tratado subterapéutico debuta con FA escala de
BW > 25 en manejo con metimazol.
Valvulopatía mitroaórtica documentada por Eco TT,
con alto riesgo de recaída a FA y alto riesgo
embolico.
Se opta por manejo con control de frecuencia y
ritmo, bloqueo de eje neurohumoral con meta de
frecuencia < 80 LPM e inicio anticoagulación plena
con heparina de bajo peso molecular.

• La glándula tiroides tiene un peso de 15-20 gr en

• Antitiroideos (Carbimazol, metimazol y

• Plasmaferesis reduce T4 y T3 en 36 horas.
• Plasma o Albúmina hacen unión con las
hormonas tiroideas libres, su efecto dura 24-48
horas.
• Colestiramina, entra en el intestino a través de
la recirculación enterohepática.
• Propanolol. 60-80 mg cada 4 h o IV 0.5-1 mg,
seguido de dosis subsecuentes de 2-3 mg por
10-15 min.

• El efecto del Propanolol con dosis altas 160
mg/día.
• Control frecuencia cardiaca.
• Disminuye la demanda de oxigeno del
músculo cardíaco.

fiebre y diaforesis.

• Historia de broncoespasmo o asma. Tratar
con:
Reserpina (2.5-5 mg IM cada 4 h)
Guanetidina (30-40 mg cada 6 h, VO)
• Esmolol ha tenido éxito en el manejo de la
Tormenta tiroidea.
• Dosis de carga de 0.25 – 0.5 mg/kg, seguido
de infusión continua de 0.05-0.1 mg/kg/min

• Hidrocortisona dosis iniciar 300 mg,

mejoría clínica, la Tormenta tiroidea
recurre.
