromboprophylaxis with LMWH
significantly reduces the risk of VTE compared
to thromboprophylaxis during hospital admittance
only, without increasing bleeding complications
after major abdominal or pelvic surgery.”
16. Enfermedad Tromboembólica Venosa en
Cirugía Fractura Osteoporótica de Cadera
The thromboprophylaxis regime was continued for 14 days after surgery.
Results: The incidence of symptomatic VTE was 2.2%. 85% of these events
occurred within 5 weeks of the fracture.
The statistically significant risk factors for symptomatic VTE were better
preoperative mobility, living in one’s own home, high mental test score, high
preoperative hemoglobin, inter-trochanteric fractures, and fixation with a
dynamic hip screw.
17. Comentarios
Los pacientes sometidos a intervenciones quirúrgicas
ortopédicas en general presentan un elevado riesgo de
desarrollo de ETEV no sólo durante el proceso quirúrgico,
su estancia hospitalaria, como durante su proceso de
rehabilitación tras el alta y las semanas (mes) posteriores.
Por ello, son subsidiarios de serles aplicados medidas
profilácticas adecuadas a su elevado riesgo, valorando el
binomio de su claro beneficio y frente al riesgo de las
mismas.

by pass coronario o < 12 meses Stent fármaco activo, o SCASEST )
con reserva de plaquetas

We concluded that the transfusion and longer waiting time
for surgery have been associated with the septic complica-
tions in elderly patients treated surgically for hip fracture.

“For patients undergoing orthopaedic surgery expected to develop
severe postoperative anaemia we currently suggest IV iron
administration during the perioperative period”.
For all other surgeries no evidence-based recommendation can be
made. We strongly recommend that large prospective randomised
controlled trials are undertaken in patients undergoing surgery expected
to develop severe post operative anaemia.
