20. Marcadores
El Colegio Americano de Patólogos (CAP) y la Sociedad Americana de
Colposcopia y Patología Cervical (ASCCP) han establecido una nueva
terminología histopatológica denominada LAST (Lower Anogenital Squamous
Terminology), que incluye los conocimientos actuales sobre la infección VPH,
incorpora el uso de biomarcadores y facilita la comunicación entre profesionales.
La determinación inmunohistoquímica de p16 permite una mejor categorización
de las lesiones “frontera” entre bajo y alto grado, fundamentalmente clasificadas
histológicamente hasta ahora como HSIL/CIN2 mediante criterios exclusivamente
morfológicos.
Las lesiones HSIL/CIN2 p16-positivas se incluyen en la
categoría HSIL dado su mayor riesgo de progresión
Y las HSIL/ CIN2 p16-negativas se re-clasifican como LSIL dado
su comportamiento benigno y su bajo riesgo de progresión.

21. Realizar biopsias dirigidas si existe alto riesgo de lesiones ≥ a HSIL/CIN 2.
Esta recomendación es aplicable a mujeres mayores de 25 años, no
embarazadas y que tengan al menos dos de los siguientes criterios:
• Citología HSIL
• Infección por VPH 16 o 18
• Impresión colposcópica con cambios grado 2.
Datos de una revisión sistemática sobre la estrategia “ver y tratar”
muestran que el 89% de las mujeres con HSIL en la citología tenían una
lesión ≥ a HSIL/CIN 2 en la pieza de escisión.
Este porcentaje es mayor si la impresión colposcópica es de cambios
grado 2 o la prueba VPH es positiva para 16 o 18, por lo que un
tratamiento inmediato es aceptable.

In women with ASCUS/LSIL cytology and a subsequent colposcopy, excluding
CIN 2+, there is an increased risk on CIN2+ during 2 years of follow up. hr-
HPV persistence is the strongest predictor, especially HPV 16. Follow-up
cytology and colposcopy are less strong predictors, while a small number of hr-
HPV negative women are found with CIN 2+ during follow-up. These results
indicate that womenwith ASCUS/LSIL and colposcopy still need follow-up
preferably with hr-HPV genotyping and cytology co-testing

Cytology is the triage strategy associated with the most data on performance
although the HR-HPV positive, cytology negative group presents challenges and
retesting intervals for this group (and choice of re-test) require careful
consideration.
As cytology relies on subjective skills, p16/Ki67 can mitigate disparities, although
a clinician-taken sample is still required.
Comparatively, genotyping and methylation markers are objective and
applicable to self-taken samples, offering logistical advantages including in LMIC.
While viral and cellular methylation markers show promise, more prospective
data are needed
26. DNA METHYLATION MARKERS FOR THE DETECTION OF CIN2/3 AND

performed on hrHPV-transformed cell lines, cervical tissue specimens and self-
samples. Candidate methylation markers were tested by quantitative methylation-
specific PCR (MSP) on cervical scrapes, self-samples and urine.

METHODS:
A total of 14 344 women who were at least 21 years old and had been sexually active,
participated in the study.
RESULTS:
The sensitivity, specificity, positive predictive value and negative predictive value of
folate receptor-mediated epithelium staining for cervical intra-epithelial neoplasia grade
2 and worse was 85.7%, 76.4%, 61.3% and 92.5%, respectively.
CONCLUSION:
Folate receptor-mediated epithelium staining is capable of detecting cervical
precancerous and cancerous lesions rapidly and cost-effectively.
