months. Prevalence of anaemia at baseline, in incidence during cancer treatment and prevalencie
of absolute and functional iron deficiency prior to chemotehrapy initiation.
N: 295 pacientes y 6 hospitales fueron incluidos en el estudio.
Anemia presente al diagnóstico en el 38,6%, pero fue tratado sólo el 32,6%.
49,4% gastrointestinal, 35,7% pulmón y 27,9% mama.
106 (60,2%) sin anemia al debut desarrollaron anemia durante el tratamiento
Sólo se analizó el metabolismo del hierro a 151 (51,2%):
- 48 (31,8%) presentaban anemia ferropénica: 64,7% acumulada
- 41 (37,3%) presentaban déficit funcional de hierro: 73,3% acumulada

48. TRANSFUSIÓN DEL PACIENTE ONCOLÓGICO
The findings of the 1998 meta-analysis were confirmed, with small variations in some
estimates. Identification of 237 references. Thirty-six studies on 12,127 patients were
included: 23 showed a detrimental effect of PBT; 22 used also multivariable analyses,
and 14 found PBT to be an independent prognostic factor.
Pooled estimates of PBT effect on colorectal cancer recurrence yielded overall
OR of 1.42 (95% CI, 1.20 to 1.67) against transfused patients in randomized
controlled studies.
Stratified meta-analyses confirmed these findings, also when stratifying patients by site
and stage of disease. The PBT effect was observed regardless of timing, type, and in a
dose-related fashion, although heterogeneity was detected. Data on surgical techniques
was not available for further analysis.

Pacientes de cirugía ortopédica programada con anemia
(Hb entre 10 y 13 g/dL) y sangrado moderado-alto
(Grade 1A)
• 600 UI/Kg por semana durante tres semanas antes (días -21, -
14 y -7) y el día de la cirugía.
• 300 UI/Kg diariamente durante 10 días consecutivos antes de la
cirugía, el día de la misma y 4 días posteriores.
• Asegurarnos de que los pacientes no presentan deficiencias de
hierro al comienzo del tratamiento (FeIV) y que reciben
tromboprofilaxis.

Artículo 15. Administración de sangre y componentes.
La administración de sangre y componentes se realizará siempre por prescripción
médica. Siempre que sea posible, el médico que establezca la indicación recabará,
después de explicarle los riesgos y beneficios de esta terapéutica, así como sus
posibles alternativas, la conformidad del paciente, conforme a lo dispuesto en la
Ley 41/2002, de 14 de noviembre, básica reguladora de la autonomía del paciente
y de derechos y obligaciones en materia de información y documentación clínica,
particularmente en sus artículos 5, 8, 9 y 10.

Artículo 18. Pruebas en la sangre del receptor.
1. Antes de la administración de cualquier componente eritro-citario
homólogo, se realizarán pruebas de compatibilidad, excepto en los
casos de requerimiento urgente, entendiendo por tales aquellos en
los que un retraso en el suministro de la sangre o componentes
sanguíneos pueda comprometer la vida del paciente. El médico
responsable del enfermo justificará la urgencia de la transfusión por
escrito.

Artículo 19. Medidas de seguridad.
1. La administración de una transfusión de sangre o de alguno de sus
componentes deberá ir precedida de la comprobación inequívoca,
por parte de la persona que la realiza, de los datos de identificación
del paciente y de los datos de identificación de la unidad de sangre o
componente sanguíneo a él destinado.
