RESULTADOS
• La elevación de LDLc y de TA tienen efecto independiente, multiplicativo y acumulativo a
largo plazo para provocar eventos cardiovasculares.
• Este estudio cuantifica los beneficios de TENER el colesterol y la TAS más baja, naturalmente, pero NO
cuantifica efecto farmacológico de reducir colesterol y TA, pues obvia los ef. 2ª de los fármacos.

ANNEXA 4: CONCLUSIONES
ClinicalHemostaticEfficacy
• Se consiguió eficacia hemostática en el 79% de los pacientes.
• Son datos preliminares de un estudio en marcha, en el contexto de sangrado mayor
asociado al uso de inhibidores del factor X.
• En este estudio el Andexanet revirtió rápidamente la actividad antifactor X y no se asoció
con efectos adversos serios.
• Se requiere estudio controlado para valorar si los eventos trombóticos ocurridos (18%)
excedieron lo esperado, en pctes de alto riesgo trombótico.

LIMITACIONES
• Este estudio no tiene el poder suficiente para demostrar diferencias en superioridad para
los objetivos de eficacia y seguridad para un IC 95%, siendo amplios y no significativos.
• Basados en el ratio de eventos reportado en este estudio, se deberían haber incluido más de
10.000 pacientes para obtener un adecuado poder de no inferioridad del edoxabán frente al
tratamiento estándar.
• El diseño abierto pudo haber contribuido a un sesgo al reportar resultados, pero todos los
eventos fueron evaluados ciegamente por un comité.

ENSURE-AF: CONCLUSIONES
• El estudio ENSURE-AF es el ensayo clínico randomizado más largo hasta la
fecha en el contexto de CVE de FANV.
• Los porcentajes del objetivo combinado primario de eficacia y de sangrado
mayor o sangrado no mayor clínicamente significativo son similarmente bajos
en ambos grupos de tratamiento, independendientemente de si es con
estrategia de ETE o no.
• El beneficio neto fue numéricamente a favor, pero no estadísticamente
significativo en el brazo del edoxabán, frente al brazo de
enoxaparina/warfarina.
• El edoxabán parece una estrategia alternativa al tratamiento con
enoxaparina/antiVKA para pacientes que van a CVE de FANV permitiendo
CVE para ser realizada siguiendo el inicio de la anticoagulación (≥ 2 horas para
estrategia de ETE, ≥ 3 semanas para estrategia sin ETE.

• AÑADIR CPAP al tratamiento habitual NO reduce eventos CV en pacientes con
SAOS que tenían previa enfermedad coronaria o cerebrovascular.
• El riesgo de ACV puede ser reducido en pacientes que tiene alto nivel de
adherencia.
• CPAP significativamente MEJORÓ objetivos 2ª como:
• Tiempo de alerta (Epwort sleepiness score (p< 0,001).
• Humor: menor ansiedad (p=0,002) y menor depresión (p< 0,001)
• Mejor calidad de vida física (SF 36, p=0,002) y mental (p< 0,001)
• Menos días de baja laboral por sentirse enfermo (p<0,001)
• NO hubo diferencias en eventos adversos serios ni en accidentes.
