Este caso clínico describe el manejo de una paciente de 43 años que ingresó con un estatus epiléptico refractario. La paciente tenía antecedentes de epilepsia focal con mal control y suspendió su medicación debido a problemas económicos. Requirió intubación, sedación profunda e infusión de múltiples antiepilépticos. Presentó nuevas convulsiones a pesar del tratamiento, pero luego se logró controlar las crisis y extubar con éxito luego de 48 horas de sedación. Se identificó la falta de adherencia al tratamiento como causa

por presentar estatus convulsivo”

por presentar estatus convulsivo”
Enfermedad Actual:
Paciente con antecedente de epilepsia conocida de difícil
control, ingresa en traslado primario por presentar
estatus convulsivo refractario a benzodiacepinas, con
múltiples episodios mioclonicos que generalizan sin
recuperación del estado de conciencia.
Acuden a hospital de Cajica donde indican manejo con
benzodiacepinas 28 mg de midazolam y fenitoina
cantidad no clara sin control de crisis por lo cual deciden
traslado inseguro a esta institución.

• Paciente en malas condiciones generales con alteración de la
conciencia con crisis epilépticas mioclonicas que generalizan
sin recuperación entre crisis con signos vitales:
• PA: 140/90 FC: 70 lpm FR: 20 rpm. SatO2: 92%

rítmicos regulares sin soplos
ni agregados. Ruidos

8. ANALISIS URGENCIAS
• Paciente con cuadro de estatus convulsivo en el momento
refractario a benzodiacepinas por lo cual se indica protección
de la vía aérea mediante secuencia de intubación rápida
Fentanyl 100 mcg bolo
Tiopental 400 mg bolo
Rocuronio 50 mg bolo.
Se indica por crisis mioclonicas que generalizaron
Acido valproico 2 gr IV bolo continuar 500 mg VO por sonda cada 8 horas
Continuar Lamotrigina 200 mg cada 8 horas por sonda
Tiopental a 5 mg minuto para infusión continua.
Analgesia con fentanyl 2 mcg kilo ahora.
Requiere traslado a uci para manejo integral
Intubación orotraqueal
Adaptan a Ventilación mecánica

reactivas a la luz, no se puede realizar resto de examen

requerido intubación orotraqueal en 7 ocasiones, quien consulta por
presentar estatus convulsivo, con requerimiento de infusión de
medicamentos anticonvulsivantes
• Con signos vitales estables
• No ha presentado nuevos episodios convulsivos
Se trasladará a unidad de cuidados intensivos para continuar
manejo

Lamotrigina 100 mg cada 8 horas
Acido valproico 500 mg cada 8 horas
Suspender tiopental una vez se ha asegurado que se le han
administrados los dos anticonvulsivantes previos y así evitar
nuevo episodio convulsivo.
Suspender sedación con midazolam y fentanilo para verificar
evolución neurológica y frecuencias cardiacas hacia la
bradicardia.

Paciente con diagnósticos anotados quien en las horas de la
tarde presenta 4 convulsiones en menos de 2 horas, por lo que
se inicia sedación profunda con midazolam, y se aumenta dosis
de acido valproico a 1 gr c/8 horas.

Paciente con antecedente de epilepsia focal
sintomática, sin nuevos episodios, acoplada a
ventilador, afebril, continúa sedación profunda
de 24-48 horas a Rass -5.

Paciente con indicación de uci por necesidad de soporte ventilatorio invasivo,
vigilancia y monitoria cardiovascular y neurológica estricta y continua.
El día de hoy cumple 48 horas de sedación profunda por lo que se decide
retiro de esta, paciente con soporte vasopresor sin embargo se valorará la
pertinencia de esta con suspensión de sedación

Paciente con apertura ocular espontánea,
orientada en persona y espacio,
desorientada parcialmente en tiempo,
nomina, comprende, repite, sigue
órdenes sencillas, isocoria reactiva, ROT +
+/++++ en 4 extremidades, fuerza de 5/5
en cuatro extremidades, respuesta
plantar neutra bilateral, no signos
meníngeos.
20. Paciente con evolución clínica adecuada, sin deterioro neurológico. Se identifica como
causa de estatus epiléptico la suspensión de la medicación debido a la no entrega por
tramites administrativos. La paciente requiere continuar de forma regular y sin
interrupción:
Lamotrigina 100 mg cada 8 horas
Clonazepam 2 mg en la noche.
Por neurología continuaremos seguimiento ambulatorio.

Se realiza MIPRES de Lamotrigina
Se cierra ic.

Paciente con antecedente de epilepsia focal sintomática quien ingresa por estatus convulsivo, el día de ayer presenta nuevos episodios convulsivos por lo que se ajusta manejo anticonvulsivo aumentando dosis de acido valproico, sin nuevos episodios, se inicia soporte nutricional enteral, reposición de potasio endovenosos, se continúa sedación profunda de
24-48 horas a rass -5. Se continúa manejo médico instaurado, vigilancia clinica.
Se considera paciente se beneficia de valoración por parte de epileptólogo y neurocirugia de manera ambulatoria.

REFRACTARIO A BENZODIAZEPINAS. EN EL SERVICIO DE URGENCIAS DE LA INSTITUCIÓN DECIDEN ASEGURAR VIA AÉREA E INICIO DE
TIOPENTAL. TRASLADAN A UCI. EN EL MOMENTO PACIENTE EN MANEJO CON LAMOTRIGINA 100 MG CADA 8 HORAS, ÁCIDO VALPROICO 1 GR
CADA 8 HORAS Y CLONAZEPAM 2 MG DÍA. PACIENTE HA TENIDO UNA EVOLUCIÓN CLÍNICA ADECUADA, NO HA PRESENTADO NUEVAS CRISIS.

REALIZAN VIDEOTELEMETRIA Y DAN EGRESO CON ORDEN DE CONTROL POR CLÍNICA DE EPILEPSIA.
PACIENTE REFIERE QUE AMBULATORIAMENTE SE ENCONTRABA EN MANEJO CON MALOTRIGINA 300 MG AL DÍA, DOSIS QUE DISMINUYÓ A 100
MG AL DÍA DESDE HACE APROXIMADAMENTE DOS SEMANAS POR NO DISPONER DE MEDICAMENTO DEBIDO A TRÁMITES ADMINISTRATIVOS.
REFIERE CONSUMO DE 5 GOTAS DE CLONAZEPAM OCASIONALES POR PROBLEMAS CON INSOMNIO DE CONCILIACIÓN. REFIERE QUE POR

This definition is conceptual, with two operational dimensions: the first is the length of the seizure and the time point (t1) beyond which the seizure should be regarded as “continuous seizure activity.” The second time point (t2) is the time of ongoing seizure activity after which there is a risk of long-term consequences. In the case of convulsive (tonic–clonic) SE, both time points (t1 at 5 min and t2 at 30 min) are based on animal experiments and clinical research. This evidence is incomplete, and there is furthermore considerable variation, so these time points should be considered as the best estimates currently available. Data are not yet available for other forms of SE, but as knowledge and understanding increase, time points can be defined for specific forms of SE based on scientific evidence and incorporated into the definition, without changing the underlying concepts

niños: crisis febriles convulsivas con las principales en adults, the most common etiologies are acute symptomatic, accounting for approximately half of all cases, followed by remote symptomatic and low antiseizure drug levels in a patient with known epilepsy [10-12]. Examples of some of the more common causes in adults include:
-Acute structural brain injury (eg, stroke, head trauma, subarachnoid hemorrhage, cerebral anoxia or hypoxia), infection (encephalitis, meningitis, abscess), or brain tumor. Stroke is the most common, especially in older patients.
●Remote or longstanding structural brain injury (eg, prior head injury or neurosurgery, perinatal cerebral ischemia, cortical malformations, arteriovenous malformations, and benign brain tumors).
●Antiseizure drug nonadherence or discontinuation in patients with prior epilepsy.
●
Withdrawal syndromes associated with the discontinuation of alcohol, barbiturates, or benzodiazepines.
●
Metabolic abnormalities (eg, hypoglycemia, hepatic encephalopathy, uremia, hyponatremia, hyperglycemia, hypocalcemia, hypomagnesemia) or sepsis. (See &quot;Evaluation and management of the first seizure in adults&quot;, section on &apos;Acute symptomatic seizures&apos;.)

Use of, or overdose with, drugs that lower the seizure threshold (eg, theophylline, imipenem, high dose penicillin G,cefepime, quinolone antibiotics, metronidazole, isoniazid, tricyclic antidepressants, bupropion, lithium, clozapine,flumazenil, cyclosporine, lidocaine, bupivacaine, metrizamide, dalfampridine, and, to a lesser extent, phenothiazines, especially at higher doses).

Excitotoxic cell injury is thought to occur when glutamate binds to the NMDA receptors. In normal circumstances, the channel is blocked by a magnesium ion. However, in GCSE the cells are severely depolarized and the magnesium ions are not available, leaving the channel open. This situation allows calcium and other ions to flow into the cell, causing intracellular accumulation of calcium, with resulting acute necrosis, delayed cell death, or both. The length of the early phase depends on an intact airway, oxygenation and circulation, and the ability of the
brain to compensate for this increased metabolism, which is decreased in patients
with underlying illness.

Multiple induction agents exist, including benzodiazepines, propofol, etomidate, and ketamine.
Propofol is regarded by many experts as the induction agent of choice in SE given its anticonvulsant properties.
However, because it can cause hypotension, it should be avoided in hypotensive patients.
Ketamine, which has antiepileptic properties (discussed later), is another induction agent that can be safely used for intubation, especially in patients who are hypotensive.
Etomidate has long been regarded as a less favorable agent for induction, because of concerns for its lowering of seizure threshold, making it a counterintuitive
choice for patients in SE. However, this fear is extrapolated from EEG data from patients undergoing electroconvulsive therapy (ECT) for psychiatric diagnoses,39 and has not been truly studied in seizing patients.

rocuronium and vecuronium.
Nondepolarizing agents are considered safer because they do not carry the concomitant risk of hyperkalemia, which could be significant in patients who have been seizing and may have developed rhabdomyolysis.
However, another consideration is the duration of their paralytic effect (30 minutes for rocuronium, 45 to 65 minutes for vecuronium) compared with 4 to 6 minutes for succinylcholine.
The paralytic stops the motor activity of the seizure despite ongoing neuronal firing, which can lead to false reassurance regarding seizure control despite continuous neuronal damage. Therefore, if a nondepolarizing paralytic is used, EEG monitoring is indicated to guide management. However, given that an EEG is not immediately available in most settings, a short-acting agent is recommended.40
Although there is a reversal agent for the nondepolarizing agents, sugammadex, its
use is not widespread outside of anesthesia, and most emergency providers are not
comfortable with its use.

Benzodiazepines can cause sedation and muscle relaxation resulting in upper airway obstruction, and in large doses hypotension. These side effects should not deter providers from using the appropriate dosages of benzodiazepines because uncontrolled SE is more likely than benzodiazepines to cause hypoxemia or airway compromise.21 Proper dosing and intubation is preferred to underdosing and
continued seizure activity.
DORMICUM
DORMICUM
Midazolam infusion
Midazolam is the preferred benzodiazepine for continuous infusion, given its short
duration of action and ability to be titrated. Lorazepam is not used as an infusion, given
its longer half-life, which makes weaning from the infusion more difficult. A bolus of
0.2 mg/kg is used, and repeated every 3 to 5 minutes to a maximum of 2 mg/kg until
seizure control is achieved. The patient is then started on an infusion of 0.05 to
2 mg/kg/h. Of note, prolonged administration of midazolam is complicated by tachyphylaxis,
an acute decrease in response, necessitating increasing the dose to maintain
seizure control.53

Diazepam has a longer half-life than lorazepam, but lorazepam has a smaller volume of distribution and active metabolites, leading to a longer duration of action than diazepam, with no significant difference in effectiveness.18 In contrast, midazolam has the shortest duration of action of the 3.22
CONSIDERAR VIAS ALTERNATIVAS: All of these medications can be given through the intravenous (IV) route, and also by the intraosseous route. Obtaining IV access on a patient who is actively seizing can be a difficult, time-consuming, and hazardous procedure, necessitating a faster and safer route of administration. Intramuscular (IM) administration is an attractive alternative, but can only be used for lorazepam and midazolam.
IM midazolam is preferable because it is more water soluble, and thus has a faster onset.23 Diazepam should not be used intramuscularly because it is not water
soluble and this leads to erratic absorption, and can take as long as 90 minutes to take effect.
Another option is using intranasal (IN) midazolam. Using a nasal spray is more convenient than rectal diazepam, and may be more effective.
Most investigators recommend setting a weight-based target dose, and administer the chosen benzodiazepine in increments to effect (Tables 2 and 3). This recommendation does not apply to patients who are seizing because of alcohol withdrawal, because they usually require very large amounts of benzodiazepines and possibly phenobarbital.
Hidantoina
Phenytoins work on sodium channels, slowing their rate of reactivation after depolarization (Table 4). Fosphenytoin is a phenytoin prodrug, metabolized to phenytoin in the body. It is more water soluble, and does not require the propylene glycol and ethanol diluents that are necessary for phenytoin, allowing a faster infusion and fewer side effects.
Increased cost associated with fosphenytoin was an initial concern on introduction to the market, but may be lower than those of phenytoin, now that it is available in generic forms. With the lower rate of adverse effects,32 it is preferred to phenytoin.18
Phenytoin is dosed at 20 to 25 mg/kg and infused at a maximum of 50 mg/min, and even slower (25 mg/min) in geriatric patients and patients with known cardiovascular disease.15 It is important to deliver weight-based amounts and the common practice of giving a gram of phenytoin underdosesmanypatients. Fosphenytoin dosage is calculated in phenytoin equivalents (PE) because it is a prodrug, and dosed at 20 to 25 mg
PE/kg. It can be infused at a maximum rate of 150 mg PE/min, but preferably slower in the elderly and patients at high risk for arrhythmias. Phenytoin cannot be administered intramuscularly, but fosphenytoin can. However, given the long absorption time and delayed onset of action, this method should not be used in SE

event of extravasation there can be localized tissue necrosis with possible resulting
limb loss, named purple glove syndrome.

from inactivation, similar to phenytoin. In addition, valproic acid increases the availability
of GABA by stimulating its synthesis and inhibiting its degradation (see
Table 4). It has been used in Europe for the treatment of SE since 2004, and since
then more than 300 cases have been documented.34 Valproic acid is at least as effective
as phenytoin in terminating SE with a more favorable side effect profile, especially
in cardiorespiratory patients.35,36 A 2014 systematic review showed that IV valproate
was effective in 68.5% of adult patients, which is comparable with, if not superior to,
published data for phenytoin and phenobarbital.20,37,38 Valproic acid is dosed at 20 to
40 mg/kg, and can be infused over 10 minutes, or even faster, at a rate of 10 mg/kg/
min. A repeat dose of another 20 mg/kg can be used, to a maximum of 60 mg/kg (see

The incidence of serious side effects with valproate is low, mostly consistent
with mild hypotension, with good cardiorespiratory tolerability. The most serious side
effect is idiosyncratic encephalopathy, which is rare and may be associated with hepatic
abnormality or hyperammonemia, and is reversible with discontinuation of the

(see Table 3).

Propofol infusion
Propofol can be used as an induction agent for intubation or as an IV infusion to control seizures. Propofol is a sedative and anesthetic that has antiepileptic properties. It works on GABAA receptors, and is a sodium channel blocker, NMDA inhibitor, and calcium channel modulator. It has a short duration of action, which makes it easily titrated. There is evidence that propofol efficiently suppresses seizures. A retrospective study in 2004 showed a 67% success rate in terminating refractory SE in patients on a propofol infusion. The caveat of that study is that the patients also received thiopental, clonazepam, or another anticonvulsant.54 However, most of the evidence comes from an underpowered study that compared propofol with barbiturates, resulting
in comparable seizure control and complications, with a marked increase in mechanicalventilation days in patients on thiopental.55 A bolus of 3 to 5 mg/kg is
used, and is repeated until seizure control is achieved. The patient is then started
on an infusion of 1 to 15 mg/kg/h. Some limitations to prolonged use of high-dose propofol
infusion are hypotension and propofol infusion syndrome; a combination of lactic
acidosis, bradycardia, heart failure, lipemia, rhabdomyolysis, and renal failure, which can be fatal.56 Note that propofol has been reported to cause jerking movements and
induce seizures as well.57

vasopressor or inotrope, it is recommended that these agents be readily available
when initiating the infusion.58 A systematic review in 2002 comparing pentobarbital
with propofol and midazolam infusions found pentobarbital to be superior in effectively
controlling refractory SE with fewer breakthrough seizures but also with an increased
incidence of hypotension.59 Therefore, the convention has been to reserve pentobarbital
for patients refractory to propofol and midazolam. Another cohort in 2012 found
that pentobarbital effectively controlled super-refractory SE in almost 90% of patients,
with infrequent complications.58

Because prolonged seizures are a major physiologic stress to the human body,
several derangements in certain parameters are expected. As the body continues to use anaerobic metabolism during the seizure, lactic acid can accumulate and the patient can have a wide anion gap acidosis. If the seizures are not terminated in a timely fashion, an increase in the serum creatinine kinase levels is also expected, indicating rhabdomyolysis and all of the resultant complications, including renal failure. These
patients should receive supportive care and proper IV hydration during their ED course. It is prudent to optimize the patient’s metabolic status to prevent further neuronal damage, because that would increase the metabolic demand and/or decrease substrate delivery and worsen neurologic outcome (Box 3). Most metabolic abnormalities resolve promptly after the resolution of the convulsive episodes. In addition, there are reports of reversible cerebral edema caused by GCSE that is also selfresolving, but may complicate the clinical picture.
